Testul Waaler-Rose are la baz\u0103 o reac\u0163ie de hemaglutinare, fiind utilizat pentru detec\u0163ia\u00a0semicantitativ\u0103 a factorului reumatoid (FR). Antigenul este reprezentat de o suspensie de eritrocite de oaie stabilizate \u015fi sensibilizate cu gamaglobuline de iepure anti-eritrocite de oaie, care aglutineaz\u0103 pe card \u00een prezen\u0163a factorului reumatoid din serul pacien\u0163ilor.<\/p>\n

Sensibilitatea testului este\u00a0ajustat\u0103 astfel \u00eenc\u00e2t s\u0103 detecteze aproximativ 6 UI\/mL dintr-un material interna\u0163ional de referin\u0163\u0103 (OMS), provenit din serul unui pacient cu poliartrit\u0103 reumatoid\u0103.<\/p>\n

Preg\u0103tire pacient <\/em><\/strong>\u2013 \u00e0 jeun (pe nem\u00e2ncate).<\/p>\n

Cauze\u00a0de respingere a probei <\/em><\/strong>\u2013 specimen intens lipemic sau intens hemolizat.<\/p>\n

Recipient de recoltare <\/em><\/strong>\u2013 vacutainer f\u0103r\u0103 anticoagulant cu\/f\u0103r\u0103 gel separator.<\/p>\n

Prelucrare necesar\u0103 dup\u0103 recoltare <\/em><\/strong>\u2013 se separ\u0103 serul prin centrifugare \u015fi se lucreaz\u0103 imediat; dac\u0103 acest lucru nu este posibil, serul va fi p\u0103strat la 2-8\u00baC sau la \u201320\u00baC.<\/p>\n

Volum prob\u0103<\/em><\/strong> \u2013 minim 1 mL ser.<\/p>\n

Interpretarea rezultatelor<\/em><\/strong><\/p>\n

Ob\u0163inerea unei aglutin\u0103ri vizibile macroscopic indic\u0103 prezen\u0163a factorului reumatoid \u00een titru \u2265 6 UI\/mL. \u00cen cazul ob\u0163inerii unui rezultat pozitiv se recurge la dilu\u0163ii succesive ale serului, fiind raportat\u0103 cea mai mare dilu\u0163ie la care se observ\u0103 aglutinare.\u00a0<\/em><\/strong><\/p>\n

Limite \u015fi interferen\u0163e<\/em><\/strong><\/p>\n

Diagnosticul nu trebuie s\u0103 se bazeze pe un singur rezultat pozitiv ob\u0163inut la acest test, ci trebuie corelat cu date clinice \u015fi cu determinarea latex-imunoturbidimetric\u0103 a factorului reumatoid. Reactivitatea cunoscut\u0103 a factorului reumatoid at\u00e2t cu IgG provenite de la iepure, c\u00e2t \u015fi cu IgG umane, adic\u0103 rezultate pozitive at\u00e2t la testul Waaler-Rose, c\u00e2t \u015fi prin latex-imunoturbidimetrie, este considerat\u0103 a fi tipic\u0103 \u00een special pentru factorul reumatoid din poliartrita reumatoid\u01032<\/sup>.<\/div>\n
\u00cen crioglobulinemia mixt\u0103 tipul II, caracterizat\u0103 \u00een general prin prezen\u0163a de factor reumatoid monoclonal tip IgM kappa, specificitatea acestuia nu este \u00eentotdeauna identic\u0103 cu cea a factorului reumatoid asociat cu artrita reumatoid\u0103 \u015fi astfel acesta nu reac\u0163ioneaz\u0103 uneori cu IgG de iepure, iar testul Waaler-Rose este negativ2<\/sup>.<\/div>\n
Cele 2 metode de detec\u0163ie a factorului reumatoid, av\u00e2nd principii diferite, nu sunt comparabile.<\/div>\n
Complexele imune circulante prezente \u00een serul pacientului pot produce rezultate eronate.<\/div>\n
De asemenea, inciden\u0163a rezultatelor pozitive la subiec\u0163i aparent s\u0103n\u0103to\u015fi este de 3-5%.<\/div>\n
Reac\u0163ii pozitive pot fi ob\u0163inute nu numai \u00een poliartrita reumatoid\u0103, dar \u015fi \u00een cazuri de mononucleoz\u0103 infec\u0163ioas\u0103, hepatit\u0103, sifilis.<\/div>\n
