Proteina transportoare a retinolului (retinol binding protein=RBP) este o glicoprotein\u0103 cu greutate molecular\u0103 mic\u0103 (21 kDa), ce face parte din familia \u03b1 globulinelor \u015fi are rol\u00a0\u00een transportul retinolului de la ficat la \u0163esuturi. RBP leag\u0103 retinolul\u00a0\u00een raport stoichiometric 1:1, solubiliz\u00e2ndu-l \u015fi totodat\u0103 protej\u00e2ndu-l impotriva oxid\u0103rii. In plasm\u0103 complexul retinol \u2013 RBP circul\u0103 legat de transtiretin (prealbumin\u0103). Acest complex elibereaz\u0103 retinolul receptorilor specifici, ce se g\u0103sesc pe suprafa\u0163a retinei, pielii, gonadelor, pl\u0103m\u00e2nilor etc. O frac\u0163iune mic\u0103 (<10%) se filtreaz\u0103 prin membrana glomerular\u0103 \u015fi se reabsoarbe la nivelul tubilor proximali, unde se \u015fi metabolizeaz\u0103. La persoanele s\u0103n\u0103toase RBP se g\u0103se\u015fte\u00a0\u00een urin\u0103 doar \u00eentr-o cantitate redus\u0103, de aceea orice modificare\u00a0\u00een excre\u0163ia urinar\u0103 indic\u0103 o afec\u0163iune tubular\u0103 proximal\u0103. Determinarea concentra\u0163iei de RBP\u00a0\u00een urin\u0103 este un marker important\u00a0\u00een diagnosticul \u015fi monitorizarea bolilor renale4,5<\/sup>.<\/p>\n

Interpretarea rezultatelor <\/em><\/strong><\/p>\n

RBP este un indicator important \u00een monitorizarea \u015fi diagnosticarea afec\u0163iunilor tubulare, nefritelor tubulo-intersti\u0163iale, glomerulopatiilor, vasculopatiilor renale, precum \u015fi \u00een evaluarea leziunilor tubulare induse de intoxica\u0163iile cu metale grele sau de consumul excesiv de medicamente nefrotoxice.<\/div>\n
O cale alternativ\u0103 la determinarea proteinei transportoare a retinolului (RBP) este investigarea alfa1-microglobulinei, deoarece nu exist\u0103 studii care s\u0103 demonstreze superioritatea unui test fa\u0163\u0103 de cel\u0103lalt4<\/sup>.<\/div>\n
