In timpul sarcinii fosfataza alcalin\u0103 poate fi derivat\u0103 din placent\u0103. Anumite tumori maligne pot de asemenea produce o izoenzim\u0103 ALP<sup>5<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Fosfataza alcalin\u0103 de origine osoas\u0103, enzima isoform\u0103 a fosfatazei alcaline, este o glicoprotein\u0103 tetrameric\u0103 care se g\u0103se\u015fte pe suprafa\u0163a osteoblastului \u015fi este eliberat\u0103 \u00een circula\u0163ie prin ac\u0163iunea enzimei fosfatidil-inozitol-glicanaza. Ostaza intervine\u00a0\u00een calcificarea matricei osoase prin \u00a0clivarea pirofosfatului \u2013 un inhibitor al mineralizarii osoase<sup>5<\/sup>. Fosfataza alcalin\u0103 osoas\u0103 este cunoscut\u0103 astfel ca un indicator sensibil \u015fi fidel al metabolismului osos. La ace\u015fti pacien\u0163i se observ\u0103 cre\u015fteri semnificative ale fosfatazei alcaline serice, astfel c\u0103 pot fi \u00eent\u00e2lnite valori de peste 10-25 de ori limita superioar\u0103 a intervalul de referin\u0163\u0103. Exist\u0103 totu\u015fi bolnavi cu forme mai pu\u0163in agresive de boal\u0103 Paget, care au concentra\u0163ii de ALP total la limita superioar\u0103 a intervalului de referin\u0163\u0103 \u015fi la care ostaza reprezint\u0103 un indicator mai sensibil al activit\u0103\u0163ii metabolice osoase<sup>3<\/sup>. De asemenea fosfataza alcalin\u0103 de origine osoas\u0103 scade \u00een cursul tratamentului antiresorbtiv din boala Paget av\u00e2nd astfel utilitate\u00a0\u00een monitorizarea eficien\u0163ei terapiei<sup>4<\/sup>.<\/span><\/li>\n<li><span style=\"font-size: 12pt;\">Cre\u015fteri moderate pot fi observate \u00een osteomalacie, \u00een timp ce \u00een\u00a0rahitism concentra\u0163ia de ALP osoas\u0103 poate dep\u0103\u015fi de 2-4 ori valoarea normal\u0103.<\/span><\/li>\n<li><span style=\"font-size: 12pt;\">Acromegalia determin\u0103 modific\u0103ri ale izoenzimei osoase, prin stimularea concentra\u0163iei de IGF-1, care activeaz\u0103 osteoblastele \u015fi implicit formarea de os.<\/span><\/li>\n<li><span style=\"font-size: 12pt;\">Timp de 3-4 luni dup\u0103 un transplant renal ALP de origine osoas\u0103 poate \u00eenregistra valori ridicate din cauza administr\u0103rii de substan\u0163e imunosupresoare, cum ar fi corticosteroizii sau ciclosporina A<sup>3<\/sup>.<\/span><\/li>\n<li><span style=\"font-size: 12pt;\">Concentra\u0163iile de ostaz\u0103 pot fi monitorizate \u00een dinamica\u00a0\u00een cursul tratamentului antiresorbtiv din osteoporoz\u0103. Mai multe studii au ar\u0103tat faptul c\u0103 un tratament este eficient dac\u0103 se ob\u0163ine o sc\u0103dere a nivelului ostazei cu cel pu\u0163in 25% fa\u0163a de nivelul bazal,\u00a0\u00een decurs de 3-6 luni de la ini\u0163ierea terapiei. Prin monitorizarea tratamentului se pot identifica pacien\u0163ii non-responderi la tratament; pe de alt\u0103 parte demonstrarea precoce a r\u0103spunsului biochimic al osului\u00a0 poate \u00eembun\u0103t\u0103\u0163i \u015fi stimula complian\u0163a pacientului la terapie<sup>4<\/sup>.<\/span><\/li>\n<li>\n<div><span style=\"font-size: 12pt;\">Tratamentul cu hormon de cre\u015ftere al copiilor cu nanism implic\u0103 costuri ridicate, de aceea este nevoie de un marker predictiv, u\u015for de determinat \u015fi care s\u0103 ofere precoce informa\u0163ii asupra r\u0103spunsului la tratament. Studii efectuate \u00een acest domeniu au dus la concluzia c\u0103 ALP de origine osoas\u0103 este un indicator important\u00a0\u00een monitorizarea terapiei cu preparate de hormon de cre\u015ftere. Astfel, se consider\u0103 c\u0103 tratamentul este eficient dac\u0103 se ob\u0163ine o cre\u015ftere semnificativ\u0103 a ostazei fa\u0163\u0103 de nivelul dinaintea tratamentului<sup>3<\/sup>.<\/span><\/div>\n<\/li>\n<li><span style=\"font-size: 12pt;\">In hiperparatiroidismul primar ALP total \u00eenregistreaz\u0103 valori normale sau u\u015for crescute; exist\u0103 \u00eens\u0103 o cre\u015ftere mult mai marcat\u0103 a ostazei care reflect\u0103 mai bine modific\u0103rile sistemului osos.<\/span><\/li>\n<li><span style=\"font-size: 12pt;\">Tratamentele prelungite cu antiepileptice (fenitoin\u0103, fenobarbital, primidona, carbamazepina) pot determina \u00een timp osteomalacie \u015fi concentra\u0163ii ridicate de fosfataz\u0103 alcalin\u0103 de origine osoas\u0103, chiar dac\u0103 uneori nivelul total de ALP este normal.
