Plumbul este cel mai larg utilizat metal neferos. Plumbul \u015fi compu\u015fii s\u0103i au numeroase aplica\u0163ii comerciale \u015fi industriale, fiind prezen\u0163i \u00een lacuri \u015fi vopseluri, baterii, pigmenti, insecticide, materiale plastice \u015fi ceramice, echipamente medicale, armament, benzin\u0103, industria sudurii.<\/span><\/p>\n

Principalele c\u0103i de expunere la plumbul anorganic sunt inhalarea \u015fi ingestia. La adult expunerea se realizeaz\u0103 \u00een special profesional, iar la copii prin ingestia de vopseluri cu plumb din case vechi (\u00een sindromul de \u201cpica\u201d). Expunerea se mai poate face \u015fi prin practicarea distil\u0103rii ilegale a alcoolului\u00a0\u00een recipiente ce con\u0163in plumb2;4<\/sup>.<\/span><\/p>\n

Plumbul este absorbit \u00een organism at\u00e2t prin tractul respirator, cat \u015fi prin cel gastrointestinal. Absorb\u0163ia este invers propor\u0163ional\u0103 cu dimensiunea particulei, ceea ce face ca inhalarea prafului cu plumb s\u0103 aib\u0103 impactul cel mai mare. Este transportat \u015fi transplacentar fiind implicat\u00a0\u00een\u00a0intoxica\u0163ia congenital\u0103, cu urm\u0103toarele consecin\u0163e posibile: avort spontan, prematuritate sau greutate mic\u0103 la na\u015ftere2<\/sup>.<\/span><\/p>\n

Simptomele precoce ale intoxica\u0163iei cu plumb (saturnism) sunt reprezentate de: apatie sau iritabilitate, astenie fizic\u0103, anorexie, grea\u0163\u0103, constipa\u0163ie, dureri abdominale intermitente, mialgii. Anemia cu reticulocitoz\u0103 \u015fi puncta\u0163ii bazofile la examinarea frotiului de s\u00e2nge periferic se num\u0103r\u0103 printre complica\u0163iile hematologice ale saturnismului. De asemenea mai pot s\u0103 apar\u0103: lizereu gingival caracteristic \u015fi depozite de plumb pe radiografii ale oaselor lungi la copii. In cazul intoxica\u0163iilor severe apar tulbur\u0103ri gastrointestinale grave \u015fi neurotoxicitate (ataxie, slabiciune muscular\u0103, convulsii, stupor, com\u0103)2;4<\/sup>.<\/span><\/p>\n

Cre\u015fterea excre\u0163iei urinare a plumbului indic\u0103 o expunere excesiv\u0103, independent de manifest\u0103rile clinice.<\/span><\/p>\n

Alte caracteristici ale saturnismului includ cre\u015fterea acidului delta-aminolevulinic urinar, cre\u015fterea protoporfirinei libere eritrocitare \u015fi sc\u0103derea dehidrazei acidului aminolevulinic2<\/sup>.<\/span><\/p>\n

Recomand\u0103ri pentru determinarea plumbului<\/em><\/strong><\/span><\/p>\n

\u2013 plumbul \u00een s\u00e2nge: cel mai bun test pentru diagnosticul expunerii la plumb;<\/span><\/p>\n

\u2013 plumbul \u00een urin\u0103: monitorizarea intoxica\u0163iei cronice cu plumb2;4<\/sup>.<\/span><\/p>\n

Preg\u0103tire pacient <\/em><\/strong>\u2013 momentul recolt\u0103rii probei de s\u00e2nge nu este important pentru monitorizarea expunerii profesionale.<\/span><\/p>\n

Specimen recoltat <\/em><\/strong>\u2013 1) s\u00e2nge venos; 2) urina din 24 ore sau 3) o prob\u0103 de urin\u0103 spontan\u01033;4<\/sup>.<\/span><\/p>\n

Cauze de respingere a probei <\/em><\/strong>\u2013 specimen coagulat3;4<\/sup>.<\/span><\/p>\n

Recipient de recoltare <\/em><\/strong>\u2013 1) vacutainer pentru metale ce con\u0163ine EDTA sau heparinat de litiu ca anticoagulant; 2) vas de 2-3 litri \u015fi pahar de plastic de unic\u0103 folosin\u0163\u0103 pentru urin\u0103, pe care se noteaz\u0103 cantitatea total\u0103 de urin\u0103 din 24 ore; nu se folosesc conservan\u0163i; 3) eprubeta pentru urin\u01033;4<\/sup>.<\/span><\/p>\n

Cantitatea recoltat\u0103 <\/em><\/strong>\u2013 1) cat permite vacuumul; 2), 3) aproximativ 10 mL3;4<\/sup>.<\/span><\/p>\n

Prelucrare necesar\u0103 dup\u0103 recoltare <\/em><\/strong>\u2013 nu este necesar\u01033<\/sup>.<\/span><\/p>\n

Stabilitate prob\u0103 <\/em><\/strong>- <\/em>probele de sange \u015fi urin\u0103 se men\u0163in la temperatura camerei sau la frigider pan\u0103 \u00een momentul lucrului3;4<\/sup>.<\/span><\/p>\n
