Aceste complexe se leag\u0103 de receptorii transferinei situa\u0163i pe fa\u0163a extern\u0103 a membranei plasmatice \u015fi sunt internalizate cu ajutorul endozomilor care vor elibera \u00een final fierul\u00a0\u00een citoplasm\u0103<sup>4<\/sup>. Atunci c\u00e2nd o celul\u0103 are nevoie de fier cre\u015fte exprimarea receptorilor pentru a facilita captarea fierului. Deoarece fierul este utilizat \u00een principal\u00a0\u00een sinteza hemoglobinei 80% din receptorii pentru transferin\u0103 se g\u0103sesc pe suprafa\u0163a progenitorilor seriei eritroide<sup>3<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Prin clivajul proteolitic al domeniului extracelular rezult\u0103 o form\u0103 solubil\u0103\u00a0a receptorului transferinei care circul\u0103 liber \u00een plasm\u0103. Concentra\u0163ia seric\u0103 a sTfr constituie un indicator al statusului fierului \u00een organism. Astfel, deficitul de fier induce cre\u015fterea exprim\u0103rii receptorilor transferinei pe suprafa\u0163a celulelor \u015fi indirect a nivelului sTrf, \u00een timp ce supra\u00eenc\u0103rcarea cu fier genereaz\u0103 un efect invers.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">De\u015fi dozarea feritinei serice reprezint\u0103 metoda acceptat\u0103 pentru evaluarea deficitului de fier, totu\u015fi aceasta reprezint\u0103 un reactant de faz\u0103 acut\u0103 care cre\u015fte \u00een condi\u0163ii care nu se coreleaz\u0103 cu depozitele de fier: infec\u0163ii, inflama\u0163ii, boli cronice, procese neoplazice. Dimpotriv\u0103, sTfr nu constituie un reactant de faz\u0103 acut\u0103, iar evaluarea statusului fierului pe baza acestui parametru nu este influen\u0163at\u0103 de condi\u0163iile patologice men\u0163ionate mai sus<sup>4<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\"><strong><em>Recomand\u0103ri pentru determinarea receptorului solubil al transferinei:<\/em><\/strong><\/span><\/p>\n<div style=\"text-align: justify; padding-left: 30px;\"><span style=\"font-size: 12pt;\">\u2022 Diagnosticul diferen\u0163ial al anemiilor microcitare (cre\u015fte \u00een anemiile feriprive, dar nu \u015fi \u00een anemia din bolile cronice);<\/span><\/div>\n<div style=\"text-align: justify; padding-left: 30px;\"><span style=\"font-size: 12pt;\">\u2022 Diagnosticul deficitului de fier la pacien\u0163ii cu boli cronice (ex. poliartrita reumatoid\u0103);<\/span><\/div>\n<div style=\"text-align: justify;
