Ele se formeaz\u0103 continuu ca rezultat al r\u0103spunsului imun. Deoarece sunt rapid distruse\u00a0\u00een circula\u0163ia sanguin\u0103, aceste complexe imune (CIC, complexe imune circulante) nu sunt detectate \u00een ser sau sunt detectate doar \u00een cantit\u0103\u0163i mici. Detec\u0163ia CIC este posibil\u0103 \u00een anumite afec\u0163iuni \u00een care exist\u0103 o concentra\u0163ie crescut\u0103 de complexe imune care dep\u0103\u015fe\u015fte clearance-ul realizat de fagocite. Cre\u015fterea CIC duce la cre\u015fterea agreg\u0103rii trombocitare \u015fi eritrocitare. De asemenea, datorit\u0103 activ\u0103rii sistemului complement se produc reac\u0163ii inflamatorii care determin\u0103 stimularea leucocitelor, activarea macrofagelor cu alter\u0103ri consecutive la nivel tisular.<\/p>\n<p style=\"text-align: justify;\">Sistemul complement\u00a0este un\u00a0sistem enzimatic multifunc\u0163ional\u00a0alc\u0103tuit din proteine \u015fi glicoproteine plasmatice (aproximativ 20-30 de componente) \u015fi are un rol esen\u0163ial \u00een ap\u0103rarea organismului. Complementul joac\u0103 un rol auxiliar \u00een cadrul r\u0103spunului imun umoral \u00eendeplinind dou\u0103 func\u0163ii \u015fi anume:<\/p>\n<p style=\"text-align: justify;\">\u2013 Ajut\u0103 la\u00a0finalizarea ac\u0163iunii anticorpului\u00a0asupra antigenului.<br \/>\n\u2013 Are capacitatea de a\u00a0recunoa\u015fte imunologic nespecific structurile non-self<\/p>\n<p style=\"text-align: justify;\">Aceste dou\u0103 func\u0163ii se exercit\u0103 fie separat, fie integrate \u00een r\u0103spunsul imun umoral.<\/p>\n<p style=\"text-align: justify;\">\u00cen calea clasic\u0103 a activ\u0103rii complementului, complexele antigen-anticorp se leag\u0103 de componentele complementului C1,C4 \u015fi C2 declan\u015f\u00e2nd cascada p\u00e2n\u0103 la C9.<\/p>\n<p style=\"text-align: justify;\">C1\u00a0este un complex trimolecular foarte mare, cu o mas\u0103 molecular\u0103 de aproximativ 700 kilodaltoni. Este alc\u0103tuit din\u00a0<span style=\"text-decoration: underline;\">C1q<\/span>\u00a0care este\u00a0unitatea de recunoa\u015ftere, care va detecta prezen\u0163a complexelor imune membranare sau solubile, <span style=\"text-decoration: underline;\">C1r<\/span> \u0438 <span style=\"text-decoration: underline;\">C1s<\/span>, ambele fiind complexe enzimatice ce\u00a0au denumirea de <em>serinesteraze<\/em>.<br \/>\nC1q\u00a0depisteaz\u0103 prezen\u0163a complexelor imune. C1q se leag\u0103 de complexele imune circulante prin fragmentul Fc al moleculelor imunoglobulinice. C1q este o glicoprotein\u0103 alc\u0103tuit\u0103 din 6 subunit\u0103\u0163i identice care se ansambleaz\u0103 dou\u0103 c\u00e2te dou\u0103 form\u00e2nd trei unit\u0103\u0163i constituite din c\u00e2te 3 lan\u0163uri polipeptidice A, B \u015fi C foarte lungi. \u00cen por\u0163iunea liniar\u0103 a unit\u0103\u0163ilor organizarea se face sub form\u0103 de triplu helix, ap\u0103r\u00e2nd ca o structur\u0103 similar\u0103 moleculei de colagen, \u00een timp ce \u00een por\u0163iunea terminal\u0103 organizarea este sub form\u0103 de por\u0163iuni globuloase direct responsabile de realizarea leg\u0103turilor cu secven\u0163ele activatoare de complement ale anticorpilor (Fc).<\/p>\n<p style=\"text-align: justify;\">Concentra\u0163ii crescute de CIC serice se pot \u00eent\u00e2lni \u00een: glomerulonefrita, boala Crohn, colita ulcerativ\u0103, boli reumatismale sistemice cu complica\u0163ii vasculitice (lupus eritematos sistemic, poliartrita reumatoid\u0103), infec\u0163ii virale, bacteriene sau parazitare ca \u015fi \u00een alergii, afec\u0163iuni cronice ale pielii \u015fi boli neurologice.<\/p>\n<p style=\"text-align: justify;\">De\u015fi detec\u0163ia CIC nu este specific\u0103 pentru o anumit\u0103 afec\u0163iune, testarea CIC poate furniza informa\u0163ii de interes despre patologia, cursul \u015fi prognoza bolii. Prezen\u0163a CIC este un indicator despre ap\u0103rarea imun\u0103 dep\u0103\u015fit\u0103 sau conflictul autoimun, ceea ce conduce la necesitatea efectu\u0103rii de teste complementare \u00een vederea clarific\u0103rii diagnosticului.<\/p>\n<p style=\"text-align: justify;\">\u00cen bolile autoimune, afec\u0163iunile neoplazice sau bacteriene, dozarea CIC este utilizat\u0103 ca marker de activitate a bolii \u015fi pentru monitorizarea tratamentului. De\u015fi exist\u0103 multe metode pentru determinarea CIC (precipitare cu PEG, imunodifuzie radial\u0103, teste celulare), testul de elec\u0163ie pentru determinarea CIC este metoda\u00a0 ELISA care permite determinarea cantitativ\u0103 in vitro a complexelor imune\u00a0care leag\u0103 frac\u0163iunea de complement C1q.<\/p>\n<p style=\"text-align: justify;\">Concentra\u0163ii persistent crescute de CIC\u00a0pot indica o boal\u0103 cronic\u0103 activ\u0103, pe c\u00e2nd normalizarea concentra\u0163iei CIC denot\u0103 succesul tratamentului. De asemenea se pot face corela\u0163ii \u00eentre nivelul CIC \u015fi semnele clinice sau\u00a0rec\u0103deri. WHO recomand\u0103 \u00eentotdeauna repetarea test\u0103rii CIC dup\u0103 c\u00e2teva s\u0103ptam\u00e2ni de la prima testare pentru a evalua persisten\u0163a complexelor imune \u00een circula\u0163ie \u015fi semnifica\u0163ia patologic\u0103. De asemenea, aceast\u0103 testare secundar\u0103 permite\u00a0\u015fi monitorizarea tratamentului.<\/p>\n<p style=\"text-align: justify;\">Conform cu recomand\u0103rile WHO urm\u0103toarele afec\u0163iuni sunt asociate cu prezen\u0163a CIC:<\/p>\n<ul style=\"text-align: justify;\">\n<li>Boli reumatice (lupus eritematos sistemic, boala mixt\u0103 de \u0163esut conjunctiv, poliartrita reumatoid\u0103, sindrom Sjoegren, periarterita nodoas\u0103, sindromul Felty, sindromul Reiter, boala Bechterew)<\/li>\n<li>Infec\u0163ii virale (hepatita B, infec\u0163ia cu CMV, EB, panencefalita sclerozant\u0103 subacut\u0103)<\/li>\n<li>Infec\u0163ii bacteriene (endocardita infec\u0163ioas\u0103, gonoreea diseminat\u0103, sifilisul, infec\u0163ia cu streptococ \u015fi meningococ)<\/li>\n<li>Parazitoze (malaria, schistosomiaza, tripanosomiaza, toxoplasmoza)<\/li>\n<li>Glomerulonefrita, cancere, colita ulcerativ\u0103, boala Crohn, pneumonia intersti\u0163ial\u0103 idiopatic\u0103, fibroza chistic\u0103, scleroza multipl\u0103, purpura trombotic\u0103 trombocitopenica, sindromul Behcet, sindromul CHURG-Strauss, boli cronice hepatice<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><strong><em>Preg\u0103tire pacient <\/em><\/strong><em>- <\/em>\u00e0 jeun (pe nem\u00e2ncate)<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Specimen recoltat <\/em><\/strong><em>- <\/em>\u0432\u0435\u043d\u043e\u0437\u043d\u0430\u044f \u043a\u0440\u043e\u0432\u044c<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Recipient de recoltare <\/em><\/strong><em>- <\/em>vacutainer f\u0103r\u0103 anticoagulant, cu\/f\u0103r\u0103 gel separator<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Prelucrare necesar\u0103 dup\u0103 recoltare <\/em><\/strong><em>- <\/em>se separ\u0103 serul prin centrifugare; se lucreaz\u0103 serul proasp\u0103t; dac\u0103 acest lucru nu este posibil, serul se p\u0103streaz\u0103 la -20\u00b0C<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Volum prob\u0103<\/em><\/strong> \u2013 minim 0.5 mL ser<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Cauze de respingere a probei<\/em><\/strong> \u2013 specimen intens hemolizat<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Stabilitate prob\u0103<\/em><\/strong> - <em>timp \u00eendelungat<\/em> la -20\u00b0C; nu decongela\u0163i\/recongela\u0163i<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Metod\u0103 <\/em><\/strong>- <span style=\"text-decoration: underline;\">ELISA<\/span>.
