Informa\u0163ii generale \u015fi recomand\u0103ri<\/strong><\/em>
\nTrombofilia reprezint\u0103 o condi\u0163ie ereditar\u0103 sau dob\u00e2ndit\u0103 asociat\u0103 cu o predispozi\u0163ie individual\u0103 fa\u0163\u0103 de evenimente trombotice.<\/p>\n

Principalele manifest\u0103ri clinice generate de trombofilie sunt: boala tromboembolic\u0103, avorturile recurente de cauz\u0103 incert\u0103, complica\u0163ii ale sarcinii (preeclampsie, retard de cre\u015ftere intrauterin\u0103, apoplexia uteroplacentar\u0103, moartea f\u0103tului \u00een uter, na\u015ftere prematur\u0103), sindroamele coronariene acute (angina instabil\u0103, infarct miocardic), necroza aseptic\u0103 de cap femural.<\/p>\n

Trombofilia trebuie suspectat\u0103 \u00een urm\u0103toarele situa\u0163ii clinice: evenimente trombotice survenite la pacien\u0163i tineri (<50 ani), tromboze recurente, antecedente familiale de afec\u0163iuni tromboembolice, complica\u0163ii multiple ale sarcinii, tromboze venoase \u00een teritorii anatomice \u201cneobi\u015fnuite\u201d: vena port\u0103, vene hepatice, mezenterice, cerebrale, etc.
\nP\u00e2n\u0103 la 70% dintre pacien\u0163ii diagnostica\u0163i cu o forma de trombofilie prezint\u0103 unul sau mai multe defecte genetice majore. Dintre acestea, factorul V Leiden \u015fi muta\u0163ia G20210A a protrombinei sunt responsabile pentru mai mult de jum\u0103tate din cazurile de trombofilie ereditar\u0103. Aceste muta\u0163ii sunt relativ recent descoperite (din 1994) \u015fi au la baza substitu\u0163ii nucleotidice unice, care pot fi detectate \u00een prezent prin tehnici moleculare6<\/sup>.<\/p>\n

Factorul V Leiden constituie o variant\u0103 anormal\u0103 a factorului V ce devine rezistent la ac\u0163iunea proteinei C activate (APC). Este generat de o muta\u0163ie punctiform\u0103 a genei de pe cromozomul 1q23 care codific\u0103 sinteza factorului V, prin substitu\u0163ia guaninei (G) cu adenina (A) \u00een pozi\u0163ia 1691. \u00cen produsul proteic, substitu\u0163ia nucleotidic\u0103 se exprim\u0103 prin \u00eenlocuirea argininei din pozi\u0163ia 506 cu glutamina (muta\u0163ia R506Q). Factorul V astfel modificat este denumit factor Leiden, alterarea const\u00e2nd \u00een modificarea situsului de legare a APC.<\/p>\n

Ca urmare a acestei muta\u0163ii, factorul V Leiden este inactivat de 10 ori mai lent dec\u00e2t cel normal \u015fi persist\u0103 mai mult timp \u00een circula\u0163ie, cea ce conduce la o generare crescut\u0103 de trombin\u0103 \u015fi astfel, la hipercoagulabilitate2;3<\/sup>.<\/p>\n

Analizele genetice au indicat faptul c\u0103 aceast\u0103 muta\u0163ie este responsabil\u0103 de 85-90% din cazurile de rezisten\u0163\u0103 la APC4<\/sup>.<\/p>\n

Muta\u0163ia este transmis\u0103 autozomal co-dominant \u015fi este prezent\u0103 la aproximativ 3-7% din popula\u0163ia caucazian\u0103, fiind astfel cea mai frecvent\u0103 cauz\u0103 de trombofilie ereditar\u01032<\/sup>. Aceasta muta\u0163ie a fost confirmat\u0103 la pacien\u0163i cu tromboz\u0103 venoas\u0103 profund\u0103, embolie pulmonar\u0103, ocluzia venei centrale a retinei, tromboza sinusurilor cerebrale \u015fi tromboza venelor hepatice. Muta\u0163ia este depistat\u0103 la 10-20% dintre pacien\u0163ii afla\u0163i la primul episod de tromboz\u0103 venoas\u0103. De asemenea, 40% din pacien\u0163ii <50 ani afla\u0163i la primul episod de tromboz\u0103 venoasa \u015fi 60% din pacientele gravide cu tromboz\u0103 venoas\u0103 sunt purt\u0103tori ai acestei muta\u0163ii.<\/p>\n

Riscul de tromboz\u0103 depinde de statusul homo- sau heterozigot pentru aceast\u0103 muta\u0163ie. Indivizii heterozigo\u0163i au un risc de tromboz\u0103 de 4-8 ori mai mare dec\u00e2t persoanele cu factor V normal, \u00een timp ce homozigo\u0163ii prezint\u0103 un risc de 80-100 ori mai mare5<\/sup>.<\/p>\n

Riscul cre\u015fte exponen\u0163ial la persoanele care asociaz\u0103 \u015fi al\u0163i factori de risc trombotic, cum ar fi: coexisten\u0163a altor defecte genetice (deficit de protein\u0103 C, S, antitrombin\u0103 III, muta\u0163ia genei protrombinei, muta\u0163ia genei care codific\u0103 metilentetrahidrofolat reductaza -MTHFR- \u015fi se asociaz\u0103 hiperhomocisteinemie), consumul de anticoncep\u0163ionale orale, sarcina5;6<\/sup>.<\/p>\n

Astfel, tromboza venoas\u0103 profund\u0103 \u015fi tromboza sinusurilor cerebrale apar mai frecvent la pacientele care iau anticoncep\u0163ionale orale, gravide sau aflate \u00een perioada postpartum6<\/sup>.
\nMiddendorf \u015fi colaboratorii s\u0103i au efectuat un studiu (2004) despre rolul factorului Leiden la pacien\u0163ii cu infarct miocardic. Concluziile studiului au fost c\u0103 pacien\u0163ii care prezint\u0103 muta\u0163ia factorului V Leiden au o predispozi\u0163ie pentru infarctul miocardic iar factorii tradi\u0163ionali de risc pentru boala coronarian\u0103 ar avea un rol mai pu\u0163in important la ace\u015fti pacien\u0163i2<\/sup>.<\/p>\n

\u00cen ceea ce prive\u015fte complica\u0163iile sarcinii, prezen\u0163a factorului V Leiden, ca \u015fi cea a muta\u0163iilor genelor protrombinei \u015fi MTHFR, se asociaz\u0103 cu un risc crescut de pierderi recurente de sarcin\u0103, \u00een special \u00een trimestrele II \u015fi III1<\/sup>.<\/p>\n

Screening-ul membrilor asimptomatici \u00een familiile la care s-a depistat muta\u0163ia nu este recomandat pe scar\u0103 larg\u0103, datorita riscului sc\u0103zut de tromboze venoase letale. Cu toate acestea, investigarea poate aduce beneficii \u00een situa\u0163iile \u00een care se au \u00een vedere sarcina sau administrarea de contraceptive orale, la persoanele care au antecedente familiale de tromboze venoase recurente ap\u0103rute la v\u00e2rste < 50 ani. Persoanele care solicita testarea \u015fi cele care ob\u0163in un rezultat pozitiv trebuie informate \u00een leg\u0103tura cu implica\u0163iile diagnosticului \u015fi cu semnele sau simptomele care necesit\u0103 interven\u0163ie medical\u0103 de urgen\u0163\u01032;6<\/sup>.<\/p>\n

Testul efectuat \u00een laboratorul Synevo utilizeaz\u0103 tehnica real-time PCR pe instrumentul LightCycler pentru detec\u0163ia \u015fi genotiparea muta\u0163ei factorului V Leiden, la pacien\u0163i cu suspiciune clinic\u0103 de trombofilie4<\/sup>.<\/p>\n

\nRecipient de recoltare<\/strong> \u2013 vacutainer ce con\u0163ine EDTA ca anticoagulant4<\/sup>.
\nCantitate recoltat\u0103<\/strong> \u2013 c\u00e2t permite vacuumul4<\/sup>.
\nCauze de respingere a probei<\/strong> \u2013 folosirea heparinei ca anticoagulant4<\/sup>.
\nStabilitate prob\u0103 <\/strong>\u2013 7 zile la 2-8\u00baC4<\/sup>.
\nMetod\u0103<\/strong> - Real-time PCR (LightCycler)<\/span>: reac\u0163ie de polimerizare \u00een lan\u0163 cu detec\u0163ie \u00een timp real a produsului PCR acumulat, prin m\u0103surarea fluorescen\u0163ei emise; la sf\u00e2r\u015fitul ciclurilor de amplificare se efectueaz\u0103 genotiparea, prin analiza curbei de topire4<\/sup>.<\/p>\n

Interpretarea rezultatelor<\/strong><\/em>
\nPentru un pacient, se raporteaz\u0103 unul din cele 3 rezultate posibile:
\n\u2013 genotip s\u0103lbatic homozigot (negativ pentru muta\u0163ia factorului V Leiden);
\n\u2013 genotip heterozigot (con\u0163ine o alel\u0103 s\u0103lbatic\u0103 \u015fi una mutant\u0103 pentru factorul V);
\n\u2013 genotip mutant homozigot (con\u0163ine ambele alele mutante)4<\/sup>.<\/p>\n

Limite \u015fi interferen\u0163e<\/strong><\/em>
\nLa nivelul genei factorului V, pe l\u00e2ng\u0103 muta\u0163ia factorului V Leiden \u00een pozi\u0163ia 1691, mai sunt posibile alte 3 muta\u0163ii: 1692, 1689 \u015fi 1696. La genotipare, aceste muta\u0163ii rare vor conduce la un rezultat fals-pozitiv (genotip heterozigot sau mutant homozigot). Deoarece numai muta\u0163ia factorului V Leiden se asociaz\u0103 cu o rezisten\u0163\u0103 crescut\u0103 la proteina C activat\u0103, to\u0163i pacien\u0163ii care prezint\u0103 un rezultat pozitiv la testul PCR vor trebui s\u0103 efectueze un test func\u0163ional (de coagulare) pentru a evalua rezisten\u0163a la proteina C activat\u0103, \u00een cazul \u00een care acest screening nu s-a efectuat anterior4<\/sup>.<\/p>\n
