Acidul homovanilic (HVA) constituie un metabolit al neurotransmi\u0163\u0103torului dopamin\u0103. HVA prezint\u0103 valori crescute (\u00eempreun\u0103 cu celelalte catecolamine \u015fi metaboli\u0163i ai acestora) la pacien\u0163ii cu tumori secretante de catecolamine (neuroblastom, feocromocitom \u015fi alte tumori derivate din creasta neural\u0103).<\/span><\/p>\n

Nivelurile HVA pot fi modificate \u015fi \u00een cazul unor tulbur\u0103ri \u00een metabolismul catecolaminelor, cum ar fi:<\/span><\/p>\n

\u2013 deficitul de monoaminoxidaz\u0103, \u00een care se pot \u00eenregistra valori sc\u0103zute de HVA urinar;<\/span><\/div>\n
\u2013 deficitul de dopamin-beta-hidroxilaza (enzima care transform\u0103 dopamina \u00een norepinefrin\u0103) caracterizat prin valori crescute de HVA urinar4<\/sup>.<\/span><\/div>\n

Neuroblastomul este cea mai frecvent\u0103 tumor\u0103 solid\u0103 extracranian\u0103 a copilului. Din punct de vedere histopatologic se constat\u0103 proliferarea malign\u0103 a unor celule neuroectodermale slab diferen\u0163iate derivate din creasta neural\u0103. V\u00e2rsta medie la debut este de 2 ani; tabloul clinic este determinat de localizarea \u015fi stadiul tumorii. HVA determinat \u00een urin\u0103 \u00eempreun\u0103 cu acidul vanilmandelic constituie testele ini\u0163iale pentru depistarea neuroblastomului1<\/sup>.<\/span><\/p>\n

Recomand\u0103ri pentru determinarea acidului homovanilic<\/em><\/strong><\/span><\/p>\n

\u2013 screening-ul tumorilor secretante de catecolamine la copii;<\/span><\/div>\n
\u2013 monitorizarea tratamentului la copiii cu neuroblastom;<\/span><\/div>\n
\u2013 evaluarea pacien\u0163ilor suspecta\u0163i ca av\u00e2nd defecte ereditare \u00een metabolismul catecolaminelor3;4<\/sup>.<\/span><\/div>\n

Preg\u0103tire pacient\u00ad <\/em><\/strong>-<\/em> se va evita administrarea de aspirin\u0103, disulfiram, rezerpina \u015fi piridoxin\u0103, cu cel pu\u0163in 48 ore \u00eenaintea recolt\u0103rii. De asemenea pe perioada recolt\u0103rii se vor evita fumatul, stresul fizic \u015fi psihic. Deoarece L-dopa interfer\u0103 cu testarea se va evita\u00a0administrarea acesteia cu 2 s\u0103pt\u0103m\u00e2ni \u00eenaintea recolt\u0103rii3<\/sup>.<\/span><\/p>\n

Specimen recoltat\u00ad <\/em><\/strong>-<\/em> urina din 24 ore: la ora 7 diminea\u0163a pacientul urineaz\u0103 \u015fi nu re\u0163ine aceast\u0103 urin\u0103; apoi colecteaz\u0103 \u00eentr-un vas curat de 2-3 litri toate emisiunile de urin\u0103 p\u00e2n\u0103 la ora 7 diminea\u0163a\u00a0\u00een ziua urm\u0103toare, inclusiv; omogenizeaz\u0103 (prin agitare) urina recoltat\u0103; m\u0103soar\u0103 \u00eentreaga cantitate; re\u0163ine aprox. 10 mL,\u00a0\u00een pahar de plastic de unic\u0103 folosin\u0163\u0103 pentru urin\u0103. Proba se \u0163ine la 2-8\u00baC\u00a0\u00een timpul colect\u0103rii \u015fi ulterior, p\u00e2n\u0103 se lucreaz\u0103 efectiv. La copiii mici determinarea se poate face pe spoturi urinare \u015fi se raporteaz\u0103 la valorile creatininei urinare, deoarece urina din 24 ore este mai dificil de recoltat2<\/sup>.<\/span><\/p>\n

Recipient de recoltare <\/em><\/strong>\u2013 vas de 2-3 litri \u015fi pahar de plastic de unic\u0103 folosin\u0163\u0103 pentru urin\u0103, pe care se noteaz\u0103 cantitatea total\u0103 de urin\u0103 din 24 de ore2<\/sup>.<\/span><\/p>\n

Cantitate recoltat\u0103 <\/em><\/strong>\u2013 10 mL2<\/sup>.<\/span><\/p>\n

Cauze de respingere a probei <\/em><\/strong>\u2013 specimen care nu a fost p\u0103strat la 2-8\u00b0C; utilizarea de substan\u0163e conservante (produc interferen\u0163e) 2<\/sup>.<\/span><\/p>\n

Prelucrare necesar\u0103 dup\u0103 recoltare<\/span> <\/i><\/b><\/span>\u2013 este de preferat s\u0103 se lucreze imediat; dac\u0103 acest lucru nu este posibil, proba se poate stoca la 2-8\u00b0C, dup\u0103 ce \u00een prealabil se adaug\u0103 acid clorhidric concentrat p\u00e2n\u0103 se ajunge la un pH <32<\/sup>.<\/span><\/p>\n

Stabilitate prob\u0103 <\/i><\/b><\/span>- 1 s\u0103pt\u0103m\u00e2n\u0103 la 2-8<\/i>\u00b0C2<\/sup>.<\/span><\/p>\n

Metod\u0103 <\/em><\/strong>- HPLC (cromatografie de lichide sub \u00eenalt\u0103 presiune)<\/span>2<\/sup>.<\/span><\/p>\n

Interpretarea rezultatelor<\/em><\/strong><\/span><\/p>\n

HVA \u015fi\/sau acidul vanilmandelic \u00eenregistreaz\u0103 valori crescute la aproximativ 95% dintre pacien\u0163ii cu neuroblastom4<\/sup>.<\/span><\/div>\n
Valori crescute de HVA pot fi \u00eent\u00e2lnite \u015fi\u00a0\u00een feocrocitom, precum \u015fi\u00a0\u00een alte tumori derivate din creasta neural\u0103. Excre\u0163ia urinar\u0103 crescut\u0103 poate fi intermitent\u01033<\/sup>.<\/span><\/div>\n

Limite \u015fi interferen\u0163e<\/em><\/strong><\/span><\/p>\n

Un procent de 20% din pacien\u0163ii cu neuroblastom nu <\/strong>prezint\u0103 excre\u0163ie urinar\u0103 crescut\u0103 de HVA3<\/sup>.\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 \u00a0<\/span><\/div>\n
