Nou-n\u0103scu\u0163ii la termen au ~75 mg Fe\/kgc provenit \u00een principal de la mam\u0103 \u00een timpul trimestrului al treilea de sarcin\u0103. <!--more-->Depozitele de fier scad \u00een timpul perioadei de cre\u015ftere. Dup\u0103 adolescen\u0163\u0103 nevoia de fier scade, b\u0103rba\u0163ii prezent\u00e2nd o cre\u015ftere gradual\u0103 a depozitelor de fier pe parcursul vietii (au un minim de 50mg Fe\/kgc). \u00cen schimb, femeile prezint\u0103 o pierdere continu\u0103 de fier p\u00e2n\u0103 la menopauz\u0103 (au ~35mg Fe\/kgc); dup\u0103 menopauz\u0103 femeile acumuleaz\u0103 fier, \u00eentr-un mod liniar, ajung\u00e2nd la un nivel asem\u0103n\u0103tor b\u0103rba\u0163ilor.<\/p>\n<hr \/>\n<p style=\"text-align: justify;\">Cea mai mare parte a fierului din organism se g\u0103se\u015fte \u00een <strong>compu\u015fii cu hem<\/strong>, \u00een special hemoglobin\u0103 \u015fi mioglobin\u0103. O cantitate foarte mic\u0103 este con\u0163inut\u0103 \u00een <strong>enzimele care utilizeaz\u0103 fierul \u00een schimbul de electroni<\/strong>: peroxidaze, catalaze \u015fi ribonucleotid reductaze. Din punct de vedere fiziologic, compu\u015fii heminici sunt implicati \u00een procesele oxidative \u015fi respiratorii. Cea mai mare parte a fierului non-hemic este depozitat sub form\u0103 de feritin\u0103 sau hemosiderin\u0103 \u00een macrofage \u015fi hepatocite. Numai o frac\u0163iune foarte mic\u0103 (~0.1%) circul\u0103 \u00een plasm\u0103 sub form\u0103 de Fe3+ legat\u0103 de o protein\u0103 de transport \u2013 transferina.<sup>1<\/sup><\/p>\n<hr \/>\n<p style=\"text-align: justify;\">La subiectul normal, con\u0163inutul total de fier din organism tinde s\u0103 r\u0103m\u00e2na \u00een limite foarte apropiate de valorile normale. Aceasta \u00eenseamn\u0103 c\u0103 pierderile de fier sunt compensate prin absorb\u0163ia fierului din alimenta\u0163ie. Fierul nu se excret\u0103 activ. El se pierde din organism prin descuamarea la nivel gastrointestinal, urinar, cutanat \u015fi prin pierderile menstruale la femeie. Cantitatea medie de fier, care se pierde zilnic \u00een condi\u0163ii normale, la b\u0103rbat \u015fi la femeia far\u0103 menstre este de 1mg\/zi; la femeile care au ciclu menstrual de 2mg\/zi, iar la gravide de aproximativ 3 ori mai mare. \u00cen situa\u0163ii normale, pierderile fiziologice de fier sunt compensate de absorb\u0163ia unei cantit\u0103\u0163i echivalente de fier provenit din diet\u0103; astfel, pentru o pierdere zilnic\u0103 de 1mg se absoarbe \u00een medie o cantitate de fier de 1mg\/zi, reprezent\u00e2nd 5-10% din fierul alimentar.<sup>2<\/sup><\/p>\n<hr \/>\n<p style=\"text-align: justify;\"><span style=\"color: #00539f;\"><strong>Absorb\u0163ia<\/strong><\/span> fierului este un proces adaptabil, \u00een sensul cre\u015fterii \u00een cazul sc\u0103derii depozitelor de fier, respectiv sc\u0103derii \u00een situa\u0163iile de supra\u00eenc\u0103rcare cu fier. Astfel balan\u0163a fierului este unic\u0103 prin faptul c\u0103 se realizeaz\u0103 prin controlul absorb\u0163iei mai degrab\u0103 dec\u00e2t prin controlul excre\u0163iei fierului.3 Fierul este absorbit din intestinul sub\u0163ire proximal sub form\u0103 de fier hemic \u015fi fier feros (Fe2+), absorb\u0163ia sa fiind influen\u0163at\u0103 de aciditatea gastric\u0103 (scade \u00een aclorhidie sau gastrectomie), factori poten\u0163iatori \u015fi inhibitori alimentari. Absorb\u0163ia se realizeaz\u0103 prin intermediul unor proteine transportoare \u015fi cu ajutorul unor enzime: ferireductaza, care converte\u015fte Fe3+ din alimente \u00een Fe2+ pentru transferul \u00een celulele mucoasei intestinale, \u015fi o ferioxidaz\u0103 (hefaestina), care converte\u015fte Fe2+ \u00een Fe3+ la nivelul membranei bazolaterale pentru transferul plasmatic.<sup>1<\/sup><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #00539f;\"><strong>Metabolismul <\/strong><\/span>fierului este dominat de rolul sau \u00een sinteza hemoglobinei. \u00cen acest proces, fierul este utilizat \u00een permanen\u0163\u0103, astfel c\u0103 mi\u015fcarea intern\u0103 a fierului poate fi descris\u0103 ca un ciclu. \u00cen centrul acesui ciclu este compartimentul fierului plasmatic, \u00een care fierul este legat de o protein\u0103 de transport, transferina, care \u00eel furnizeaz\u0103 celulelor care au capacitatea de a sintetiza hemoglobina (eritrobla\u015ftii). Fierul \u00eencorporat \u00een hemoglobina este redat circula\u0163iei prin eritrocitele circulante. Dup\u0103 aproximativ 120 zile eritrocitele sunt captate de macrofage, \u00een principal din splin\u0103, care elibereaz\u0103 fierul din hemoglobin\u0103 \u015fi \u00eel depun sub form\u0103 de feritin\u0103 sau hemosiderin\u0103, dar cea mai mare parte \u00eel redau transferinei, \u00eenchiz\u00e2nd ciclul. O cantitate de aproximativ 30mg fier particip\u0103 la acest ciclu zilnic. O cantitate mic\u0103, suplimentar\u0103 de fier, mai pu\u0163in de 2 mg p\u0103r\u0103sesc plasma zilnic pentru a intra \u00een celulele parenchimatoase hepatice \u015fi alte \u0163esuturi; aici fierul este depozitat sau utilizat pentru sinteza compu\u015filor heminici tisulari (mioglobina, citocromii).<\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #00539f;\"><a style=\"color: #00539f;\" href=\"\/shop\/transferina\/\"><strong>Transferina<\/strong><\/a><\/span>, proteina specific\u0103 ce leag\u0103 fierul din s\u00e2nge, este o beta-1-globulin\u0103 ce fixeaz\u0103 \u00een condi\u0163ii fiziologice 100\u00b5g fier\/dl, ceea ce \u00een mod curent este denumit \u201csideremia\u201d sau \u201cfierul circulant\u201d. Spre deosebire de sideremie, care prezint\u0103 varia\u0163ii diurne mari, nivelul transferinei serice este mult mai constant. Capacitatea total\u0103 a transferinei de fixare a fierului este de 300 \u00b5g fier\/dl (TIBC); astfel, satura\u0163ia transferinei (denumit\u0103 \u015fi \u201ccoeficientul de satura\u0163ie\u201d) este normal \u00een medie de 30%.<sup>2,3<\/sup><\/p>\n<hr \/>\n<p style=\"text-align: justify;\"><a href=\"https:\/\/www.synevo.md\/shop\/transferina\/\" rel=\"nofollow noopener\" target=\"_blank\"><strong><span style=\"color: #000080;\">Afec\u0163iunile caracterizate prin tulbur\u0103ri ale metabolismului fierului sunt:<\/span><\/strong><\/a><br \/>\u2013 prin deficit de fier, caracterizate prin sc\u0103derea cantitatii totale de fier din organism; important pentru diagnostic \u00een toate stadiile deficien\u0163ei de fier este determinarea fertinei serice cu o \u00eenalta sensibilitate \u015fi specificitate;<br \/>\u2013 prin distribu\u0163ie anormal\u0103 a fierului \u00een organism; apar \u00een special \u00een afectiuni cronice (infec\u0163ii, inflama\u0163ii cronice, neoplazii) \u015fi se caracterizeaz\u0103 printr-o relativ\u0103 supra\u00eenc\u0103rcare a depozitelor de fier \u015fi o deficien\u0163\u0103 la nivelul fierului circulant \u015fi func\u0163ional;<br \/>\u2013 prin supra\u00eenc\u0103rcare cu fier (hemoliz\u0103, eritropoeza ineficient\u0103, hemocromatoza primar\u0103, supra\u00eenc\u0103rcarea iatrogen\u0103 cu fier).4<\/p>\n<p style=\"text-align: justify;\"><small>\u00a0<\/small><\/p><div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-1 wp-block-columns-is-layout-flex\"><div
