IgA reprezint\u0103 10-15% din totalul imunoglobulinelor1<\/sup>. IgA sunt anticorpii secretori majori; se g\u0103sesc \u00een lacrimi, saliv\u0103, secre\u0163ii respiratorii, gastrointestinale \u015fi urogenitale. In forma dimeric\u0103 (IgA2<\/sub>) con\u0163in suplimentar 2 lan\u0163uri oligopeptice: unul jonc\u0163ional \u015fi altul secretor (o glicoprotein\u0103). IgA2<\/sub> este rezistent\u0103 la ac\u0163iunea proteolitic\u0103 \u015fi are rol\u00a0\u00een aglutinarea bacteriilor, \u00eempiedic\u00e2nd astfel penetrarea mucoaselor5<\/sup>.<\/p>\n

Concentra\u0163ii crescute de IgA monoclonale apar\u00a0\u00een afec\u0163iuni limfoproliferative,\u00a0\u00een special mielomul multiplu IgA \u015fi limfomul \u201emediteranean\u201d cu localizare intestinal\u0103. Un v\u00e2rf monoclonal >2g\/dL constituie un criteriu major de diagnostic pentru mielomul multiplu. Cre\u015fteri ale IgA pot fi \u00eent\u00e2lnite\u00a0\u00een numeroase afec\u0163iuni ale suprafe\u0163elor mucoase. Sc\u0103deri ale IgA se \u00eenregistreaz\u0103 la pacien\u0163ii cu boala sinopulmonar\u0103 cronic\u0103, ataxie-telengiectazie \u015fi deficit congenital. Unele deficite clinic semnificative de IgA sunt \u00eenso\u0163ite de un deficit concomitent de IgG2<\/sub> \u015fi IgG4<\/sub>.<\/p>\n

Pacien\u0163ii cu deficit congenital de IgA sunt predispu\u015fi la afec\u0163iuni autoimune \u015fi pot dezvolta anticorpi fa\u0163\u0103 de IgA, avand risc de anafilaxie dac\u0103 sunt transfuza\u0163i cu s\u00e2nge ce con\u0163ine IgA1;4<\/sup>.<\/p>\n
