Desi sunt dezvoltate mai recent au inceput sa fie utilizate in anumite laboratoare si centre specializate datorita specificitatii lor inalte.<\/p>\n<p>Astfel, reactia de polimerizare in lant poate fi un test util in <strong>diagnosticul sifilisului primar<\/strong> daca este efectuata din probele recoltate de la nivelul leziunii, mai ales in situatiile in care microscopia pe fond intunecat nu este disponibila,<\/p>\n<p>Iar testele serologice sunt negative. Pe de alta parte, utilitatea testului PCR din leziunile cutanate din sifilisul secundar ramane incerta<sup>1<\/sup>.<\/p>\n<p>De asemenea testul PCR efectuat din lichidul cefalo-rahidian prezinta importanta in <strong>diagnosticul\u00a0 <\/strong><strong>neurosifilisului<\/strong>, in special la pacientii cu sindromul imunodeficientei dobandite (SIDA)<sup>2;4<\/sup>.<\/p>\n<p>In ceea ce priveste <strong>diagnosticul sifilisului congenital<\/strong> detectarea ADN-ului poate fi efectuata din:<\/p>\n<ul>\n<li>lichidul amniotic: pentru diagnosticul infectiei fetale testul are o sensibilitate similara inocularii intratesticulare a prelevatelor patologice la iepure (RIT = rabbit infectivity test);<\/li>\n<li>sange si lichid cefalorahiadian la nou-nascut<sup>1;2<\/sup><span style=\"font-size: 16px;\">.<\/span><\/li>\n<\/ul>\n<p><strong><em>Pregatire pacient<\/em><\/strong> \u2013 nu este necesara<sup>2<\/sup>.<\/p>\n<p><strong><em>Specimen recoltat<\/em><\/strong> \u2013 a) produs prelevat din leziune (ulceratie) pe tampon uscat; b) sange venos; c) lichid cefalorahidian (LCR) recoltat prin punctie lombara; d) lichid amniotic<sup>2<\/sup>.<\/p>\n<p><strong><em>Recipient de recoltare<\/em><\/strong>\u00a0\u2013 a) tampon fara mediu de transport; b) vacutainer ce contine EDTA ca anticoagulant; c), d) recipient steril<sup>2<\/sup>.<\/p>\n<p><strong><em>Cantitate recoltata <\/em><\/strong>\u2013 b) cat permite vacuumul; b), c) minimum 2 mL<sup>2<\/sup>.<\/p>\n<p><strong><em>Prelucrare necesara dupa recoltare <\/em><\/strong>\u2013 probele <strong>nu<\/strong> vor fi centrifugate<sup>2<\/sup>.<\/p>\n<p><strong><em>Cauze de respingere a probei<\/em><\/strong> \u2013 b) probe vechi, coagulate, hemolizate; folosirea heparinei ca anticoagulant<sup>2<\/sup>.<\/p>\n<p><strong><em>Stabilitate proba <\/em><\/strong>\u2013 probele de sange, produs din leziune, LCR sau lichid amniotic sunt stabile <em>1 saptamana\u00a0 <\/em>la 2-4\u00b0C <sup>2<\/sup>.<\/p>\n<p><strong><em>Metoda<\/em><\/strong> - <span style=\"text-decoration: underline;\">Real-time PCR:<\/span> reactie de polimerizare in lant cu detectie in timp real a produsului PCR\u00a0 acumulat, prin masurarea fluorescentei emise; tinta ADN este reprezentata de gena <em>polA<\/em><sup>2<\/sup>.<\/p>\n<p><strong><em>Valori de referinta <\/em><\/strong>\u2013 Treponema pallidum \u2013 ADN nedetectabil<sup>2<\/sup>.<\/p>\n<p><strong><em>Limita de detectie<\/em><\/strong> \u2013 1.75 copii\/reactie<sup>2<\/sup>.<\/p>\n<p><strong><em>Interpretarea rezultatelor<\/em><\/strong><\/p>\n<p>Detectia Treponema pallidum-ADN in diverse produse patologice sugereaza un diagnostic de infectie datorata acestei bacterii<sup>3<\/sup>.<\/p>\n<p><strong><em>Limite si interferente<\/em><\/strong><\/p>\n<p>Detectia treponemei pallidum prin PCR este o metoda specifica, dar nu poate distinge intre microorganismele vii si cele moarte.<\/p>\n<p>Un rezultat negativ pentru Treponema pallidum-ADN in LCR sau lichidul amniotic nu exclude sifilisul<sup>1<\/sup>.<\/p>\n<p><span style=\"font-size: small;\">\u00a0<\/span><\/p>\n<p><span style=\"font-size: small;\">\u0411\u0438\u0431\u043b\u0438\u043e\u0433\u0440\u0430\u0444\u0438\u044f<\/span><\/p>\n<p><span style=\"font-size: small;\">1.
