Astfel, hemoglobina este transformata sub actiunea sucurilor digestive pana la hematina, care se regaseste in materiile fecale. Hematochezia (sange rosu emis ca atare, izolat sau amestecat cu materiile fecale) este sugestiva pentru o hemoragie care a avut loc in portiunile terminale ale intestinului, adica in rect, colon, eventual in ileon (colite ulcerative, boala Crohn, carcinom colorectal, polipoza)<sup>1;6<\/sup>.<\/p>\n<p>Determinarea hemoragiilor oculte este un test screening deosebit de important pentru medicina preventiva. Numeroase studii au aratat ca folosirea regulata a acestui test, izolat sau impreuna cu sigmoidoscopia, reduce semnificativ mortalitatea prin cancer colo-rectal<sup>4<\/sup>.<\/p>\n<p><strong><em>Recomandari pentru determinarea hemoragiilor oculte (sangerarilor oculte)<\/em><\/strong><\/p>\n<p style=\"padding-left: 30px;\">\u2013 screening-ul pacientilor asimptomatici in scopul depistarii cancerului colo-rectal (se recomanda efectuarea anuala a testului la pacientii peste 45-50 ani)<sup>4<\/sup><\/p>\n<p style=\"padding-left: 30px;\">\u2013 suspiciune de hemoragie digestiva superioara sau inferioara (indiferent de etiologie: benigna sau maligna) la pacienti cu discomfort abdominal, alterari ale tranzitului intestinal, pierdere ponderala recenta sau anemie feripriva cronica inexplicabila<sup>1;3;5:6<\/sup>.<\/p>\n<p><strong><em>Pregatire pacient <\/em><\/strong><em>- <\/em>nu este necesara o pregatire speciala (fiind vorba de un test immunologic, nu este necesara respectarea unei diete inaintea recoltarii)<sup>2<\/sup>.<\/p>\n<p><strong><em>Specimen recoltat <\/em><\/strong><em>- <\/em>materii fecale recoltate in orice moment al zilei.<\/p>\n<p><span style=\"color: #ff0000;\"><span style=\"text-decoration: underline;\">ATENTIE !<\/span> <\/span>75% din sangele aflat la suprafata bolului fecal difuzeaza in apa vasului WC, in 4-12 minute (reactii fals negative). Sangele menstrual sau urina hematurica pot contamina bolul fecal (reactii fals pozitive)<sup>2<\/sup>.<\/p>\n<p><strong><em>Recipient de recoltare <\/em><\/strong><em>- <\/em>recipient de plastic de unica folosinta pentru\u00a0 fecale<sup>2<\/sup>.<\/p>\n<p><strong><em>Cantitate recoltata <\/em><\/strong><em>-<\/em> se recolteaza 3 portiuni cat o aluna din puncte diferite ale bolului fecal<sup>2<\/sup>.<\/p>\n<p><strong><em>Prelucrare necesara dupa recoltare <\/em><\/strong>\u2013 se lucreaza in aceeasi zi<sup>2<\/sup>.<\/p>\n<p><strong><em>Stabilitate proba <\/em><\/strong><em>-<\/em> 24 ore\u00a0la temperatura de 2\u00b0C \u2013 8\u00b0C.<\/p>\n<p><strong><em>Metoda<\/em><\/strong> - <span style=\"text-decoration: underline;\">imunocromatografica<\/span><sup>2<\/sup>.<\/p>\n<p><strong><em>Valori de referinta <\/em><\/strong><em>- <\/em>absenta hemoragiei (test negativ)<sup>2<\/sup>.<\/p>\n<p><strong><em>Limite<\/em><\/strong> <strong><em>si interferente<\/em><\/strong><\/p>\n<p>Testul este folosit drept screening pentru cancerul colorectal; totusi, nu orice sangerare colorectala se datoreaza polipilor cancerosi sau precancerosi.<\/p>\n<p>Rezultatele negative nu exclud sangerarea, care poate fi intermitenta<sup>2:5<\/sup>.<\/p>\n<p><span style=\"text-decoration: underline;\">Reactii fals pozitive<\/span>: contaminarea fecalelor cu sange menstrual sau cu urina hematurica.<\/p>\n<p><span style=\"text-decoration: underline;\">Reactii fals negative<\/span>: difuzarea sangelui din bolul fecal in apa din WC<sup>2<\/sup>.<\/p>\n<p>\u2022 Medicamente<\/p>\n<p><span style=\"text-decoration: underline;\">Reactii pozitive<\/span>: medicamente care produc sangerare gastrointestinala: aspirina, corticosteroizi, antiinflamatoare nesteroidiene (indometacin, ibuprofen, sulindac); medicamente care produc colita (ex.: metildopa si unele antibiotice)<sup>2:4<\/sup>.<\/p>\n<p>\u2022 Interferente analitice<\/p>\n<p>Testul nu prezinta nici o reactie incrucisata cu hemoglobina de porc, vaca, iepure, cal sau oaie<sup>2<\/sup>.<\/p>\n<p><span style=\"font-size: small;\">\u00a0<\/span><\/p>\n<p><span style=\"font-size: small;\">\u0411\u0438\u0431\u043b\u0438\u043e\u0433\u0440\u0430\u0444\u0438\u044f<\/span><\/p>\n<p><span style=\"font-size: small;\">1.
