Transferina este o glicoprotein\u0103 plasmatic\u0103 care transport\u0103 fierul la receptorii celulari specifici. Din punct de vedere structural este un polipeptid ce prezint\u0103 la cap\u0103tul amino-terminal dou\u0103 lan\u0163uri polizaharidice. Aceste dou\u0103 lan\u0163uri se ramific\u0103 form\u00e2nd 2 sau 3 structuri antenare ce au ca reziduu terminal acidul sialic, determin\u00e2nd astfel existen\u0163a a 4 izoforme: pentasialo, tetrasialo, trisialo \u015fi disialo. Izoforma disialo reprezint\u0103, la persoanele normale, mai pu\u0163in de 1%, dar cre\u015fte de 10-15 ori la marii consumatori de alcool, la care mai apare \u015fi o form\u0103 special\u0103, asialo. Frac\u0163ia disialo \u00eempreun\u0103 cu cea asialo formeaz\u0103<\/span> transferina deficitar\u0103 \u00een carbohidrat<\/span>.<\/p>\n

\u00cen compara\u0163ie cu al\u0163i parametrii disponibili pentru diagnosticarea abuzului cronic de alcool CDT s-a dovedit a avea cea mai mare specificitate, fiind un marker important \u00een identificarea \u015fi monitorizarea consumatorilor cronici de alcool1;3<\/sup>.<\/p>\n

Recomand\u0103ri pentru determinarea CDT<\/em><\/strong><\/p>\n

\u2013 screening-ul persoanelor suspectate de abuz de alcool, la care alte teste sunt negative;<\/div>\n
\u2013 identificarea alcoolicilor cronici dintre persoanele traumatizate, pentru a preveni complica\u0163iile\u00a0posttraumatice; \u00a0<\/em><\/strong><\/div>\n
\u2013 identificarea persoanelor cu risc de a dezvolta dependen\u0163\u0103;\u00a0<\/em><\/strong><\/div>\n
\u2013 monitorizarea pe termen lung a rec\u0103derilor la pacien\u0163ii afla\u0163i sub tratament3<\/sup>.\u00a0<\/em><\/strong><\/div>\n

Interpretarea rezultatelor<\/em><\/strong><\/p>\n

Cre\u015fterea CDT (>3 %) reprezint\u0103 un marker pentru abuzul cronic de alcool (specificitate > 90%). Sensibilitatea este 75% pentru b\u0103rba\u0163i \u015fi 50% pentru femei.<\/div>\n
Un episod singular de abuz de alcool nu duce la cre\u015fterea concentra\u0163iei CDT. Cantitatea minim\u0103 de alcool care determin\u0103 cre\u015fterea CDT este de 50-80 g etanol\/zi, cel pu\u0163in 7 zile consecutiv. Dup\u0103 abstinen\u0163\u0103, valorile se men\u0163in ridicate timp de 2-4 s\u0103pt\u0103m\u00e2ni.<\/div>\n
P\u00e2n\u0103 acum nu s-a dovedit c\u0103 ar exista o rela\u0163ie liniar\u0103 \u00eentre valoarea CDT \u015fi cantitatea de alcool consumat\u0103, totu\u015fi este clar c\u0103 valori ridicate ale CDT sunt legate de un consum abuziv de alcool.<\/div>\n
Valori crescute ale CDT se pot g\u0103si \u015fi \u00een hepatita cronic\u0103 activ\u0103, carcinom hepato-celular, ciroza biliar\u0103 primitiv\u0103, sarcin\u0103 \u015fi \u00een sindromul foarte rar de deficit de carbohidrat\u2013glicoprotein\u0103, condi\u0163ionat genetic.<\/div>\n
Valori cuprinse \u00eentre 1.75 \u015fi 3% nu pot indica cu certitudine abuzul de alcool \u015fi necesit\u0103 monitorizare2;3<\/sup>.<\/div>\n
