Testul con\u021bine <strong>Sideremie<\/strong> \u0438 <\/span><strong><span style=\"color: #ff0000;\">Transferin\u0103.\u00a0<\/span> <\/strong><\/p>\n<p style=\"text-align: justify;\"><strong><em>\u041e\u0431\u0449\u0430\u044f \u0438\u043d\u0444\u043e\u0440\u043c\u0430\u0446\u0438\u044f<\/em><\/strong><\/p>\n<p style=\"text-align: justify;\">Transferina reprezint\u0103 proteina de legare plasmatic\u0103 a fierului; este o glicoprotein\u0103 de 79.57 kD, gena sa fiind localizat\u0103 pe cromozomul 3q21; este generat\u0103 \u00een principal de celulele parenchimatoase hepatice, dar cantit\u0103\u0163i mai mici sunt sintetizate \u015fi\u00a0\u00een sistemul nervos central, ovar, testicul, celule T helper; rata de sintez\u0103 este invers propor\u0163ional\u0103 cu depozitele de fier. Transferina are dou\u0103 situsuri de legare a fierului, fiecare dintre acestea leg\u00e2nd un ion feric (Fe<sup>3+<\/sup>), iar \u00een s\u00e2nge exist\u0103 at\u00e2t transferina monoferic\u0103, c\u00e2t \u015fi diferic\u0103. 1g de transferin\u0103 poate lega 25.1 \u00b5mol sau 1.4 mg de fier. \u00cen mod normal, numai o treime din situsurile de legare ale transferinei sunt ocupate. Spre deosebire de sideremie, care prezint\u0103 varia\u0163ii diurne mari, nivelul transferinei serice este mult mai constant. Transferina furnizeaz\u0103 fier c\u0103tre precursorii eritroizi \u015fi alte celule prin legarea de receptori specifici de pe suprafa\u0163a celular\u0103. Cantitatea de fier furnizat\u0103 este reglat\u0103 printr-un mecanism de feedback negativ\u00a0\u00een func\u0163ie de nevoile celulare pentru sinteza de hemoglobin\u0103<sup>1;7<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Recomand\u0103ri pentru determinarea transferinei<\/em><\/strong> (\u00een combina\u0163ie cu sideremia \u015fi feritina):<\/p>\n<div style=\"text-align: justify;
