Trichineloza este o infectie cauzata de Trichinella spiralis<\/em>, un nematod ce paraziteaza in stadiul larvar musculatura unor mamifere salbatice (porc mistret, urs, lup, vulpe, unele rozatoare) sau domestice (porc). Infectia este contractata la om prin ingestia de carne cruda sau insuficient preparata termic (in principal cea de porc), determinand forme clinice cu manifestari variate1;3<\/sup>.<\/p>\n

Intreaga evolutie a parazitului se desfasoara intr-o singura gazda. Carnea contaminata contine parazitul in stadiu larvar, cuprins intr-o celula musculara modificata- \u201ecelula doica\u201d. Larva este eliberata sub actiunea sucului gastric si patrunde in mucoasa intestinului subtire; in urmatoarele 24 de ore larva naparleste de patru ori, transformandu-se in adult. Dupa fecundare, in aproximativ cinci zile de la ingestia carnii infectate, femelele elimina prima generatie de larve tinere. Aceste larve migreaza pe cale sanguina sau limfatica, localizandu-se in final in musculatura scheletica. Pot fi afectate toate grupele musculare, mai ales muschii foarte activi, bine oxigenati: muschii laringelui, limbii, intercostalii, muschii extrinseci ai globului ocular1<\/sup>.<\/p>\n

Severitatea infectiei depinde de numarul de larve ingerate. Dupa o perioada de incubatie de 5-10 zile apar manifestari digestive (varsaturi, diaree apoasa) insotite de febra, care coincid cu faza enterala a bolii. In faza parenterala, ca urmare a migrarii larvelor si cantonarii lor in tesuturi, se produc febra, exanteme, edeme, endocardita si mialgii; acestea din urma pot sa persiste o perioada indelungata2<\/sup>.<\/p>\n

Se descriu urmatoarele forme clinice:<\/p>\n

\u2022 asimptomatice: infectia este recunoscuta epidemiologic, testele serologice sunt pozitive, usoara eozinofilie tranzitorie;<\/p>\n

\u2022 usoare: infectia este recunoscuta epidemiologic, testele serologice sunt pozitive, eozinofilie crescuta,\u00a0 subfebrilitate cu durata <1 saptamana, recuperare rapida;<\/p>\n

\u2022 moderate: mialgie, edeme periorbitale, febra cu durata de pana la 2 saptamani, remisiune in 5-7 saptamani;<\/p>\n

\u2022 severe: febra >39\u00baC mai mult de doua saptamani, hipoalbuminemie <2.5 g\/L,\u00a0 vasculite alergice cu edem si\/sau hemoragie mai ales oculara, complicatii pulmonare, nervoase, cardiace, eozinofilie pana la 90%1<\/sup>.<\/p>\n

Una dintre metodele clasice de diagnostic este punerea in evidenta a larvelor din materialul bioptic. Testele serologice reprezinta, de asemenea, un instrument important pentru diagnostic, fiind necesare si in scop epidemiologic. Una dintre cele mai vechi reactii folosite este reactia de precipitare larvara (RPL), test specific, dar cu sensibilitate redusa. Testele imunoenzimatice de tip ELISA sunt mai rapide si au o sensibilitate mai mare, anticorpii fiind detectabili, cel mai devreme, dupa 10 zile de la infectie (de obicei, dupa 12-14 zile).<\/p>\n

Pozitivitatea testelor persista ani de zile, motiv pentru care este imprecisa diagnosticarea pe baza unui singur test1-3<\/sup>.<\/p>\n

Pregatire pacient<\/em><\/strong> \u2013 testul nu necesita o pregatire prealabila3<\/sup>.<\/p>\n

Specimen recoltat <\/em><\/strong>\u2013 sange venos3<\/sup>.<\/p>\n

Volum proba<\/em><\/strong> \u2013 minim 0.5 mL ser3<\/sup>.<\/p>\n

Cauze de respingere a probei<\/em><\/strong> \u2013 specimen hemolizat, lipemic sau contaminat bacterian3<\/sup>.<\/p>\n

Stabilitate prob\u0103 <\/em><\/strong>\u2013 serul separat este stabil 7 zile<\/em> la 2-4\u00b0C \u015fi 1 lun\u0103 <\/em>la -20\u00b0C3<\/sup>.<\/p>\n

Metoda <\/em><\/strong>- ELISA<\/span>; testul foloseste un antigen excretor-secretor purificat provenit din larva de Trichinella (de la porci infectati)3<\/sup>.<\/p>\n

Valori de referinta<\/em><\/strong>\u2013 test calitativ, dar rezultatul se exprima in unitati de absorbanta (densitate optica OD):<\/p>\n

Limite si interferente<\/em><\/strong><\/p>\n

Rezultatele testului trebuie corelate intotdeauna cu diagnosticul clinic si alte teste de laborator. Un rezultat negativ nu exclude o trichineloza; datorita nivelului scazut de anticorpi in stadiile precoce ale infectiei, testul poate indica rezultate negative sau echivoce. Daca exista o suspiciune clinica, testul trebuie repetat dupa 2-4 saptamani. Un rezultat pozitiv nu exclude influenta altor patogeni3<\/sup>.<\/p>\n
