Substan\u0163a este produs\u0103 de c\u0103tre toate celulele nucleate, rata sa de sintez\u0103 fiind constant\u0103 pe tot parcursul vie\u0163ii, chiar\u00a0\u00een prezen\u0163a proceselor infec\u0163ioase, inflamatorii sau neoplazice. De asemenea, produc\u0163ia de cistatin\u0103 nu este influen\u0163at\u0103 de masa muscular\u0103, diet\u0103, medica\u0163ie asociat\u0103, sex \u015fi ras\u0103<sup>3;4;5<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Ca urmare a masei moleculare mici \u015fi a sarcinii electrice pozitive, cistatina C este filtrat\u0103 liber la nivel glomerular \u015fi reabsorbit\u0103 complet \u00een tubii proximali unde este \u015fi metabolizat\u0103. Astfel,\u00a0\u00een absen\u0163a unei leziuni tubulare, cistatina C nu este prezent\u0103\u00a0\u00een urina final\u0103<sup>5<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Datorit\u0103 acestor caracteristici cistatina C reprezint\u0103 un <span style=\"text-decoration: underline;\">marker endogen al ratei filtr\u0103rii glomerulare (RFG)<\/span>, corela\u0163ia fiind invers propor\u0163ional\u0103: valorile crescute indic\u0103 o rata sc\u0103zut\u0103 a filtr\u0103rii glomerulare, \u00eentr-un mod similar creatininei serice<sup>1;3;4<\/sup>. \u00a0<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Conform studiilor efectuate, cistatina C constituie un indicator mai bun al func\u0163iei renale dec\u00e2t creatinina. Astfel, cistatina C are specificitate mai mare dec\u00e2t creatinina \u00een diferen\u0163ierea pacien\u0163ilor cu rata anormal\u0103 a filtr\u0103rii glomerulare (num\u0103rul rezultatelor fals-pozitive este mai mic). De asemenea, la pacien\u0163ii cu insuficien\u0163\u0103 renal\u0103 cronic\u0103 s-a eviden\u0163iat o corela\u0163ie mai str\u00e2ns\u0103 \u00eentre valorile cistatinei C serice \u015fi cea a ratei filtr\u0103rii glomerulare, dec\u00e2t cea dintre creatinina \u015fi RFG, ceea ce permite o mai bun\u0103 monitorizare\u00a0a afec\u0163iunii<sup>3<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Beneficiile test\u0103rii cistatinei C sunt mai mari \u00een special \u00een urm\u0103toarele situa\u0163ii:<\/span><\/p>\n<ul>\n<li style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">rata a filtr\u0103rii glomerulare moderat sc\u0103zut\u0103 (\u00eentre 80 \u015fi 40 mL\/min);<\/span><\/li>\n<li style=\"text-align: justify;\">pacien\u0163i cu insuficien\u0163\u0103 renal\u0103 acut\u0103 care necesit\u0103 administrarea de medicamente cu eliminare glomerular\u0103;<\/li>\n<li style=\"text-align: justify;\">v\u00e2rstnici, copii, gravide cu suspiciune de pre-eclampsie, diabetici, pacien\u0163i cu diverse afec\u0163iuni musculare sau cu transplant renal<sup>2;3<\/sup><span style=\"font-size: 12pt;\">.<\/span><\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Recent, s-a luat \u00een discu\u0163ie posibilitatea ca cistatina C s\u0103 fie un marker prognostic pentru insuficien\u0163a cardiac\u0103 acut\u0103<sup>2<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\"><strong><em>Preg\u0103tire pacient <\/em><\/strong>-preferabil \u00e0 jeun (pe nem\u00e2ncate)<sup>2<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\"><strong><em>Specimen recoltat <\/em><\/strong>-s\u00e2nge venos<sup>2<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\"><strong><em>Recipient de recoltare <\/em><\/strong>-vacutainer f\u0103r\u0103 anticoagulant cu\/f\u0103r\u0103 gel separator<sup>2<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\"><strong><em>Prelucrare necesar\u0103 dup\u0103 recoltare <\/em><\/strong>\u2013 se separ\u0103 serul prin centrifugare<sup>2<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\"><strong><em>Volum prob\u0103<\/em><\/strong> \u2013 minim 0.5 mL ser<sup>2<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\"><strong><em>Cauze de respingere a probei <\/em><\/strong>\u2013 specimen hemolizat<sup>2<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\"><strong><em>Stabilitate prob\u0103 <\/em><\/strong>\u2013 serul separat este stabil <em>7 zile<\/em> la 2-8\u00b0C; <em>timp \u00eendelungat<\/em> la -18\u00b0C<sup>2<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\"><strong><em>Metod\u0103 <\/em><\/strong>- <span style=\"text-decoration: underline;\">nefelometric\u0103<sup>2<\/sup><\/span>.<\/span><\/p>\n<p><span style=\"font-size: 12pt;\"><strong><em>Valori de referin\u0163\u0103<\/em><\/strong><strong><sup>2<\/sup><\/strong><\/span><\/p>\n<p><span style=\"font-size: 12pt;\">1-20 ani: 0.70-1.38 mg\/L;\u00a0\u00a0 20-50 ani: 0.70-1.21 mg\/L;\u00a0\u00a0 &gt;50 ani: 0.84-1.55 mg\/L.<\/span><\/p>\n<p><span style=\"font-size: 12pt;\">Se calculeaz\u0103 de asemenea rata estimat\u0103 a filtr\u0103rii glomerulare dup\u0103 formula CKD-EPI Cystatin C 2012:<br \/>\n<\/span><\/p>\n<p><span style=\"font-size: 12pt;\">133 x min(SCysC\/0.8, 1)<sup> -<\/sup><sup>0.499<\/sup> x max(SCysC\/0.8, 1)<sup> -<\/sup><sup>1.328<\/sup> x 0.996<sup>v\u00e2rst\u0103<\/sup> [x 0.932 la femei], <\/span><span style=\"font-size: 12pt;\">unde SCysC este cistatina C \u00een ser (mg\/l), min este minimul raportului SCysC\/0.8 sau 1 \u015fi max este maximul raportului SCysC\/0.8 sau 1.<\/span><\/p>\n<p><span style=\"font-size: 12pt;\">Valori de referin\u0163\u0103 pentru rata estimat\u0103 a filtr\u0103rii glomerulare conform formulei CKD-EPI Cystatin C 2012: \u2265 60 mL\/min\/1.73 m<sup>2<\/sup>.<br \/>\n<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\"><strong><em>Interpretarea rezultatelor<\/em><\/strong><\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Nivelurile crescute de cistatin\u0103 C indic\u0103 o rat\u0103 sc\u0103zut\u0103 a filtr\u0103rii glomerulare<sup>4<\/sup>.
