Aproximativ 60% din cantitatea circulant\u0103 de protein\u0103 S este legat\u0103 de proteina de legare a frac\u0163iunii C4b a complementului (C4b-binding protein). Restul cantit\u0103\u0163ii de protein\u0103 se g\u0103se\u015fte sub form\u0103 liber\u0103, capabil\u0103 s\u0103 ac\u0163ioneze ca un cofactor non-enzimatic al proteinei C activate. Proteina S intensific\u0103 inactivarea factorilor Va \u015fi VIIIa la nivelul suprafe\u0163elor fosfolipidice \u015fi de asemenea exercit\u0103 un rol \u00een efectul fibrinolitic al proteinei C activate. Studiile recente indic\u0103 o posibil\u0103 implicare a frac\u0163iunii legate a proteinei S \u00een activitatea anticoagulant\u0103 independent\u0103 de proteina C activat\u0103<sup>1;2<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Deficitul de protein\u0103 S se asociaz\u0103 cu tromboz\u0103 venoas\u0103 recurent\u0103, cu sau f\u0103r\u0103 embolism pulmonar, survenit\u0103 la pacien\u0163i tineri<sup>1;2;3<\/sup>. Inciden\u0163a deficitului de protein\u0103 S \u00een popula\u0163ia general\u0103 nu este bine precizat\u0103; \u00een popula\u0163ia caucazian\u0103 deficitul a fost detectat la 1-5% din pacien\u0163ii care au prezentat un episod trombotic<sup>3<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Exist\u0103 3 tipuri de deficit de protein\u0103 S:<\/p>\n<p style=\"text-align: justify;\">\u2013 tipul I: proteina S este normal\u0103 func\u0163ional, dar se g\u0103se\u015fte \u00eentr-o cantitate insuficient\u0103 pentru a controla cascada coagul\u0103rii (sunt sc\u0103zute at\u00e2t forma liber\u0103 c\u00e2t \u015fi cea legat\u0103, iar activitatea este redus\u0103);<\/p>\n<p style=\"text-align: justify;\">\u2013 tipul II: cantitate normal\u0103 de protein\u0103 S, care este \u00eens\u0103 incapabil\u0103 s\u0103-\u015fi exercite func\u0163ia (activitate redus\u0103) ca urmare a unor defecte moleculare;<\/p>\n<p style=\"text-align: justify;\">\u2013 tipul III: cantitate redus\u0103 de protein\u0103 S liber\u0103 asociat\u0103 cu o activitate sc\u0103zut\u0103, \u00een prezen\u0163a unei cantit\u0103\u0163i normale de protein\u0103 S total\u0103<sup>3<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Deficitul dob\u00e2ndit de protein\u0103 S poate fi \u00eent\u00e2lnit \u00een sarcin\u0103, administrarea contraceptivelor orale, afec\u0163iuni hepatice, tratament cu anticoagulante orale, nefropatie diabetic\u0103, tromboza venelor cerebrale, purpura trombotic\u0103 trombocitopenic\u0103 precum \u015fi \u00een alte afec\u0163iuni autoimune \u015fi inflamatorii \u00een care se \u00eenregistreaz\u0103 o cre\u015ftere a proteinei de legare a frac\u0163iunii C4b \u015fi concomitent o sc\u0103dere a frac\u0163iunii libere<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Recomand\u0103ri pentru determinarea proteinei S<\/em><\/strong><\/p>\n<ul style=\"text-align: justify;\">\n<li style=\"text-align: justify;\">investigarea unor complica\u0163ii tromboembolice neexplicabile, \u00een special la pacien\u0163i &lt;50 ani \u015fi cu istoric familial pozitiv de evenimente trombotice;<\/li>\n<li style=\"text-align: justify;\">\u00eenaintea administr\u0103rii contraceptivelor orale la paciente cu episoade trombotice \u00een antecedente sau cu istoric familial pozitiv<sup>4<\/sup>.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><strong><em>Preg\u0103tire pacient<\/em><\/strong> <em>- <\/em>\u00e0 jeun; dac\u0103 pacientul se afla sub terapie cu anticoagulante orale, acestea vor fi \u00eentrerupte cu 2 s\u0103pt\u0103m\u00e2ni \u00eenaintea recolt\u0103rii; de asemenea, tratamentul cu heparin\u0103 va fi \u00eentrerupt cu 2 zile \u00eenaintea recolt\u0103rii<sup>4;5<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Specimen recoltat <\/em><\/strong><em>-<\/em> \u0432\u0435\u043d\u043e\u0437\u043d\u0430\u044f \u043a\u0440\u043e\u0432\u044c<sup>4<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Recipient de recoltare <\/em><\/strong><em>-<\/em> vacutainer cu citrat de Na 0.105 M (1\/9) (capac bleu\/verde)<sup>4<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Cantitatea recoltat\u0103 <\/em><\/strong>\u2013 c\u00e2t permite vacuumul<sup>3<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Cauze de respingere a probei<\/em><\/strong><\/p>\n<ul style=\"text-align: justify;\">\n<li>specimen hemolizat<\/li>\n<li>specimen coagulat<\/li>\n<li>vacutainer care nu este plin<\/li>\n<li>specimen contaminat cu heparin\u0103<sup>4<\/sup>.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><strong><em>Prelucrare necesar\u0103 dup\u0103 recoltare <\/em><\/strong>\u2013 se separ\u0103 plasma prin centrifugare \u015fi se lucreaz\u0103 imediat; dac\u0103 acest lucru nu este posibil, plasma se poate stoca la -20\u00b0C; probele de plasm\u0103 recoltate \u00een afara sediului laboratorului vor fi transportate \u00een recipientul destinat probelor congelate<sup>4<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Stabilitate prob\u0103<\/em><\/strong> \u2013 plasma separat\u0103 este stabil\u0103 <em>4 ore <\/em>la 18-25\u00b0C; <em>1 lun\u0103<\/em> la -20C<sup>4<\/sup>; se va evita decongelarea\/recongelarea<sup>4<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Metod\u0103 <\/em><\/strong><em>-<\/em> <u>test de coagulare care m\u0103soar\u0103 activitatea proteinei S<\/u>. Se determin\u0103 activitatea proteinei S de cofactor al proteinei C activate \u00een inactivarea FVa, \u00eentr-un test bazat pe APTT modificat\u00a0; plasma de pacient diluat\u0103 se amestec\u0103 cu o plasm\u0103 deficitar\u0103 \u00een proteina S \u00een prezen\u0163a unei proteine C activate purificate \u015fi a factorului Va; dup\u0103 o scurt\u0103 incubare se adaug\u0103 clorura de calciu pentru a ini\u0163ia coagularea; prelungirea timpului de coagulare este propor\u0163ional\u0103 cu nivelul de protein\u0103 S din plasma pacientului<sup>4<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Valori de referin\u0163\u0103:\u00a0<\/em><\/strong><\/p>\n<p style=\"text-align: justify;\">Femei: 57-131%<\/p>\n<p style=\"text-align: justify;\">B\u0103rba\u0163i: 66-143%<sup>4<\/sup><\/p>\n<p style=\"text-align: justify;\"><strong><em>Interpretarea rezultatelor <\/em><\/strong><\/p>\n<p style=\"text-align: justify;\">Testul detecteaz\u0103 toate cele 3 tipuri de deficit, dar nu permite diferen\u0163ierea lor. De aceea, \u00een cazul ob\u0163inerii unei activit\u0103\u0163i sc\u0103zute a proteinei S se recomand\u0103 testarea antigenului proteinei S totale \u015fi proteinei S libere<sup>4<\/sup>.<\/p>\n<p style=\"text-align: justify;\">\u00cen cazul deficitului heterozigot de protein\u0103 S riscul de tromboembolism venos este de ~ 10 ori mai mare dec\u00e2t \u00een popula\u0163ia general\u0103<sup>6<\/sup>, astfel c\u0103 60-80% dintre pacien\u0163i vor dezvolta tromboz\u0103 \u00een cursul vie\u0163ii<sup>7<\/sup>. Majoritatea evenimentelor trombotice apar la persoane cu v\u00e2rste &lt; 40-45 ani<sup>7<\/sup>. Alte expresii fenotipice ale deficitului de protein\u0103 S includ: pierderi recurente de sarcin\u0103, complica\u0163ii obstetricale (preeclampsie, abruptio placentae, restric\u0163ie de cre\u015ftere intrauterin\u0103, moartea f\u0103tului <em>in utero<\/em>) \u015fi posibil tromboz\u0103 arterial\u0103 (asocierea este controversat\u0103)<sup>6;7<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Deficitul homozigot de protein\u0103 S este rar \u015fi se manifest\u0103 ca purpur\u0103 fulminans neonatal\u0103, reflect\u00e2nd coagularea intravascular\u0103 sever\u0103 \u015fi fibrinoliza\/coagularea intravascular\u0103 diseminat\u0103 cauzate de valorile nedetectabile sau extrem de scazute ale activit\u0103\u0163ii proteinei S<sup>6<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Limite \u015fi interferen\u0163e<\/em><\/strong><\/p>\n<p style=\"text-align: justify;\">Nou-n\u0103scu\u0163ii prezint\u0103 o activitate sc\u0103zut\u0103 a proteinei S<sup>5<\/sup>, \u00eens\u0103 dup\u0103 3-6 luni se \u00eenregistreaz\u0103 valori apropiate de cele ale adultului<sup>6<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Sarcina \u015fi folosirea contraceptivelor orale\u00a0se pot asocia de asemenea cu o reducere a activit\u0103\u0163ii proteinei S<sup>1;5<\/sup>. \u00cen cursul sarcinii se produce o reducere progresiv\u0103 a activit\u0103\u0163ii proteinei S, astfel c\u0103 \u00een trimestrul III pot fi \u00eenregistrate valori cuprinse \u00eentre 16 \u015fi 42%; din acest motiv nu se recomand\u0103 aceast\u0103 testare \u00een sarcin\u0103<sup>8<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Nu se recomand\u0103 testarea \u00een cursul unui proces trombotic acut deoarece nivelul proteinei S poate fi sc\u0103zut<sup>7<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Prezen\u0163a inhibitorilor de trombin\u0103 (hirudin sau argatroban), anticoagulan\u0163ilor lupici sau altor anticorpi antifosfolipidici \u00een plasma testat\u0103 poate determina o supraestimare a activit\u0103\u0163ii proteinei S. Pe de alt\u0103 parte, rezisten\u0163a la proteina C activat\u0103 (datorat\u0103 muta\u0163iei factorului V Leiden) poate induce m\u0103surarea unei activit\u0103\u0163i sc\u0103zute a proteinei S<sup>4;5<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Nu se cunoa\u015fte semnifica\u0163ia clinic\u0103 a nivelului crescut de activitate a proteinei S<sup>6<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><u>Influen<\/u><u>\u0163a tratamentului anticoagulant asupra determin\u0103rii proteinei S<\/u><\/p>\n<ul style=\"text-align: justify;\">\n<li style=\"text-align: justify;\"><strong>heparina nefrac\u0163ionat\u0103<\/strong> influen\u0163eaz\u0103 determinarea activit\u0103\u0163ii proteinei S prin metoda coagulometric\u0103 prin inducerea unor valori fals crescute, de aceea nu se recomand\u0103 testarea sub aceast\u0103 terapie;<\/li>\n<li style=\"text-align: justify;\"><strong>heparinele cu greutate molecular\u0103 mic\u0103<\/strong> nu influen\u0163eaz\u0103 testarea, totu\u015fi se recomand\u0103 ca recoltarea probei s\u0103 se efectueze la 12 ore de la ultima administrare sau imediat \u00eenaintea administr\u0103rii urm\u0103toarei doze;<\/li>\n<li style=\"text-align: justify;\">tratamentul cu <strong>fondaparinux<\/strong> nu influen\u0163eaz\u0103 testarea;<\/li>\n<li style=\"text-align: justify;\"><strong>anticoagulantele orale<\/strong> reduc activitatea proteinei S, de aceea se recomand\u0103 ca testarea s\u0103 se efectueze dup\u0103 minimum 2 s\u0103pt\u0103m\u00e2ni de la \u00eentreruperea tratamentului; dac\u0103 nu este posibil\u0103 stoparea anticoagul\u0103rii pacientul poate fi trecut temporar pe o heparin\u0103 cu greutate molecular\u0103 mic\u0103;<\/li>\n<li style=\"text-align: justify;\"><strong>dabigatranul <\/strong>(inhibitor de trombin\u0103) influen\u0163eaz\u0103 determinarea activit\u0103\u0163ii proteinei S prin metoda coagulometric\u0103 prin inducerea unor valori fals crescute, de accea nu se recomand\u0103 testarea sub aceast\u0103 terapie;<\/li>\n<li style=\"text-align: justify;\">de asemenea, inhibitorii direc\u0163i de factor Xa (<strong>rivaroxaban<\/strong> \u0438\u043b\u0438 <strong>apixaban<\/strong>) influen\u0163eaz\u0103 determinarea activit\u0103\u0163ii proteinei S prin metoda coagulometric\u0103 prin inducerea unor valori fals crescute, de aceea nu se recomand\u0103 testarea sub aceste terapii<sup>4;9<\/sup>.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><strong><span style=\"font-size: 10pt;\">\u0411\u0438\u0431\u043b\u0438\u043e\u0433\u0440\u0430\u0444\u0438\u044f<\/span><\/strong><\/p>\n<ol>\n<li style=\"text-align: justify;\"><span style=\"font-size: 10pt;\">Fischbach F.
