Excretia de magneziu controleaza nivelul seric al acestui element si este dependenta de dieta<sup>1<\/sup>, pe de alta parte fiind supusa, impreuna cu cea a calciului, efectelor hormonului paratiroidian; cresterea reabsorbtiei calciului conduce la inhibitia competitiva a absorbtiei magneziului<sup>5<\/sup>. Hipercalcemia, hipofosfatemia si acidoza se numara printre inhibitorii reabsorbtiei tubulare de magneziu<sup>4<\/sup>. Cand functia renala este scazuta, magneziul este retinut determinand cresterea nivelului seric.<\/p>\n<p><strong><em>Recomandari pentru determinarea magneziului urinar <\/em><\/strong><\/p>\n<p style=\"padding-left: 30px;\">\u2022\u00a0 investigarea statusului electrolitic; stabilirea cauzei unui nivel seric anormal de magneziu;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 \u00a0investigarea nefrolitiazei<sup>1<\/sup>.<\/p>\n<p><strong><em>Specimen recoltat <\/em><\/strong><em>-<\/em> urina din 24 ore; la ora 7 dimineata pacientul urineaza si nu retine aceasta urina, apoi colecteaza intr-un vas de plastic toate emisiunile de urina pana la ora 7 dimineata in ziua urmatoare, inclusiv; se omogenizeaza urina recoltata; se masoara intreaga cantitate, se retin 100 ml in pahar de plastic de unica folosinta pentru urina; nu se folosesc conservanti; ambele recipiente trebuie sa fie bine inchise cu capac, ele se pastreaza la 2\u20138<sup>\u00ba<\/sup>C pana in momentul lucrului<sup>3<\/sup>. !Sunt inacceptabile probele care vin in contact cu orice fel de metal<sup>1<\/sup>.<\/p>\n<p><strong><em>Recipient de recoltare <\/em><\/strong><em>-<\/em> vas de plastic de 2-3 L si pahar de plastic pentru urina (de unica folosinta), pe care se noteaza cantitatea totala de urina din 24 ore<sup>3<\/sup>.<\/p>\n<p><strong><em>Cantitatea recoltata <\/em><\/strong><em>-<\/em> toata urina din 24 ore din care se retin 100 mL<sup>3<\/sup>.<\/p>\n<p><strong><em>Prelucrare necesara dupa recoltare <\/em><\/strong><em>- <\/em>acidifiere la pH = 3-4 cu acid clorhidric 6 N<sup>3<\/sup>.<\/p>\n<p><strong><em>Stabilitate proba <\/em><\/strong><em>-<\/em><em>7 zile<\/em> la temperatura camerei; <em>7 zile<\/em> la 4-8\u00b0C; <em>1 an<\/em> la -20\u00b0C<sup>3<\/sup>.<\/p>\n<p><strong><em>Metoda <\/em><\/strong><em>- <\/em><span style=\"text-decoration: underline;\">spectrofotometrica (colorimetrica)<\/span><sup>3<\/sup>.<\/p>\n<p><strong><em>Valori de referinta <\/em><\/strong><em>-<\/em> 72.9-121.5 mg\/24h<sup>3<\/sup>.<\/p>\n<p><strong><em>Factor de conversie: <\/em><\/strong><em>mmol\/L x 2.43 = mg\/dL; mEq\/L x 0.5 = mmol\/L; mEq\/L x 1.2 = mg\/dL<\/em><em>.<\/em><\/p>\n<p><strong><em>Limita de detectie<\/em><\/strong><em> - <\/em>0.03 mmol\/L (0.07 mg\/dL)<sup>3<\/sup>.<em>\u00a0<\/em><\/p>\n<p><strong><em>Interpretarea rezultatelor<\/em><\/strong><\/p>\n<table class=\"wp-block-table w-100\" border=\"1\" cellspacing=\"0\" cellpadding=\"0\">\n<tbody>\n<tr>\n<td valign=\"top\" width=\"312\">\n<p style=\"text-align: center;\"><strong><em>Cresteri<\/em><\/strong><\/p>\n<\/td>\n<td valign=\"top\" width=\"312\">\n<p style=\"text-align: center;\"><strong><em>Scaderi<\/em><\/strong><\/p>\n<\/td>\n<\/tr>\n<tr>\n<td valign=\"top\" width=\"312\">\n<p style=\"text-align: center;\">\u2022 sindrom Bartter; \u2022 boli renale cu pierderi excesive de magneziu (glomerulonefrita cronica, pielonefrita cronica, acidoza tubulara renala, faza diuretica a necrozei tubulare acute, diureza postobstructiva); \u2022hiperaldosteronism; \u2022 pacientii cu transplant renal sub tratament imunosupresor<sup>1;4<\/sup><sup>;5<\/sup>.<\/p>\n<\/td>\n<td valign=\"top\" width=\"312\">\n<p style=\"text-align: center;\">\u2022 malabsorbtie; \u2022 alcoolism cronic (malnutritie); \u2022tratament parenteral indelungat; \u2022 insuficienta renala cronica; \u2022 hipoparatiroidism; \u2022 hipercalciurie; \u2022 boala Addison<sup>1;4<\/sup>.<\/p>\n<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p>Scaderea magneziului urinar sugereaza deficit de magneziu, iar daca pierderea este de cauza renala, magneziul urinar este crescut.<\/p>\n<p><strong><em>Limite si interferente<\/em><\/strong><\/p>\n<p>Alcoolemia ridicata determina cresterea magneziului urinar<sup>1<\/sup>.
