Sunt impartite in patru clase principale in functie de marime (dupa greutatea moleculara), densitate (separare prin ultracentrifugare si flotatie) si incarcatura electrica (separare prin electroforeza).<\/p>\n<p>Clasele de lipoproteine sunt urmatoarele:<\/p>\n<ul>\n<li><span style=\"text-decoration: underline;\">Chilomicronii<\/span>: diametru 5000\u00c5, densitate intre 0,940-0,980; se depun in zona \u03b3 a lipoproteinelor; constituiti in proportie de 90% din trigliceride; cresc mult postprandial, fiind generati ca urmare a ingestiei si absobtiei intestinale a lipidelor alimentare; dau opalescenta serului; reprezinta 0-2% din cantitatea de lipoproteine.<\/li>\n<li><span style=\"text-decoration: underline;\">\u03b2<\/span><span style=\"text-decoration: underline;\">-Lipoproteine sau LDL (Low Density Lipoproteins):<\/span> densitate intre 1,019-1,063; migreaza in zona \u03b2-globulinelor; transporta lipoproteinele de densitate joasa spre tesuturi periferice si in particular spre peretele arterial; sunt bogate in colesterol (45%), iar cresterea acestora este asociata cu riscul cardiac \u2013 \u201ccolesterol nociv\u201d; reprezinta\u00a0 aproximativ 33-70% din totalul lipoproteinelor.<\/li>\n<li><span style=\"text-decoration: underline;\">Pre<\/span><span style=\"text-decoration: underline;\">\u03b2<\/span><span style=\"text-decoration: underline;\">-Lipoproteinele sau VLDL<\/span> (Very Low Density Lipoproteins): densitateintre 0,95-1,006; sunt mai mobile decat lipoproteinele \u03b2, migreaza la nivelul \u03b12-globulinelor,transporta trigliceride, fosfolipide, colesterol (15%); sintetizate in ficat fiinddenumite si chilomicroni endogeni;reprezinta aproximativ 7-24% din lipoproteine.<\/li>\n<\/ul>\n<ul>\n<li><span style=\"text-decoration: underline;\">\u03b1<\/span><span style=\"text-decoration: underline;\">-Lipoproteine sau HDL (High Density Lipoproteins):<\/span> densitate intre 1,063-1,210; transporta proteine, fosfolipide, colesterol (18%, in special esterificat); asigura intoarcerea la ficat (pentru a fi metabolizat) a colesterolului din tesuturi, si mai ales din peretele vascular -\u201dcolesterol protector\u201d; indicator pentru estimarea riscului cardiovascular; reprezinta aproximativ 10-30% din totalul lipoproteinelor.<\/li>\n<\/ul>\n<p>Pe langa aceste 4 tipuri principale\u00a0 mai sunt descrise urmatoarele lipoproteine:<\/p>\n<ul>\n<li><span style=\"text-decoration: underline;\">Pre<\/span><span style=\"text-decoration: underline;\">\u03b1<\/span><span style=\"text-decoration: underline;\">-Lipoproteine<\/span>: densitate &gt; 1,210; migreaza la nivelul albuminei; contin acizi grasi liberi, iar cantitatea lor aproximativa este de 8-13%.<\/li>\n<li>O fractiune suplimentara intre \u03b1 si pre \u03b2 (intre VLDL si HDL) reprezentata de <span style=\"text-decoration: underline;\">lipoproteina A-Lp(a)<\/span> a carei mobilitate variaza in functie de fenotip; formata din fosfolipide, colesterol si apoproteina\u00a0 B100, asemanatoare cu molecula plasminogenului; introduce LDL in celule in absenta receptorilor; reprezinta un factor de risc independent pentru procesul de ateroscleroza;\u00a0 cantitate normala: &lt;30 mg\/dl.<\/li>\n<li><span style=\"text-decoration: underline;\">Lipoproteine cu densitate intermediara IDL (Intermediar Density Lipoproteins):<\/span> densitate intre 1,004-1,010; se formeaza in plasma circulanta ca urmare a eliberarii enzimatice a trigliceridelor din chilomicroni si VLDL; cresc mult postprandial si apar in serul bolnavilor cu dislipidemie tip III (\u201cBroad Beta Disease\u201d \u2013 BBD).<\/li>\n<\/ul>\n<p>Cazuri particulare\u00a0:<\/p>\n<p style=\"padding-left: 30px;\">-Fractiunea pre\u03b2 crescuta poate masca Lp(a); de aceea, se lasa serul 24-48h la temperatura ambianta dupa care se reincepe analiza.<\/p>\n<p style=\"padding-left: 30px;\">-<span style=\"text-decoration: underline;\">Lipoproteina X<\/span>: inlocuirea chilomicronilor cu o forma patologica de lipoproteina ce migreaza la catod; structura asemanatoare LDL (contine trigliceride si colesterol liber in cantitate crescuta); apare in colestaza si in absenta congenitala a enzimei LCAT (lecitin-colesterol-aciltransferaza)<sup>1;2;3;5<\/sup>.<\/p>\n<p><strong><em>Recomandari pentru efectuarea electroforezei lipoproteinelor <\/em><\/strong>\u2013 suspiciune de tezaurismoze lipidice si identificarea dislipidemiilor familiale; arc cornean precoce (pacienti sub 40 ani); pacienti cu xantelasme si xantoame; valoare a trigliceridelor &gt; 300 mg \/dl; ser lipemic \u00e0 jeun; complicatii cardiovasculare (afectiuni coronariene si istoric familial de boala coronariana prematura, arterita a membrelor inferioare, accidente vasculare cerebrale, insuficienta circulatorie in teritoriul vertebro-bazilar, HTA); afectiuni metabolice (diabet zaharat, obezitate, hiperuricemie, hipotiroidism); alte afectiuni: sindrom nefrotic, alcoolism cronic; administrare de contraceptive orale<sup>2;3<\/sup>.<\/p>\n<p><strong><em>Pregatire pacient <\/em><\/strong><em>-<\/em> pacientul trebuie sa respecte o dieta constanta timp de 2-3 saptamani; inainte de recoltare va ajuna 12-14 ore<sup>5;6<\/sup>.<\/p>\n<p><strong><em>Specimen recoltat <\/em><\/strong><em>-<\/em> sange venos<sup>6<\/sup>.<\/p>\n<p><strong><em>Recipient de recolta <\/em><\/strong><em>- <\/em>vacutainer fara anticoagulant cu\/fara gel separator<sup>6<\/sup>.<\/p>\n<p><strong><em>Prelucrare necesara dupa recoltare <\/em><\/strong><em>-<\/em> se separa serul prin centrifugare;
