Exist\u0103 dou\u0103 fonduri de carbamilfosfat, unul mitocondrial, ureogenetic \u015fi altul citosolic, pirimidinogenetic. Sinteza <em>de novo<\/em> a pirimidinelor implic\u0103 o serie de 6 reac\u0163ii, \u00eencep\u00e2nd cu formarea carbamilfosfatului \u015fi termin\u00e2nd cu formarea acidului orotidilic (OMP) din acid orotic. OMP va fi convertit la uridinmonofosfat (UMP) \u015fi uridindifosfat (UDP)<sup>5<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Exist\u0103 dou\u0103 mecanisme prin care apare o hiperproduc\u0163ie de acid orotic:<\/p>\n<ul>\n<li style=\"text-align: justify;\">Defectul enzimelor din calea de sintez\u0103 a pirimidinelor (orotat-fosforiboziltransferaza \u015fi orotidin-decarboxilaza). Apare o hiperproduc\u0163ie de acid orotic \u015fi o deficien\u0163\u0103 \u00een nucleotide pirimidinice (aciduria orotic\u0103 ereditar\u0103 \u2013 afec\u0163iune cu transmitere autosomal recesiv\u0103). Hiperproduc\u0163ia de acid orotic este atribuit\u0103 absen\u0163ei feed-back-ului negativ exercitat de nucleotidele pirimidinice asupra primei enzime a c\u0103ii, carbamilfosfatsintetaza. Deficien\u0163a \u00een nucleotide pirimidinice se reflect\u0103 \u00een afectarea diviziunii celulare, rezult\u00e2nd o anemie megaloblastic\u0103, retardul cre\u015fterii \u015fi al dezvolt\u0103rii<sup>2<\/sup>.<\/li>\n<li style=\"text-align: justify;\">Tulbur\u0103ri ereditare ale ciclului ureei (cu excep\u0163ia deficien\u0163ei de carbamilfosfat sintetaz\u0103 \u015fi N-acetil glutamat sintetaz\u0103): deficien\u0163a de ornitin-transcarbamilaza, citrulinemie, acidurie argininsuccinica,\u00a0 defect de transport al aminoacizilor dibazici \u2013 intoleran\u0163\u0103 proteic\u0103 cu lizinurie \u015fi sindromul HHH (hiperornitinemie \u2013 hiperamoniemie \u2013 homocitrulinemie). Hiperproduc\u0163ia de acid orotic se datoreaz\u0103 excesului de carbamilfosfat<sup>4<\/sup><span style=\"font-size: 16px;\">.<\/span><\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><strong><em>Recomand\u0103ri pentru determinarea acidului orotic \u00een urin\u0103<\/em><\/strong><em>\u00a0<\/em><\/p>\n<ul>\n<li style=\"text-align: justify;\">Evaluarea pacien\u0163ilor cu hiperamoniemie, \u00een paralel cu determinarea aminoacizilor plasmatici \u015fi urinari \u015fi a acizilor organici \u00een urin\u0103<sup>2<\/sup>. Copiii cu boli ereditare ale ciclului ureei apar ini\u0163ial normali, dar ulterior vor dezvolta edem cerebral cu letargie, anorexie, hiper\/hipoventilatie, hipotermie, convulsii, com\u0103. La pacien\u0163ii cu deficit enzimatic par\u0163ial, primele manifest\u0103ri apar cu o \u00eent\u00e2rziere de luni\/ani<sup>1<\/sup>.<\/li>\n<li style=\"text-align: justify;\">Depistarea femeilor heterozigote pentru deficien\u0163a de ornitin transcarbamilaz\u0103 (OCT) X-linkata (testul cu allopurinol).
