{"id":9331,"date":"2020-04-23T04:34:57","date_gmt":"2020-04-23T01:34:57","guid":{"rendered":"https:\/\/www.synevo.ro\/product\/urocultura-cu-antibiograma-dupa-caz\/"},"modified":"2026-02-19T00:22:56","modified_gmt":"2026-02-18T22:22:56","slug":"urocultura-cu-antibiograma-dupa-caz","status":"publish","type":"product","link":"https:\/\/old.synevo.md\/ru\/shop\/urocultura-cu-antibiograma-dupa-caz\/","title":{"rendered":"\u0423\u0440\u043e\u043a\u0443\u043b\u044c\u0442\u0443\u0440\u0430 (\u0410\u043d\u0442\u0438\u0431\u0438\u043e\u0442\u0438\u043a\u043e\u0433\u0440\u0430\u043c\u043c\u0430 \u043f\u043e \u043d\u0435\u043e\u0431\u0445\u043e\u0434\u0438\u043c\u043e\u0441\u0442\u0438)"},"content":{"rendered":"<p><strong><em>\u041e\u0431\u0449\u0430\u044f \u0438\u043d\u0444\u043e\u0440\u043c\u0430\u0446\u0438\u044f<\/em><\/strong><\/p>\n<p><strong>Urocultura depisteaza infectiile de tract urinar (ITU)<\/strong> &#8211; afectiuni inflamatorii produse de diferite microorganisme care ajung la nivelul aparatului urinar, unde se multiplica si determina in timp modificari in functionarea normala a rinichilor si a cailor urinare.<\/p>\n<p>Dupa localizare, ITU pot fi <em>\u043d\u0438\u0436\u043d\u0438\u0435<\/em>, cand sunt afectate uretra si vezica urinara si <em>inalte,<\/em> cand sunt cuprinse si ureterele si rinichii<sup>1,2<\/sup>.<\/p>\n<p>Dupa simptomatologie, ITU pot fi <em>\u0431\u0435\u0441\u0441\u0438\u043c\u043f\u0442\u043e\u043c\u043d\u044b\u0435<\/em>\u00a0 \u2013\u00a0 bacteriurie fara simptomatologie clinica si <em>\u0441\u0438\u043c\u043f\u0442\u043e\u043c\u0430\u0442\u0438\u0447\u0435\u0441\u043a\u0438\u0435<\/em> \u2013 cand apare bacteriurie insotita de reactie inflamatorie (piurie) si manifestari clinice.<\/p>\n<p>In mod normal tractul urinar este steril, exceptand uretra distala, care este colonizata cu bacterii provenite de pe tegumentul perineal, iar la femei si din vulva. Aceste bacterii contamineaza accidental urina vezicala in cazul unei recoltari inadecvate a probei de urina.<strong>\u00a0<\/strong><\/p>\n<p>Raportul pe sexe a incidentei ITU variaza in functie de varsta, astfel: in primul an de viata, ITU sunt mai frecvente la baieti (consecinta a fimozelor si parafimozelor intalnite la aceasta varsta), apoi raportul tinde sa se egalizeze in prima copilarie, dupa care este dominat de sexul feminin, incidenta crescand proportional cu varsta (fiind asociata cu activitatea sexuala si sarcina). Diferenta dintre sexe dispare insa la varstnici, prin cresterea prevalentei la barbati, datorita afectiunilor prostatei.<\/p>\n<p>ITU sunt favorizate de anomaliile structurale si neurologice care interfera cu fluxul urinar si de bolile sistemice (de exemplu diabetul si neoplaziile).<\/p>\n<p>La pacientii varstnici, tarati, imunodeprimati si la copii ITU pot evolua asimptomatic sau nespecific (febra, tulburari digestive, scadere in greutate).<\/p>\n<p>Inca din 1956 E. H. Kass a stabilit ca este semnificativ pentru ITU pragul de 10<sup>5<\/sup> UFC pentru bacilii Gram negativi\/mL urina; pentru genul <em>Staphylococcus<\/em> pragul este 5\u00d710<sup>4<\/sup> UFC\/mL, iar pentru <em>Candida albicans<\/em> 10<sup>4<\/sup> UFC\/mL<sup>1;2<\/sup>.<\/p>\n<p><strong><em>Recoltarea probelor<\/em><\/strong><\/p>\n<p>Rezultatul investigatiilor microbiologice depinde de calitatea recoltarii si a transportului probelor:<\/p>\n<p style=\"padding-left: 30px;\">\u2022 in conditii obisnuite se va preleva prima urina de dimineata sau la cel putin 4 ore de la ultima mictiune; <em>inaintea inceperii tratamentului cu antibiotice<\/em>;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 in situatii de urgenta se poate recolta proba de urina in orice moment \u2013 pe cererea catre laborator se va mentiona daca timpul de la ultima mictiune &lt;4h;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 in cazul urmaririi eficientei tratamentului cu antibiotice, se va repeta recoltarea dupa 48-96 ore;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 inaintea recoltarii probei se va face o toaleta locala riguroasa cu apa si sapun;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 proba va fi recoltata din jetul urinar mijlociu, fara a intrerupe fluxul, intr-un recipient steril; pentru a preveni contaminarea probei, evitati sa atingeti recipientul de zona genitala;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 pentru recoltarea probei la sugari se va utiliza punga pediatrica (conform instructiunilor primite la receptia laboratorului);<\/p>\n<p style=\"padding-left: 30px;\">\u2022 proba va fi transportata cat mai repede la laborator (in 1-2 ore)<sup>3<\/sup>. \u00cen situa\u0163ia \u00een care transportul c\u0103tre laborator nu se poate efectua \u00een dou\u0103 ore, proba \u00ee\u015fi men\u0163ine stabilitatea \u00een condi\u021bii refrigerat (4\u00b0C) maxim 4 ore.<\/p>\n<p style=\"padding-left: 30px;\">\u2022 pentru mai multe detalii cu privire la recoltarea urinei la sugari accesa\u0163i <a href=\"\/ru\/instructiuni-pentru-recoltarea-urinei-la-sugari\/\">AICI<\/a><\/p>\n<p><strong><em>Durata rezultatului: 5 zile lucratoare<\/em><\/strong><\/p>\n<p><strong><em>Metode de lucru<\/em><\/strong><\/p>\n<p>Urocultura poate fi efectuata prin mai multe metode: metoda dilutiilor, metoda ansei calibrate, sistemul Uricult Plus, ultimul fiind utilizat si in cadrul laboratoarelor Synevo (<em>vezi fig.<\/em>).<\/p>\n<p><img decoding=\"async\" class=\"alignnone wp-image-9295 size-full\" src=\"\/wp-content\/uploads\/2009\/08\/urocultura2.jpg\" alt=\"Urocultura proba\" width=\"292\" height=\"210\" \/><img decoding=\"async\" class=\"alignnone size-full wp-image-9295\" src=\"\/wp-content\/uploads\/2009\/08\/urocultura2.jpg\" alt=\"-\" width=\"292\" height=\"210\"><\/p>\n<p><span style=\"font-size: small;\">Fig. 1 Sistemul Uricult Plus<\/span><\/p>\n<p>Sistemul Uricult Plus este alcatuit din 3 medii de cultura ce permit cresterea celor mai frecvent intalniti agenti etiologici ai infectiilor urinare.<\/p>\n<p>Dupa insamantare incubarea se face la 35-37\u00b0C timp de 18-24 de ore. In absenta oricarei date clinice de la medicul trimitator se comunica germenul identificat pana la nivel de specie si cantitatea cresterii, astfel: \u201dBacil \u2026. intre \u2026\u2026.. UFC\/mL.. Rezultatul va fi interpretat in context clinic.<\/li>\n<\/ul>\n<p>Numarul UFC\/mL semnificativ pentru infectia urinara depinde de metoda de recoltare si statusul clinic (<em>vezi tabelul de mai sus<\/em>). Daca medicul considera necesar se va solicita antibiograma dupa o noua recoltare a probei\u201d.<\/p>\n<ul>\n<li>Prezenta a trei sau mai multe specii bacteriene in cantitate mai mica de 100 000 se va comunica: \u2019\u2019 Bacteriurie &lt; 100 000 UFC\/mL (prezente 3 specii bacteriene) \u2013 posibila contaminare. Va rugam sa repetati recoltarea\u201d.<\/li>\n<li>\u00b7 Prezenta a doua tipuri de izolate care depasesc fiecare 10<sup>5 <\/sup>UFC\/mL urina reprezinta cel mai frecvent o contaminare. In aceasta situatie se ia legatura cu clinicianul si se solicita o noua proba recoltata in conditii de igiena corecta. Daca din a doua proba se izoleaza in cantitate semnificativa aceleasi bacterii, se confirma relevanta clinica a izolatelor si se testeaza sensibilitatea celor 2 germeni la antibiotice. Infectia mixta este confirmata doar in aproximativ 10% din cazurile in care pacientii recolteaza probele de urina din jetul mijlociu si la aproximativ 36% din pacientii cateterizati.<\/li>\n<li>Prezenta mai multor germeni, izolati fiecare in cantitate de cca. 10<sup>5 <\/sup>UFC\/mL de urina indica o contaminare grosiera. Raportarea acestor probe ca fiind pozitive si efectuarea antibiogramei (la care din germeni?) este eronata si daunatoare pentru pacient. In acest caz rezultatul se comunica : <em>\u201e<\/em>Bacteriurie &gt; 100 000 UFC\/mL (prezente 3 specii bacteriene) \u2013 posibila contaminare. Va rugam sa repetati recoltarea<em>\u201d.<\/em><\/li>\n<li>\u00b7 In cazul prezentei unei singure specii bacteriene in cantitate \u2265 10<sup>5<\/sup>UFC\/mL urina se continua cu identificarea pana la nivel de specie si se efectueaza antibiograma, iar rezultatul se raporteaza:\u2019\u2019 Bacil \u2026.\u00a0\u00a0\u00a0 &gt; 100 000 UFC\/mL\u201d.<\/li>\n<\/ul>\n<p>In cazul in care medicul\/pacientul solicita simultan cu urocultura si examenul biochimic al urinei medicul de laborator poate efectua comentarii suplimentare pentru clarificarea rezultatelor obtinute (daca nu sunt disponibile alte date clinice):<\/p>\n<p style=\"padding-left: 30px;\">-leucociturie semnificativa la examenul biochimic al urinei + urocultura negativa: \u201eDiscordanta intre examenul biochimic de urina si urocultura. Pacient sub tratament antibiotic sau cu afectiune uro-genitala?\u201d;<\/p>\n<p style=\"padding-left: 30px;\">-examen biochimic normal + urocultura cu &gt; 10<sup>5<\/sup>UFC\/mL: \u201eDiscordanta intre examenul biochimic de urina si urocultura. Posibil debut de infectie urinara sau urina incorect conservata.\u201d;<\/p>\n<p style=\"padding-left: 30px;\">-leucociturie semnificativa la examenul biochimic al urinei + urocultura cu &lt; 10<sup>5<\/sup>UFC\/mL: \u201eDiscordanta intre examenul biochimic de urina si urocultura. Pacient sub tratament antibiotic? Recoltarea probei s-a efectuat corect?\u201d<sup> 2;3<\/sup>.<\/p>\n<p><strong><em>Bacterii patogene in urocultura<\/em><\/strong><\/p>\n<p>Germenii cel mai frecvent intalniti in etiologia ITU sunt:<\/p>\n<ul>\n<li>bacili Gram negativi glucozo-fermentativi (<em>Escherichia coli, Klebsiella, Proteus, Enterobacter, Citrobacter, Morganella, Providencia<\/em>);\n<ul>\n<li>bacili Gram negativi glucozo-nefermentativi (<em>Pseudomonas<\/em>, <em>Acinetobacter<\/em>);<\/li>\n<\/ul>\n<\/li>\n<li>coci Gram pozitivi (<em>Enterococcus spp., Staphylococcus aureus, Staphylococcus saprophyticus, Streptococcul hemolitic grup B<\/em>).<\/li>\n<li><em>Corynebacterium urealyticum<\/em> \u2013 cauza ITU la pacientii cu tratamente antibiotice prelungite sau interventii urologice.<\/li>\n<\/ul>\n<p><em>Candida albicans<\/em> determina uretrocistite la pacientii diabeticii cu un control metabolic suboptimal<sup>1,2<\/sup>.<em>\u00a0<\/em><\/p>\n<p>Un diagnostic microbiologic corect necesita informatii clinice despre pacient, acestea putand fi oferite de catre medical curant prin intermediul fisei de microbiologie, care trebuie atent si corect completata cu datele pacientului (varsta, sex, sarcina, tratament cu antibiotice, patologie urinara asociata).<\/p>\n<p>Mentiune: In laboratorul Synevo Bucuresti este disponibil, pentru cazurile de urgenta, un sistem automat de identificare bacteriana si efectuare a antibiogramei, cu eliberarea rezultatului in 24 ore de la izolarea germenului in cultura pura<sup>3<\/sup>.<\/p>\n<p><span style=\"font-size: small;\">\u00a0<\/span><\/p>\n<p><span style=\"font-size: small;\">\u0411\u0438\u0431\u043b\u0438\u043e\u0433\u0440\u0430\u0444\u0438\u044f<\/span><\/p>\n<p><span style=\"font-size: small;\">1.
