De obicei sunt anticorpi de tip IgM, dar pot fi \u015fi de tip IgG sau IgA<sup>1;4<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Nivelul seric al factorului reumatoid este crescut \u00een peste 70% din cazurile de poliartrita reumatoid\u0103, av\u00e2nd un rol important \u00een producerea manifest\u0103rilor extraarticulare. Factorul reumatoid precede debutul bolii uneori cu mai mul\u0163i ani. In prezen\u0163a sa, riscul de apari\u0163ie a artritei reumatoide este de 5-40 de ori mai mare dec\u00e2t la persoanele seronegative<sup>5<\/sup>. Nu este implicat \u00een ini\u0163ierea procesului inflamator care cauzeaz\u0103 artrita reumatoid\u0103, dar probabil determin\u0103 perpetuarea \u015fi amplificarea acestuia.<\/p>\n<p style=\"text-align: justify;\">\u00cen func\u0163ie de prezen\u0163a sau absen\u0163a factorului reumatoid, poliartrita reumatoid\u0103 este clasificat\u0103 \u00een 2 categorii: seropozitiv\u0103 sau seronegativ\u0103. Pacien\u0163ii seropozitivi au un risc mai mare dec\u00e2t cei seronegativi de a dezvolta artrit\u0103 eroziv\u0103 cu impoten\u0163\u0103 func\u0163ional\u0103 articular\u0103 \u015fi manifest\u0103ri extraarticulare cum ar fi: noduli reumatoizi, determinare pulmonar\u0103 de boal\u0103, vasculit\u0103, sindrom Felty (neutropenie \u015fi splenomegalie asociate cu artrit\u0103 reumatoid\u0103), polineuropatie, serozite \u015fi sindrom Sj\u00f6gren secundar.<\/p>\n<p style=\"text-align: justify;\">Factorul reumatoid nu este specific pentru poliartrita reumatoid\u0103, fiind \u00eent\u00e2lnit \u015fi \u00een alte boli ale \u0163esutului conjunctiv, precum \u015fi \u00een infec\u0163ii cronice (endocardit\u0103, tuberculoz\u0103, hepatita B)<sup>1<\/sup>. Totu\u015fi exist\u0103 doar un num\u0103r mic de pacien\u0163i cu artrit\u0103 reumatoid\u0103 care au factor reumatoid de tip IgG sau IgA f\u0103r\u0103 factor reumatoid tip IgM. Pe de alt\u0103 parte, un rezultat pozitiv pentru toate cele trei tipuri de Ig este considerat specific pentru artrita reumatoid\u0103. De asemenea, \u00een multe cazuri, titruri mari de factor reumatoid IgG sau IgA se asociaz\u0103 cu progresia leziunilor erozive, iar manifest\u0103rile extra-articulare se consider\u0103 c\u0103 sunt asociate \u00een primul r\u00e2nd cu factor reumatoid tip IgA<sup>5<\/sup>.<\/p>\n<div style=\"text-align: justify;\">Absen\u0163a factorului reumatoid nu exclude diagnosticul de poliartrit\u0103 reumatoid\u0103, \u00een special dac\u0103 s-a efectuat o singur\u0103 determinare. Un marker mai sensibil \u015fi mai specific sunt anticorpii anti-CCP<sup>2<\/sup>.<\/div>\n<div style=\"text-align: justify;\">Niveluri u\u015for crescute ale factorului reumatoid pot fi \u00eent\u00e2lnite \u00eentr-un procent crescut (p\u00e2n\u0103 la 20%) la persoanele &gt;60 ani, \u00een absen\u0163a bolii.<\/div>\n<div style=\"text-align: justify;\">Dup\u0103 unele vaccin\u0103ri se poate \u00eenregistra o cre\u015ftere tranzitorie a factorului reumatoid<sup>1<\/sup>.<\/div>\n<div style=\"text-align: justify;\">\u00cen cazuri foarte rare gamapatiile monoclonale, \u00een special tip IgM (macroglobulinemia Waldenstr\u00f6m) poate cauza rezultate eronate<sup>3<\/sup>.<\/div>\n<p style=\"text-align: justify;\"><span style=\"font-size: small;\">\u00a0<\/span><\/p>\n<p style=\"text-align: justify;\"><strong><span style=\"font-size: 10pt;\">\u0411\u0438\u0431\u043b\u0438\u043e\u0433\u0440\u0430\u0444\u0438\u044f<\/span><\/strong><\/p>\n<div style=\"text-align: justify;\"><span style=\"font-size: small;\">1. <span style=\"font-size: 10pt;\">Francesco Dati &amp; Erwin Metzmann.
