Tiroxina (T4) este un hormon tiroidian cu efecte asupra metabolismului general, dar reprezint\u0103 \u015fi o component\u0103 fiziologic\u0103 a circuitului de reglare a glandei tiroide. Majoritatea tiroxinei circulante este legat\u0103 de proteine de transport (TBG, prealbumina \u015fi albumina). Restul hormonului circul\u0103 liber sub forma de FT4 (tiroxina liber\u0103), biologic activ\u01031;3<\/sup>. \u00a0\u00a0\u00a0<\/span><\/p>\n

Recomand\u0103ri pentru determinarea FT4<\/em><\/strong><\/span><\/p>\n

Determinarea FT4 este un element important \u00een diagnosticul clinic de rutin\u0103. FT4 se determin\u0103 \u00eempreun\u0103 cu TSH atunci c\u00e2nd se suspecteaz\u0103 afec\u0163iuni tiroidiene. Determinarea FT4 este de asemenea util\u0103 \u00een monitorizarea terapiei de supresie tiroidian\u0103. Determinarea FT4 are avantajul de a fi independent\u0103 de concentra\u0163ia \u015fi propriet\u0103\u0163ile de legare ale proteinelor care transport\u0103 tiroxin\u0103, corel\u00e2ndu-se astfel fidel cu statusul clinic al pacientului4<\/sup>.<\/span><\/p>\n

Preg\u0103tire pacient <\/em><\/strong>- <\/em>\u00e0 jeun (pe nem\u00e2ncate). La pacien\u0163ii hipotiroidieni, dup\u0103 administrarea de tiroxin\u0103, nivelul de tiroxin\u0103 liber\u0103 cre\u015fte, ating\u00e2nd un maxim la 1-6 ore de la ingestie; \u00een consecin\u0163\u0103 proba trebuie prelevat\u0103 exact \u00eenainte de administrarea dozei urm\u0103toare. Dac\u0103 pacientul se afl\u0103 \u00een tratament cu medicamente hipolipemiante care con\u0163in tiroxin\u0103, recoltarea de s\u00e2nge pentru determinare FT4 se va face la 4-6 s\u0103pt\u0103m\u00e2ni dup\u0103 \u00eentreruperea acestuia5<\/sup>.<\/span><\/p>\n
