Climatul de iarna favorizeaza infectia cu rotavirusuri, care determina pana la 85% din cazurile de diaree la copiii sub varsta de 3 ani. In sezonul cald, frecventa imbolnavirilor cu rotavirus scade la 40%<sup>2<\/sup>.<\/p>\n<p>Boala este puternic contagioasa si se poate asocia cu simptome respiratorii. Scaunele sunt lichide sau gleroase, cu mucus si sange, precedate frecvent de varsaturi si febra. Afectiunea este autolimitanta (5-8 zile), dar poate avea o evolutie severa la pacientii imunodeprimati. In 5-10% din cazuri se poate instala o intoleranta secundara la lactoza<sup>4;5<\/sup>.<\/p>\n<p>Calea fecal-orala si rolul \u201cmainii murdare\u201d sunt principalele modalitati de transmitere a infectiei. Exista o susceptibilitate particulara dependenta de varsta, in sensul ca numarul receptorilor pentru rotavirus, situati pe enterocit, scade odata cu inaintarea in varsta, ceea ce evolueaza paralel cu scaderea susceptibilitatii la aceasta infectie. Sunt posibile epidemii in colectivitatile aglomerate de sugari si copii de varsta mica<sup>2<\/sup>.<\/p>\n<p>La adulti infectiile cu rotavirus sunt de obicei asimptomatice, cu exceptia pacientilor care au primit transplant de maduva osoasa si a persoanelor varstnice institutionalizate care pot prezenta o evolutie severa<sup>7<\/sup>.<\/p>\n<p>La debut si in faza acuta a bolii particulele de rotavirus sunt excretate in cantitati mari in materiile fecale (10<sup>8<\/sup> particule virale\/g scaun)<sup>3<\/sup> astfel ca este posibila detectarea antigenului cu ajutorul unor teste ce utilizeaza anticorpi specifici legati de o faza solida. Rata rezultatelor pozitive obtinute poate varia in functie de varsta, anotimp, localizare geografica, categoria de pacienti investigata<sup>7<\/sup>.<\/p>\n<p><strong>Adenovirusurile<\/strong> umane (51 serotipuri identificate pana in prezent) pot determina o varietate mare de afectiuni, ce includ infectii respiratorii, oculare si gastrointestinale. Aceasta varietate se explica prin existenta numeroaselor serotipuri si prin tropismul tisular diferit al adenovirusurilor. Adenovirusurile enterice (tipurile 40-41) constituie un agent patogen important al gastroenteritei copilului, fiind implicate in unele epidemii sporadice de diaree survenite in comunitatile de copii dar si in spitale<sup>1;6<\/sup>. Transmiterea infectiei in aceste situatii se face pe cale fecal-orala.<\/p>\n<p>Diareea acuta infantila se manifesta clinic prin scaune apoase, insotite de febra si varsaturi, cu o durata medie de 8-12 zile. Aproximativ 2-5% din cazurile de boala diareica acuta la copii mici sunt cauzate de adenovirusurile 40 si 41.<\/p>\n<p>Au fost raportate uneori si alte serotipuri de adenovirus in diareea infantila, cum ar fi 2, 3, 8 si 31<sup>1<\/sup>.<\/p>\n<p>Detectia directa a antigenului Adenovirus in scaun in faza acuta a bolii diareice cu ajutorul anticorpilor specifici legati de faza solida constituie o metoda de diagnostic rapid<sup>1;3<\/sup>.<\/p>\n<p><strong><em>Recomandari pentru efectuarea antigenului Rotavirus si Adenovirus <\/em><\/strong>\u2013 suspiciune clinica de gastroenterita virala<sup>4<\/sup>.<\/p>\n<p><strong><em>Specimen recoltat <\/em><\/strong>\u2013 materii fecale proaspete, emise spontan<sup>3<\/sup>.<\/p>\n<p><strong><em>Recipient de recoltare <\/em><\/strong>\u2013 recipient steril cu capac pentru fecale, fara mediu de transport<sup>3<\/sup>.<\/p>\n<p><strong><em>Volumul proba <\/em><\/strong>\u2013 2 mL de materii fecale lichide sau aproximativ 1 g de materii fecale semiformate<sup>3<\/sup>.<\/p>\n<p><strong><em>Stabilitatea probei <\/em><\/strong>\u2013 proba de materii fecale este stabila <em>72 ore<\/em> la 2-8\u00b0C<sup>3<\/sup>.<\/p>\n<p><strong><em>Metoda <\/em><\/strong>- <span style=\"text-decoration: underline;\">imunocromatografica<\/span><sup>3<\/sup>.<\/p>\n<p><strong><em>Valori de referinta <\/em><\/strong><\/p>\n<p style=\"padding-left: 30px;\">Antigen Rotavirus in materii fecale negativ.<\/p>\n<p style=\"padding-left: 30px;\">Antigen Adenovirus in materii fecale negativ<sup>3<\/sup>.<\/p>\n<p><strong><em>Limite si interferente <\/em><\/strong><\/p>\n<p>Un rezultat negativ nu exclude infectia. Rezultate fals-negativ pot sa apara in urmatoarele situatii:<\/p>\n<p style=\"padding-left: 30px;\">-recoltare si\/sau transport inadecvat al probei (contaminare cu detergenti sau folosirea de conservanti);<\/p>\n<p style=\"padding-left: 30px;\">-nivel redus al particulelor virale, sub limita de detectie a metodei<sup>6<\/sup>.<\/p>\n<p>Pacientii trebuie investigati in paralel si pentru o posibila etiologie bacteriana a sindromului diareic, prin efectuarea coproculturii<sup>3<\/sup>.<\/p>\n<p><span style=\"font-size: small;\">\u00a0<\/span><\/p>\n<p><span style=\"font-size: small;\">\u0411\u0438\u0431\u043b\u0438\u043e\u0433\u0440\u0430\u0444\u0438\u044f<\/span><\/p>\n<p><span style=\"font-size: small;\">1.
