Desi se poate face o evaluare rezonabila in privinta originii hepatice sau non-hepatice a cresterii fosfatazei alcaline utilizand doar date clinice, exista si metode biochimice ce pot face diferentierea intre izoenzime<sup>1;3<\/sup> .<\/p>\n<p><strong><em>Recomandari pentru determinarea fosfatazei alcaline<\/em><\/strong><\/p>\n<p>Determinarea fosfatazei alcaline este de obicei folosita pentru diagnosticul diferential al bolilor hepatice. Alta arie de utilizari clinice include afectiunile osoase, fiind la ora actuala singura enzima cu importanta practica pentru patologia tesutului osos, si in hiperparatiroidism. In tumorile de diverse etiologii fosfataza alcalina are valoare de marker tumoral (depistarea metastazelor hepatice sau osoase)<sup>1;5<\/sup>.<\/p>\n<p><strong><em>Pregatire pacient<\/em><\/strong> \u2013 \u00e0 jeun (pe nemancate)<sup>4<\/sup>.<\/p>\n<p><strong><em>Specimen recoltat<\/em><\/strong> \u2013 sange venos<sup>4<\/sup>.<\/p>\n<p><strong><em>Recipient de recoltare <\/em><\/strong>\u2013 vacutainer fara anticoagulant cu\/fara gel separator<sup>4<\/sup>.<\/p>\n<p><strong><em>Prelucrare necesara dupa recoltare<\/em><\/strong> \u2013 se separa serul prin centrifugare<sup>4<\/sup>.<\/p>\n<p><strong><em>Volum proba<\/em><\/strong> \u2013 minim 0.5 mL ser<sup>4<\/sup>.<\/p>\n<p><strong><em>Cauze\u00a0 de respingere a probei<\/em><\/strong> \u2013 specimen intens hemolizat, lipemic sau contaminat bacterian<sup>4<\/sup>.<\/p>\n<p><strong><em>Stabilitate proba <\/em><\/strong>\u2013 serul separat este stabil <em>7 zile<\/em> la temperatura camerei; hipertiroidism; boala Paget; metastaze osoase; tumori osoase; osteomalacie, rahitism; in evolutia fracturilor multiple<sup>1<\/sup>.<\/p>\n<p>\u2022\u00a0 <em>Valori crescute de origine placentara<\/em>: apar in saptamanile 16-20 ale unei sarcini normale, crescand pana la nivelul maxim (de 2 ori normalul) la debutul travaliului<sup>5<\/sup>.<\/p>\n<p>\u2022 <em>Valori crescute de origine intestinala<\/em>: in diverse afectiuni ulcerative ale tractului gastrointestinal, malabsorbtie severa, infarct intestinal.<\/p>\n<p>\u2022 <em>Valori crescute in neoplasme<\/em>: ocazional tumorile (in special hipernefromul) pot produce o fosfataza alcalina identica sau similara cu forma placentara (izoenzima Regan)<sup>5<\/sup>.<strong><em>\u00a0<\/em><\/strong><\/td>\n<\/tr>\n<tr>\n<td width=\"631\">\n<p style=\"text-align: center;\"><strong><em>Scaderi<\/em><\/strong><\/p>\n<\/td>\n<\/tr>\n<tr>\n<td valign=\"top\" width=\"631\">\n<p style=\"text-align: center;\">\u2022 hipofosfatazia familiala; \u2022 hipotiroidism, cretinism; \u2022 malnutritie; \u2022 deficit de zinc; \u2022 deficit de magneziu; \u2022 femeile aflate in post menopauza cu osteoporoza care urmeaza tratament estrogenic substitutiv<sup>1;3<\/sup>.<\/p>\n<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p><strong><em>Limite si interferente<\/em><\/strong><\/p>\n<p>Valoarea ALP interpretata izolat poate crea confuzii de diagnostic. Determinarea GGT reprezinta un test suplimentar util, pentru a confirma originea hepato-biliara a fosfatazei alcaline crescute in prezenta unor valori normale ale transaminazelor.<\/p>\n<p>\u2022 Medicamente<\/p>\n<p><span style=\"text-decoration: underline;\"><strong>Cresteri<\/strong><\/span><\/p>\n<p><em>Medicamente care produc colestaza<\/em>: acid aminosalicilic, amitriptilina, steroizi anabolizanti, androgeni, azatioprina, benzodiazepine, carbamazepin, carbazona, clorotiazida, clorpropamida, acid clavulanic, dapsona.<\/p>\n<p><em>Medicamente care produc afectare hepatocelulara<\/em>: acetaminofen, alopurinol, acid aminosalicilic, amiodarona, amitriptilina, steroizi anabolizanti, androgeni, asparaginaza, aspirina, azatioprina, carbamazepin, chenodiol, clorambucil, cloramfenicol, clorpropamida, cimetidina, ciclosporina, danazol, dantrolen, dapsona, diclofenac, dicumarol (rar), disulfiram, eritromicina, estrogeni, etionamida, gliburid (glibenclamid), saruri de aur, imipramina, mercaptopurina, metimazol, acid nicotinic, nitrofurantoin, contraceptive orale, estrogeni, etanol (exces), etionamida, fluconazol, halotan, ibuprofen, imipramina, indometacin, saruri de fier (supradozare), izoniazida, ketoconazol, inhibitori de MAO, mercaptopurina, metotrexat, metoxifluran, metildopa, naproxen, papaverina, parametadion, penicilamina, peniciline, fenotiazine, fenilbutazona, progestative, propoxifen, sulfonamide, sulfone, sulindac, tamoxifen, tolbutamida, perhexilin, fenazopiridina, fenidiona, fenobarbital, fenilbutazona, fenitoin, plicamicina (mitramicina), probenecid, procainamida, propiltiouracil, pirazinamida, chinidina, rifampicina, salicilati, sulfasalazina, sulfonamide, tamoxifen, tetracicline, trimetadiona, acid valproic, vitamina A, warfarina (rar).<\/p>\n<p><em>Multe alte medicamente pot determina cresteri<\/em> care, in general, sunt tranzitorii, dar in anumite cazuri indica hepatotoxicitate. Acestea includ: acebutolol, aminoglutetimid, aminoglicozide, bromocriptina, carboplatin, captopril, cefalosporine, clindamicin, clotrimazol, colchicina, ciclosporina, citarabina, dapsona,\u00a0 desipramina, dicumarol, didanosin, disopiramid, enalapril, etambutol, etopozid, filgrastim, flucitozin, foscarnet, ganciclovir, gentamicina, interferon, izotretinoin, ketoconazol, labetolol, levamisol, lincomicina, mebenzadol, mefenitoin, nifedipin, antiinflamatoare nesteroidiene (ex.: ibuprofen, naproxen), omeprazol, ondansetron, peniciline, fenitoina, propoxifen, protriptilina, streptozocin, sulfoniluree, tioguanina, ticlopidina, verapamil, zalcitabin<sup>2<\/sup>.<\/p>\n<p><span style=\"text-decoration: underline;\"><strong>Scaderi<\/strong><\/span>: acyclovir, azatioprina, calcitriol, ciclosporina, contraceptive orale, clofibrat, danazol, estrogeni, pamidronat, penicilamina, prednisolon, prednison, tamoxifen, ursodiol, vitamina D<sup>2<\/sup>.<\/p>\n<p>\u2022 Interferente analitice<\/p>\n<p>Hemoliza intensa poate genera valori fals scazute ale fosfatazei alcaline<sup>4<\/sup>.<\/p>\n<p><span style=\"font-size: small;\">\u00a0<\/span><\/p>\n<p><span style=\"font-size: small;\">\u0411\u0438\u0431\u043b\u0438\u043e\u0433\u0440\u0430\u0444\u0438\u044f<\/span><\/p>\n<p><span style=\"font-size: small;\">1.
