FSH \u015fi LH sunt secreta\u0163i pulsatil de hipofiza anterioar\u0103, ca r\u0103spuns la eliberarea intermitent\u0103 de <em>gonadotropin releasing hormone<\/em> (GnRH) de la nivelul hipotalamusului. Nivelul sanguin al hormonilor gonadotropi este controlat de hormonii sexuali (estrogen, progesteron, testosteron etc.) prin feedback negativ pe hipotalamus<sup>4<\/sup>.<\/p>\n<p style=\"text-align: justify;\">La b\u0103rba\u0163i FSH stimuleaz\u0103 spermatogenez\u0103 (un nivel sc\u0103zut de hormoni gonadotropi poate cauza azoospermie). La femei FSH, al\u0103turi de LH, stimuleaz\u0103 cre\u015fterea \u015fi maturarea foliculilor \u015fi biosinteza estrogenului la nivel folicular. Nivelul de FSH are un maxim \u00een perioada de mijloc a ciclului menstrual (faza ovulatorie), \u00eens\u0103 cu o cre\u015ftere mai mic\u0103 dec\u00e2t\u00a0\u00een cazul LH<sup>5;6;8<\/sup>.<\/p>\n<p style=\"text-align: justify;\">\u00cen mod simplist, nivelul de FSH \u015fi LH este crescut \u00een cazurile \u00een care hormonii sexuali au o produc\u0163ie deficitar\u0103 (hipogonadism, sindrom Klinefelter, anorhidie, sindromul testiculului feminizant, alcoolism, castrare, menopauz\u0103). Nivelul de FSH \u015fi LH este sc\u0103zut \u00een insuficien\u0163\u0103 hipofizar\u0103 sau hipotalamic\u0103, tumori testiculare sau suprarenaliene care influen\u0163eaz\u0103 secre\u0163ia de estrogeni sau androgeni.<\/p>\n<p style=\"text-align: justify;\">Cre\u015fterea nivelului bazal de LH, cu un raport LH\/FSH &gt;2, la o pacient\u0103 cu anovula\u0163ie cronic\u0103 este sugestiv\u0103 pentru ovarul polichistic<sup>5;6;8<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Recomand\u0103ri pentru determinarea FSH<\/em><\/strong><\/p>\n<p style=\"text-align: justify;\"><span style=\"text-decoration: underline;\">La b\u0103rba\u0163i<\/span>: elucidarea disfunc\u0163iilor axului hipotalamo-hipofizo-gonadic (\u00een principal pentru diagnosticul diferen\u0163ial \u00eentre insuficien\u0163a testicular\u0103 primar\u0103 \u015fi deficitul de stimulare testicular\u0103), afec\u0163iuni genetice cu abera\u0163ii cromozomiale (ex.: sindrom Klinefelter), definirea disfunc\u0163iilor testiculare, sindromul testiculului feminizant<sup>3;5;6<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><span style=\"text-decoration: underline;\">La femei<\/span>: elucidarea disfunc\u0163iilor axului hipotalamo-hipofizo-gonadic (\u00een principal pentru diagnosticul diferen\u0163ial \u00eentre insuficien\u0163a ovarian\u0103 primar\u0103 \u015fi deficitul de stimulare ovarian\u0103), ovarul polichistic, tulbur\u0103ri menstruale (inclusiv amenoreea), definirea fazelor ciclului menstrual\u00a0\u00een cursul evalu\u0103rii infertilit\u0103\u0163ii, evaluarea sindromului menopauzal<sup>3;5;6<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Preg\u0103tire pacient <\/em><\/strong><em>- <\/em>\u00e0 jeun (pe nem\u00e2ncate). Alcoolismul determin\u0103 cre\u015fteri ale FSH; hemocromatoza \u015fi anorexia nervoas\u0103 cauzeaz\u0103 reducerea nivelului FSH<sup>1<\/sup>.<\/div>\n<p style=\"text-align: justify;\">\u2022 Medicamente<\/p>\n<p style=\"text-align: justify; padding-left: 30px;\"><span style=\"text-decoration: underline;\"><strong>Cre\u015fteri<\/strong><\/span>: bromocriptina, cimetidina, clomifen, digitala, eritropoietina, fenitoin, hidrocortizon, ketoconazol, levodopa, naloxon<sup>2<\/sup>.<\/p>\n<p style=\"text-align: justify; padding-left: 30px;\"><span style=\"text-decoration: underline;\"><strong>Sc\u0103deri<\/strong><\/span>: carbamazepin, corticosteroizi, estrogeni (efectele depind de doza \u015fi durata administr\u0103rii: dozele mari determin\u0103 sc\u0103deri ale FSH, iar dozele mici pot determina cre\u015fteri ale FSH), analogi ai GnRH (<em>gonadotropin releasing hormone<\/em>) \u00een administrare continu\u0103, megestrol, contraceptive orale, fenotiazine, stanozolol<sup>2<\/sup>.<\/p>\n<p style=\"text-align: justify; padding-left: 30px;\"><span style=\"text-decoration: underline;\"><strong>Efecte variabile<\/strong><\/span>: danazol, finasterid, pravastatin, tamoxifen<sup>2;7;8<\/sup>.<\/p>\n<p style=\"text-align: justify;\">\u2022 Interferen\u0163e analitice<\/p>\n<p style=\"text-align: justify;\">Pot produce interferen\u0163e cu unele componente ale kit-ului \u015fi conduce la rezultate neconcludente urm\u0103toarele:<\/p>\n<div style=\"text-align: justify; padding-left: 30px;\">\u2013 tratamentul cu biotin\u0103 \u00een doze mari (&gt;5 mg\/zi); de aceea se recomand\u0103 ca recoltarea de s\u00e2nge s\u0103 se fac\u0103 dup\u0103 minimum 8 ore de la ultima administrare;<\/div>\n<div style=\"text-align: justify; padding-left: 30px;\">\u2013 titrurile foarte crescute de anticorpi anti-streptavidin\u0103 \u015fi anti-ruteniu<sup>7<\/sup>.<\/div>\n<p style=\"text-align: justify;\"><span style=\"font-size: small;\">\u00a0<\/span><strong><span style=\"font-size: small;\">\u0411\u0438\u0431\u043b\u0438\u043e\u0433\u0440\u0430\u0444\u0438\u044f<\/span><\/strong><\/p>\n<div style=\"text-align: justify;\"><span style=\"font-size: small;\">1.
