Cuprul este un component esen\u0163ial al unor sisteme enzimatice implicate \u00een producerea hemoglobinei, metabolismul glucidic, biosinteza catecolaminelor \u015fi formarea leg\u0103turilor \u00eencruci\u015fate (\u201ccross-linking\u201d) \u00eentre fibrele de colagen, elastina \u015fi keratin\u0103 din firul de p\u0103r.<\/span><\/p>\n

Deficitul de cupru determin\u0103 anemie, neutropenie \u015fi tulbur\u0103ri de cre\u015ftere, \u00een special la copii.<\/span><\/p>\n

Aportul de cupru se realizeaz\u0103 \u00een principal prin diet\u0103; absorb\u0163ia\u00a0se efectueaz\u0103 cu usurin\u0163\u0103, dar este limitat\u0103 de mecanismele homeostatice dup\u0103 atingerea necesarului. Dup\u0103 absorb\u0163ie cuprul este legat de albumin\u0103 \u015fi ceruloplasmin\u0103, iar apoi este depozitat\u00a0\u00een ficat \u015fi mai pu\u0163in\u00a0\u00een rinichi. Principala cale de eliminare a cuprului este cea biliar\u0103, excre\u0163ia urinar\u0103 fiind mic\u01032;4<\/sup>.<\/span><\/p>\n

Boala Wilson este o afec\u0163iune autozomal-recesiv\u0103 caracterizat\u0103 prin perturbarea homeostaziei normale a cuprului: alterarea excre\u0163iei biliare a cuprului, reten\u0163ie excesiv\u0103 a cuprului \u00een ficat, sc\u0103derea concentra\u0163iei ceruloplasminei \u015fi cre\u015fterea excre\u0163iei urinare a cuprului. Clinic se manifest\u0103 prin leziuni hepatice, renale, neurologice, corneene \u015fi anemie hemolitic\u0103.<\/span><\/p>\n

Sindromul Menkes este o afec\u0163iune multisistemic\u0103 letal\u0103 asociat\u0103 cu simptome neurodegenerative \u015fi anomalii ale \u0163esutului conjunctiv, care are drept cauza deficitul uneia sau mai multor enzime dependente de cupru2<\/sup>.<\/span><\/p>\n

Inhalarea cronic\u0103 a particulelor de cupru care se formeaz\u0103 \u00een cursul diverselor procese de prelucrare (metalurgie, placarea obiectelor cu cupru, sudur\u0103) poate determina grea\u0163\u0103, v\u0103rs\u0103turi, hepatomegalie, manifest\u0103ri neurologice. Expunerea acut\u0103 conduce la irita\u0163ia c\u0103ilor respiratorii superioare, febr\u0103 \u015fi dureri musculare4<\/sup>.<\/span><\/p>\n

Recomand\u0103ri pentru determinarea cuprului<\/em><\/strong><\/span><\/p>\n

-cuprul \u00een plasm\u0103: suspiciune de boala Wilson (se testeaz\u0103 \u00eempreun\u0103 cu ceruloplasmina); monitorizarea alimenta\u0163iei enterale sau parenterale; intoxica\u0163ii acute;<\/span><\/p>\n

-cuprul urinar: suspiciune de boala Wilson, monitorizarea intoxica\u0163iei cronice2;4<\/sup>.<\/span><\/p>\n

Preg\u0103tire pacient <\/em><\/strong>\u2013 momentul recolt\u0103rii probei de s\u00e2nge nu este important pentru monitorizarea expunerii profesionale3;4<\/sup>.<\/span><\/p>\n

Specimen recoltat <\/em><\/strong>\u2013 1) s\u00e2nge venos; 2) o prob\u0103 de urin\u0103 spontan\u0103.<\/span><\/p>\n

Recipient de recoltare <\/em><\/strong>\u2013 1) vacutainer pentru metale ce con\u0163ine EDTA sau heparinat de litiu ca anticoagulant; 2) eprubeta pentru urin\u0103.

Interpretarea rezultatelor<\/em><\/strong>4<\/sup><\/span><\/p>\n

Concentra\u0163ia cuprului seric (plasmatic) reflect\u0103 at\u00e2t cuprul legat de ceruloplasmina, c\u00e2t \u015fi cuprul \u201cliber\u201d legat mai slab de albumin\u0103 sau peptide circulante mici. Ceruloplasmina este un reactant de faz\u0103 acut\u0103 \u015fi, deoarece aceast\u0103 protein\u0103 leag\u0103 o cantitate mare de cupru, at\u00e2t cuprul seric, c\u00e2t \u015fi ceruloplasmina cresc \u00een condi\u0163iile asociate cu inflama\u0163ie (ex. poliartrita reumatoida), sarcin\u0103 \u015fi tratament cu estrogeni. Pe de alt\u0103 parte, nivelul cuprului seric scade \u00een afec\u0163iuni \u00eenso\u0163ite de hipoproteinemie: nefroz\u0103, malnutri\u0163ie, malabsorb\u0163ie, f\u0103r\u0103 s\u0103 reflecte neap\u0103rat reducerea depozitelor hepatice de cupru. De asemenea se mai \u00eenregistreaz\u0103 o sc\u0103dere a cuprului seric \u00een cursul tratamentului cu ACTH, glucocorticoizi \u015fi valproat.<\/span><\/p>\n
