Acidul metilmalonic este un intermediar \u00een calea de degradare a unor aminoacizi alifatici cu caten\u0103 ramificat\u0103: valina, izoleucina, treonina \u015fi metionina1<\/sup>.\u00a0<\/em><\/strong><\/p>\n

Conversia metilmalonil CoA \u00een succinil CoA necesit\u0103 enzima metilmalonil CoA mutaza \u015fi are drept coenzima 5-deoxiadenozilcobalamina, un derivat sintetizat\u00a0\u00een organism din vitamina B121<\/sup>. Blocajul acestei c\u0103i este urmat de acumularea \u00een organism de acid metilmalonic \u015fi al\u0163i compu\u015fi poten\u0163iali toxici5<\/sup>.\u00a0<\/em><\/strong><\/p>\n

Acidul metilmalonic este un marker specific pentru un grup de tulbur\u0103ri ereditare de metabolism cunoscute sub denumirea de acidemie metilmalonica<\/span>, ce include cel pu\u0163in 8 entit\u0103\u0163i identificate pe baza fenotipului biochimic.<\/p>\n

Inciden\u0163a acestor tulbur\u0103ri \u00een popula\u0163ie este de aproximativ 1:48000 de na\u015fteri4<\/sup>. Aproximativ jum\u0103tate din cazuri se datoreaz\u0103 deficien\u0163ei enzimei metilmalonil CoA mutaza. Transmiterea este autosomal recesiv\u0103. Copiii afecta\u0163i prezint\u0103 acidoz\u0103 metabolic\u0103, hipotonie, letargie, hepatomegalie, afectare renal\u0103 cronic\u0103, deficit intelectual, iar \u00een formele severe com\u0103 \u015fi \u015foc\u00a0\u00een primele s\u0103pt\u0103m\u00e2ni de via\u0163\u01035;6<\/sup>.<\/p>\n

Cauzele dob\u00e2ndite de acidemie metilmalonic\u0103 sunt mai frecvent \u00eent\u00e2lnite. Deficien\u0163a de vitamina B12 (cobalamina), ca urmare a maldigestiei, malabsorb\u0163iei sau a lipsei din diet\u0103 se asociaz\u0103 cu cre\u015fterea nivelului de acid metilmalonic\u00a0\u00een ser \u015fi\u00a0\u00een urin\u0103. Pacien\u0163ii v\u00e2rstnici cu deficien\u0163\u0103 de vitamina B12 pot prezenta neuropatie periferic\u0103, ataxie, pierdere sim\u0163ului pozitiei \u015fi vibra\u0163iei, tulbur\u0103ri de memorie, depresie \u015fi demen\u0163\u0103 \u00een absen\u0163a anemiei4<\/sup>. Unele studii au sugerat c\u0103 acidul metilmalonic \u00een ser este un marker mai relevant \u015fi mai precoce al deficien\u0163ei de cobalamin\u0103 dec\u00e2t determinarea acesteia\u00a0\u00een ser1<\/sup>.<\/p>\n

Recomand\u0103ri pentru determinarea acidului metilmalonic \u00een ser \u015fi urin\u0103<\/em><\/strong> \u2013 evaluarea pacien\u0163ilor cu suspiciune clinic\u0103 de acidemie metilmalonic\u0103 ereditar\u0103; diagnosticul diferen\u0163ial al semnelor \u015fi simptomelor asociate cu deficien\u0163\u0103 de vitamina B12 (cobalamin\u0103)4<\/sup>.<\/p>\n

La pacien\u0163ii pediatrici nivelurile mult crescute sunt sugestive pentru acidemia metilmalonic\u0103. La pacien\u0163ii adul\u0163i nivelurile moderat crescute indic\u0103 deficien\u0163a de vitamina B12 (cobalamina).<\/div>\n
Niveluri moderat crescute mai pot fi \u00eent\u00e2lnite\u00a0\u00een insuficien\u0163a renal\u0103, hipovolemie, exacerbarea florei intestinului sub\u0163ire4<\/sup>.<\/div>\n
