\u041e\u0431\u0449\u0430\u044f \u0438\u043d\u0444\u043e\u0440\u043c\u0430\u0446\u0438\u044f<\/em><\/strong><\/p>\n

Tiroglobulina (TG) este o glicoprotein\u0103 dimeric\u0103, servind ca matrice pentru sinteza hormonilor tiroidieni. Reprezint\u0103 componenta proteic\u0103 cea mai important\u0103 a tiroidei, fiind sintetizat\u0103 \u00een cantit\u0103\u0163i mari de c\u0103tre tirocite \u015fi stocat\u0103\u00a0\u00een lumenul folicular1<\/sup>.<\/p>\n

Fiecare macromolecul\u0103 de tiroglobulin\u0103 con\u0163ine 25 de molecule de tirozin\u0103, principalul substrat al sintezei hormonilor tiroidieni. Biosinteza hormonilor tiroidieni se realizeaz\u0103 \u00een patru etape:<\/p>\n

1) \u00a0captarea \u015fi concentrarea iodurilor \u00een tiroid\u0103;<\/p>\n

2)\u00a0 oxidarea iodurilor cu formare de iod elementar;<\/p>\n

3) \u00eencorporarea iodului molecular\u00a0\u00een tiroglobulin\u0103 cu formare rapid\u0103 de 3-monoiodotirozin\u0103 (MIT) \u015fi 3,5-diiodotirozin\u0103 (DIT); prin condensarea unui radical MIT \u015fi un radical DIT se formeaza T3<\/sub>, iar prin condensarea a doi radicali DIT se formeaz\u0103 T4<\/sub>;<\/p>\n

4)\u00a0proteoliza tiroglobulinei \u2013 etapa final\u0103, stimulat\u0103 de TSH, cu eliberarea \u00een circula\u0163ie a hormonilor T3<\/sub> si T4<\/sub>1;2<\/sup>.<\/sub><\/p>\n

\u00cen cursul sintezei tiroglobulinei \u00een tirocite \u015fi transportului\u00a0\u00een foliculii tiroidieni se elibereaz\u0103\u00a0\u00een circula\u0163ie cantit\u0103\u0163i mici de protein\u0103. Astfel\u00a0\u00een serul indivizilor s\u0103n\u0103to\u015fi sunt detectate concentra\u0163ii sc\u0103zute de tiroglobulin\u0103, indic\u00e2nd prezen\u0163a \u0163esutului tiroidian. Pe de alt\u0103 parte, leziuni ale peretelui folicular se \u00eenso\u0163esc de eliberarea\u00a0\u00een torentul circulator a unei cantit\u0103\u0163i mai mari de tiroglobulin\u0103; de aceea proteina reprezint\u0103 un marker al integrit\u0103\u0163ii morfologice a glandei tiroide3<\/sup>.<\/p>\n

Tiroglobulina prezint\u0103 valori crescute \u00een trei tipuri de afec\u0163iuni tiroidiene: gu\u015f\u0103 cu hiperfunc\u0163ie tiroidian\u0103, inflama\u0163ia sau leziunea traumatic\u0103 a tiroidei \u015fi cancerul tiroidian diferen\u0163iat.<\/p>\n

Absen\u0163a tiroglobulinei din serul nou-n\u0103scu\u0163ilor indic\u0103 atireoza congenital\u01034<\/sup>.<\/p>\n

Recomand\u0103ri pentru determinarea tiroglobulinei<\/em><\/strong><\/p>\n

\u2022 monitorizarea pacien\u0163ilor cu cancer tiroidian diferen\u0163iat dup\u0103 tiroidectomie total\u0103 \u015fi tratament cu iod radioactiv.<\/p>\n

La pacien\u0163ii cu cancer tiroidian, f\u0103r\u0103 \u0163esut tiroidan restant\u00a0postoperator sau postiradiere, nivelurile de tiroglobulin\u0103 trebuie s\u0103 fie nedetectabile. Astfel tiroglobulina este un marker tumoral util \u00een evaluarea prezen\u0163ei carcinomului papilar-folicular tiroidian rezidual dup\u0103 rezec\u0163ia chirurgical\u0103 \u015fi\/sau iradiere4<\/sup>.<\/p>\n

Dac\u0103 \u00een timpul perioadei de monitorizare \u015fi terapiei supresive cu tiroxin\u0103 se ob\u0163in valori ale tiroglobulinei >3 \u03bcg\/L, acestea reprezint\u0103 un indicator al recuren\u0163ei tumorale \u015fi se recomand\u0103 efectuarea scintigrafiei corporale totale cu iod131 <\/sup>. \u00a0<\/sup><\/em><\/p>\n

\u2022\u00a0\u00a0tiroidita destructiv\u0103;<\/div>\n
\u2022 diagnosticul diferen\u0163ial \u00eentre tiroidita subacut\u0103 \u015fi tirotoxicoza artificial\u0103 (provocat\u0103) \u2013\u00a0\u00een tirotoxicoza artificial\u0103 valorile tiroglobulinei sunt foarte mici sau nedetectabile4<\/sup>;<\/div>\n
\u2022 hipotiroidism congenital, pentru a distinge \u00eentre absen\u0163a complet\u0103 a glandei tiroide \u015fi hipoplazia tiroidian\u0103 sau alte condi\u0163ii patologice3<\/sup>.<\/div>\n

Preg\u0103tire pacient <\/em><\/strong>\u2013 \u00e0 jeun (pe nem\u00e2ncate) sau postprandial3<\/sup>.<\/p>\n

Specimen recoltat <\/em><\/strong>\u2013 s\u00e2nge venos3<\/sup>.<\/p>\n

Recipient de recoltare <\/em><\/strong>\u2013 vacutainer f\u0103r\u0103 anticoagulant, cu\/f\u0103r\u0103 gel separator3<\/sup><\/p>\n

Prelucrare necesar\u0103 dup\u0103 recoltare<\/em><\/strong> \u2013 se separ\u0103 serul prin centrifugare; se lucreaz\u0103 serul proasp\u0103t \u00een aceea\u015fi zi; dac\u0103 acest lucru nu este posibil, serul se p\u0103streaz\u0103 la 2-8\u00b0C, la -20\u00b0C sau la -70\u00b0C1<\/sup>.<\/p>\n

Volum prob\u0103<\/em><\/strong> \u2013 minim 0.5 mL ser3<\/sup>.<\/p>\n

Cauze de respingere<\/em><\/strong> a probei<\/em><\/strong> \u00a0<\/em><\/strong><\/p>\n

\u2022 specimen hemolizat;<\/div>\n
\u2022 specimen expus la temperatur\u0103 ridicat\u0103;<\/div>\n
\u2022 specimen contaminat bacterian3<\/sup>.<\/div>\n

Stabilitate prob\u0103 <\/em><\/strong>\u2013 serul separat este stabil 24 ore<\/em> la temperatura camerei; 3 zile<\/em> la 2-8\u00b0C; 1 lun\u0103<\/em> la -20\u00b0C sau la -70\u00b0C3<\/sup>. Evita\u0163i decongelarea\/recongelarea3<\/sup>.<\/p>\n

Metod\u0103 <\/i><\/b>- imunochimic\u0103 cu detec\u0163ie prin electrochemilumiscen\u0163\u0103 (ECLIA)<\/span>3<\/sup>.<\/p>\n

Valori de referin\u0163\u0103\u00a0 <\/i><\/b>\u2013 3.5-77 ng\/mL (\u03bcg\/L);<\/p>\n

\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 la pacien\u0163i atiroidieni: < 0.04 ng\/mL3<\/sup>.<\/p>\n

Limita de detec\u0163ie<\/i><\/b> \u2013 0.04 ng\/mL3<\/sup>.<\/p>\n

Limite \u015fi interferen\u0163e<\/i><\/b><\/p>\n

Tiroglobulina nu <\/b>este recomandat\u0103 \u00een screening-ul cancerului tiroidian. De asemenea nu<\/b> este util\u0103 \u00een diagnosticul cancerului tiroidian diferen\u0163iat, ci doar \u00een monitorizarea acestuia.<\/p>\n

La pacien\u0163ii cu tumori tiroidiene mici se \u00eentalnesc de obicei valori normale ale markerului4<\/sup>.<\/p>\n

\u2022 Interferen\u0163e analitice<\/p>\n

Prezen\u0163a anticorpilor anti-tiroglobulin\u0103 poate interfera cu determin\u0103rile de tiroglobulin\u0103.<\/p>\n

Pentru metoda ECLIA, pot produce interferen\u0163e cu unele componente ale kit-ului \u015fi conduce la rezultate neconcludente urm\u0103toarele:<\/p>\n

\u2013 tratamentul cu biotin\u0103 \u00een doze mari (>5 mg\/zi); de aceea se recomand\u0103 ca recoltarea de s\u00e2nge s\u0103 se fac\u0103 dup\u0103 minimum 8 ore de la ultima administrare;<\/div>\n
\u2013 titrurile foarte crescute de anticorpi anti-streptavidina \u015fi anti-ruteniu;<\/div>\n
\u2013 anticorpii monoclonali proveni\u0163i de la \u015foarece administra\u0163i la unii pacien\u0163i \u00een scop diagnostic sau terapeutic.<\/div>\n

Pentru metoda imunoenzimatic\u0103 cu detec\u0163ie prin chemiluminiscen\u0163\u0103, anticorpii heterofili prezen\u0163i \u00een serul pacien\u0163ilor pot interac\u0163iona cu imunoglobulinele incluse\u00a0\u00een componentele kit-ului \u015fi duce la rezultate neconcludente3<\/sup>.<\/p>\n
