Pierderea renala de clor este proportionala cu aportul din 24h<sup>1<\/sup>.<\/p>\n<p><strong><em>Recomandari pentru determinarea clorului urinar <\/em><\/strong>\u2013 deshidratare; monitorizarea administrarii de solutii perfuzabile in perioada postoperatorie; monitorizarea efectelor regimului hiposodat; evaluarea acidozei metabolice hipercloremice; diagnosticul diferential al alcalozei metabolice<sup>2;3<\/sup>.<\/p>\n<p><strong><em>Specimen\u00a0 recoltat <\/em><\/strong>\u2013 a) urina din 24 ore; la ora 7 dimineata pacientul urineaza si nu retine aceasta urina, apoi colecteaza intr-un vas de plastic toate emisiunile de urina pana la ora 7 dimineata in ziua urmatoare, inclusiv; se omogenizeaza urina recoltata; se masoara intreaga cantitate, se retin 100 mL in pahar de plastic de unica folosinta pentru urina; nu se folosesc conservanti; ambele recipiente trebuie sa fie bine inchise cu capac, ele se pastreaza la 2\u20138<sup>\u00ba<\/sup>C pana in momentul lucrului<sup>2<\/sup>.<\/p>\n<p>b) o proba de urina spontana (preferabil prima urina de dimineata)<sup>2<\/sup>.<\/p>\n<p><strong><em>Recipient de recoltare:<\/em><\/strong> a) vas de plastic de 2-3 L si pahar de plastic pentru urina (unica folosinta), pe care se noteaza cantitatea totala de urina din 24 ore; b) eprubeta pentru urina<sup>2<\/sup>.<\/p>\n<p><strong><em>Cantitatea recoltata:<\/em><\/strong> a) toata urina din 24 ore din care se retine 100 mL; b) 10 mL<sup>2<\/sup>.<\/p>\n<p><strong><em>Prelucrare necesara dupa recoltare <\/em><\/strong>\u2013 nu este necesara<sup>2<\/sup>.<\/p>\n<p><strong><em>Stabilitate proba <\/em><\/strong>- <em>7 zile<\/em> la temperatura camerei, <em>7 zile<\/em> la 2-8\u00b0C si <em>timp indelungat<\/em> la -20\u00b0C<sup>2<\/sup>.<\/p>\n<p><strong><em>Metoda <\/em><\/strong>- <span style=\"text-decoration: underline;\">potentiometrica ISE (ion selective electrode)<\/span><span style=\"text-decoration: underline;\"><sup>2<\/sup><\/span><\/p>\n<p><strong><em>Valori de referinta <\/em><\/strong>\u2013\u00a0 a) 110-250 mmol\/24h; b) 46-168 mmol\/L<sup>2<\/sup><\/p>\n<p><em>Factor de conversie: mmol\/L=mEq\/L<\/em><\/p>\n<p><strong><em>Limita de detectie<\/em><\/strong> \u2013 10 mmol\/L<sup>3<\/sup>.<em>\u00a0<\/em><\/p>\n<p><strong><em>Interpretarea rezultatelor<\/em><\/strong><\/p>\n<table class=\"wp-block-table w-100\" border=\"1\" cellspacing=\"0\" cellpadding=\"0\">\n<tbody>\n<tr>\n<td valign=\"top\" width=\"597\">\n<p style=\"text-align: center;\"><strong><em>Cresteri<\/em><\/strong><\/p>\n<\/td>\n<\/tr>\n<tr>\n<td valign=\"top\" width=\"597\">\n<p style=\"text-align: center;\">\u2022 aport crescut de sare, depletie de potasiu, insuficienta corticosuprarenaliana, sindrom Bartter, nefrite cu pierdere de sare<sup>1<\/sup>.<\/p>\n<\/td>\n<\/tr>\n<tr>\n<td valign=\"top\" width=\"597\">\n<p style=\"text-align: center;\"><strong><em>Scaderi<\/em><\/strong><\/p>\n<\/td>\n<\/tr>\n<tr>\n<td valign=\"top\" width=\"597\">\n<p style=\"text-align: center;\"><em>conditii asociate cu depletie de clor<\/em> <em>care raspund la administrarea de clor<\/em> (Cl- urinar &lt;10 mmol\/L): \u2022 sindromul secretiei inadecvate de ADH, \u2022 varsaturi, \u2022 diaree, \u2022 transpiratii profuze, \u2022 sonda nasogastrica, \u2022 boala Addison, \u2022 abuz de diuretice, \u2022 tumori viloase de colon.<\/p>\n<p style=\"text-align: center;\"><em>conditii asociate cu un exces endogen sau exogen de corticosteroizi<\/em>, <em>care nu raspund la administrarea de clor<\/em> (Cl- urinar &gt;20 mmol\/L): \u2022 sindrom Cushing, \u2022 sindrom Conn, \u2022 retentie postoperatorie de clor<sup>1;2<\/sup>.<\/p>\n<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p><strong><em>Limite si interferente<\/em><\/strong><\/p>\n<p>Determinarea izolata a clorului urinar poate crea confuzii de diagnostic.<\/p>\n<p>\u2022 Alti factori care produc interferente<\/p>\n<p style=\"padding-left: 30px;\"><span style=\"text-decoration: underline;\"><strong>Cresteri<\/strong>:<\/span> administrare excesiva de ser fiziologic, clorura de amoniu, ingestie de halogeni (bromuri).<\/p>\n<p style=\"padding-left: 30px;\"><span style=\"text-decoration: underline;\"><strong>Scaderi:<\/strong><\/span> dieta hiposodata, ingestia de alcaline, tratamentul cu carbenicilina<sup>3<\/sup>.<span id=\"_marker\">\u00a0<\/span><\/p>\n<p><span style=\"font-size: small;\">\u0411\u0438\u0431\u043b\u0438\u043e\u0433\u0440\u0430\u0444\u0438\u044f<\/span><\/p>\n<p><span style=\"font-size: small;\">1.
