S-a constatat c\u0103 \u00een perioadele acute ale afec\u0163iunilor demielinizante ale SNC (dintre care cea mai cunoscut\u0103 este scleroza multipl\u0103) se elibereaz\u0103 \u00een lichidul cefalorahidian (LCR) fragmente de mielin\u0103 care pot constitui astfel un marker de activitate a bolii<sup>3<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Scleroza multipl\u0103 este o boal\u0103 demielinizant\u0103 care afecteaz\u0103 substan\u0163a alb\u0103 din creier \u015fi m\u0103duva spin\u0103rii. Apare mai frecvent la femei dec\u00e2t la b\u0103rba\u0163i \u015fi este legat\u0103 mai ales de perioada de adult t\u00e2n\u0103r. Tabloul clinic al bolii este polimorf. Aproximativ 50% din pacien\u0163i prezint\u0103 altern\u0103ri ale puseelor de boal\u0103 activ\u0103 cu perioade de\u00a0remisiune, aceasta suger\u00e2nd o afectare cronic\u0103 progresiv\u0103. De obicei, diagnosticul de scleroz\u0103 multipl\u0103 are la baz\u0103 manifest\u0103rile clince \u015fi excluderea altor afec\u0163iuni.<\/p>\n<p style=\"text-align: justify;\">Din punct de vedere al investiga\u0163iilor de laborator, cel mai frecvent este folosit\u0103 examinarea LCR. Aceast\u0103 investiga\u0163ie furnizeaz\u0103 informa\u0163ii care coroborate vin s\u0103 sus\u0163in\u0103 diagnosticul de scleroz\u0103 multipl\u0103. Sinteza de imunglobuline G (IgG) cre\u015fte anormal \u00een LCR la pacien\u0163ii cu scleroz\u0103 multipl\u0103, de aceea m\u0103surarea indexului IgG \u015fi a ratei de sintez\u0103 a IgG se dovede\u015fte util, dar nu este un test specific. Evaluarea LCR pentru benzile oligoclonale este, de asemenea, util\u0103 \u00een aprecierea statusului clinic, fiind identificate la mai mult de 90% din pacien\u0163ii cu scleroz\u0103 multipl\u0103. Totu\u015fi prezen\u0163a lor nu are specificitate de diagnostic deoarece acestea sunt \u00a0depistate \u015fi \u00een alte afec\u0163iuni cum ar fi: neurosifilis, vasculite ale SNC, boala Lyme, panencefalita subacut\u0103 sclerozant\u0103, boala Creutzfeldt-Jakob, sindrom Guillain-Barr\u00e9, tumori.<\/p>\n<p style=\"text-align: justify;\">De\u015fi cauzele sclerozei multiple nu sunt cunoscute, mecanismul autoimun pare s\u0103 joace un rol important \u00een patogeneza acestei boli. Cel mai frecvent doza\u0163i sunt anticorpii anti-MOG (glicoproteina oligodendrocitar\u0103 a mielinei) \u015fi anticorpii anti-MBP. S-a constatat c\u0103 79% din pacien\u0163ii cu scleroz\u0103 multipl\u0103 prezint\u0103 \u00een LCR celule care produc anticorpi anti-mielin\u0103 \u015fi 57% prezint\u0103 \u00een LCR celule care produc anticorpi anti-MBP de tip IgG. Propor\u0163ia acestor celule \u00een sangele periferic este semnificativ sc\u0103zut\u0103 fa\u0163\u0103 de LCR. De asemenea\u00a0 propor\u0163ia acestor celule \u00een LCR este mult mai sc\u0103zut\u0103 la pacien\u0163ii cu meningoencefalit\u0103 aseptic\u0103<sup>4<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Studiile au aratat c\u0103 \u00a0nivelurile de anticorpi anti-MBP sunt crescute la pacien\u0163ii cu scleroz\u0103 multipl\u0103 activ\u0103 clinic \u015fi c\u0103 exist\u0103 o corela\u0163ie \u00eentre statusul clinic \u015fi nivelul anticorpilor. Ace\u015fti anticorpi nu au fost depista\u0163i \u00een cazurile de boal\u0103 stabilizat\u0103.
