Informatii generale si recomandari<\/em><\/strong><\/p>\n

Inhibitorul activatorului plasminogenului (PAI-1) detine un rol important in procesul fibrinolitic. Astfel, nivelurile plasmatice crescute de PAI-1 sunt responsabile de scaderea activitatii fibrinolitice si se coreleaza cu dezvoltarea trombozelor arteriale sau venoase.<\/p>\n

Gena PAI-1 este situata pe cromozomul 7 si contine 8 introni si 9 exoni. Polimorfismul 4G\/5G (deletia\/insertia guanozinei in pozitia 675 a promotorului genei PAI-1) este un determinant genetic major al\u00a0 nivelurilor de PAI-1. Desi prezenta alelei 4G a fost asociata cu niveluri crescute de PAI-1 studiile publicate au dat rezultate contradictorii in ceea ce priveste incidenta alelei 4G la pacientii cu tromboza venoasa profunda. Unele studii au raportat ca nu exista nici o diferenta in distributia genotipurilor 4G\/5G la pacienti si subiectii de control, altele au demonstrat aceasta asociere6;7;8;10<\/sup>. Pe de alta parte, unii autori au sugerat ca asocierea alelei 4G cu alte defecte trombofilice (cum ar fi factorul V Leiden, mutatia protrombinica, hiperhomocisteinemia, scaderea activitatii proteinei C sau S, niveluri crescute factor VIII) creste riscul de tromboza3<\/sup>.<\/p>\n

In ceea ce priveste patologia sarcinii, potrivit unui studiu, prezenta genotipurilor 4G\/4G sau 4G\/5G este clinic semnificativa pentru dezvoltarea tromboembolismului venos in sarcina. Un alt studiu a aratat ca genotipul 4G\/4G, statusul homozigot pentru mutatia Val34Leu a factorului XIII sau homozigotia\/heterozigotia compusa pentru ambele defecte se asociaza cu pierderile de sarcina din trimestrul I, prin afectarea formarii placentei2<\/sup>.<\/p>\n

Un studiu publicat recent a raportat ca prezenta alelei 4G constituie un factor de risc independent pentru infarctul miocardic la pacienti tineri (\u2264 45 ani), la fel ca fumatul, hipertensiunea arteriala si istoricul familial de afectiuni cardiovasculare4<\/sup>.<\/p>\n

La pacientii cu boala coronariana prezenta genotipului 4G\/4G a fost asociata cu un risc crescut de moarte subita1<\/sup>.<\/p>\n

Mai multe studii nu au reusit sa demonstreze asocierea polimorfismului cu riscul crescut de accident vascular ischemic9<\/sup>.<\/p>\n

Metoda <\/em><\/strong>- Real-time PCR (LightCycler)<\/span>: reactie de polimerizare in lant cu detectie in timp real a produsului PCR\u00a0 acumulat, prin masurarea fluorescentei emise; la sfarsitul ciclurilor de amplificare se efectueaza genotiparea, prin analiza curbei de topire6<\/sup>.<\/p>\n

Interpretarea rezultatelor<\/em><\/strong><\/p>\n

Pentru un pacient, se raporteaza unul din cele 3 rezultate posibile:<\/p>\n

-genotip homozigot 5G\/5G;<\/p>\n

-genotip heterozigot 4G\/5G;<\/p>\n

-genotip homozigot 4G\/4G.<\/p>\n

Homozigotii 4G\/4G\u00a0 (frecventa 11% in populatia europeana) prezinta niveluri PAI-1 cu 25% mai mari fata de homozigotii 5G\/5G6;10<\/sup>. Niveluri de PAI-1 peste limita superioara a normalului sunt intalnite si in cazul genotipului heterozigot 4G\/5G care are o frecventa de 40% in populatia europeana.<\/p>\n

Starea de purtator al alelei 4G este asociata cu adipozitate, hipertensiune arteriala si cu un risc usor crescut de infarct miocardic6<\/sup>.<\/p>\n
