Mieloperoxidaza (MPO) este o enzim\u0103 ce se g\u0103se\u015fte \u00een granula\u0163iile primare ale neutrofilelor \u015fi \u00een lizozomii monocitelor. MPO catalizeaz\u0103 conversia peroxidului de hidrogen \u00een hipoclorit \u015fi acid hipocloros; sunt necesare modific\u0103ri postransla\u0163ionale pentru a genera enzima activ\u0103 din leucocite.\u00a0 Anticorpii anti-MPO pot s\u0103 apar\u0103 \u00een c\u00e2teva afec\u0163iuni mediate imun \u015fi ar putea fi implica\u0163i \u00een patogenia inflama\u0163iei vasculare asociate cu poliangeita microscopic\u0103.<\/div>\n

Astfel, pacien\u0163ii cu poliangeit\u0103 microscopic\u0103 dezvolt\u0103 adesea anti-MPO \u015fi pot prezenta azotemie secundar\u0103 glomerulonefritei (forma necrotizant\u0103 pauciimuna). Anti-MPO nu sunt specifici pentru poliangeita microscopic\u0103 fiind detecta\u0163i, de asemenea, la pacien\u0163i cu lupus eritematos sistemic, cu sau f\u0103r\u0103 nefrita lupic\u0103, sindrom Goodpasture sau sindrom Churg-Strauss.<\/div>\n

Nivelul anticorpilor se coreleaz\u0103 cu activitatea bolii, fiind crescut \u00een puseele de activitate \u015fi sc\u0103zut \u00een perioadele de remisiune3<\/sup>.<\/div>\n

Un rezultat pozitiv are o valoare predictiv\u0103 mare pentru poliangeita microscopic\u0103 la pacien\u0163ii f\u0103r\u0103 anticorpi anti-nucleari sau anti-membran\u0103 bazal\u0103 glomerular\u0103.<\/div>\n
Un rezultat negativ reduce semnificativ probabilitatea unei poliangeite microcopice3<\/sup>.<\/div>\n

Limite \u015fi interferen\u0163e<\/em><\/strong><\/p>\n

De\u015fi determin\u0103rile seriate pot fi utilizate pentru monitorizarea activit\u0103\u0163ii bolii \u015fi a r\u0103spunsului la tratament \u00een poliangeita microscopic\u0103, deciziile clinice nu trebuie s\u0103 se bazeze numai pe nivelul anti-MPO3<\/sup>.<\/p>\n
