Cel mai important minereu de crom este cromitul. Cromul si sarurile sale anorganice prezinta numeroase aplicatii industriale: placarea metalelor, producerea de aliaje diverse, materiale refractare, pigmenti si conservanti<sup>2<\/sup>.<\/span><\/p>\n<p><span style=\"font-size: medium;\">Cromul in stare trivalenta este cea mai abundenta forma de crom din mediu. Fiziologic, cromul detine un rol important in metabolismul glucozei<sup>2<\/sup> si de asemenea in sinteza colesterolului<sup>1<\/sup>.\u00a0 Scaderea cromului in organism este asociata cu rezistenta la insulina (hiperinsulinism), risc crescut de insuficienta cardiaca congestiva, hipercolesterolemie, reducerea fertilitatii<sup>1<\/sup>. Desi cromul trivalent in sine are o biodisponibilitate orala redusa, absorbtia sa intestinala este crescuta in prezenta \u201cfactorului de toleranta la glucoza\u201d, care formeaza un complex cu Cr<sup>3+<\/sup>. Cromul trivalent este considerat a fi relativ netoxic <em>in vivo<\/em><sup>2<\/sup>.<\/span><\/p>\n<p><span style=\"font-size: medium;\">Fierul inhiba competitiv legarea cromului de transferina, astfel ca pacientii cu hemocromatoza prezinta o retentie scazuta de crom in organism. S-a formulat ipoteza ca deficienta cromului la nivel celular ar avea un rol in dezvoltarea diabetului la pacientii cu hemocromatoza<sup>4<\/sup>.<\/span><\/p>\n<p><span style=\"font-size: medium;\">Pe de alta parte, cromul hexavalent reprezinta forma cea mai importanta de crom din punct de vedere toxicologic. Spre deosebire de Cr<sup>3+<\/sup>, Cr<sup>6+<\/sup> este absorbit cu usurinta de majoritatea tesuturilor, astfel ca ingestia accidentala sau intentionata de Cr<sup>6+<\/sup> determina intoxicatie acuta manifestata prin varsaturi si leziuni gastrointestinale generalizate, cu hemoragii digestive grave ce pot induce stare de soc hipovolemic. Pacientii care supravietuiesc efectelor toxice initiale dezvolta necroza tubulara acuta, necroza hepatocelulara si afectarea tesuturilor hematoformatoare<sup>2<\/sup>.<\/span><\/p>\n<p><span style=\"font-size: medium;\">Expunerea cronica prin inhalarea compusilor de crom insolubili poate produce pneumoconioza cu alterarea functiei pulmonare. Expunerea la saruri anorganice solubile poate precipita aparitia de ulceratii cutanate, dermatita, perforarea septului nazal si manifestari respiratorii de hipersensibilizare<sup>4<\/sup>.<\/span><\/p>\n<p><span style=\"font-size: medium;\">Efectul carcinogen al cromului asupra tractului respiratorii a fost documentat prin numeroase studii<sup>2<\/sup>.<\/span><\/p>\n<p><span style=\"font-size: medium;\"><strong><em>Recomandari pentru determinarea cromului<\/em><\/strong><\/span><\/p>\n<p style=\"padding-left: 30px;\"><span style=\"font-size: medium;\">-cromul in sange: evaluarea posibilului deficit indus de denutritie (in special la varstnici), traumatisme severe si stres<sup>1<\/sup>; monitorizarea pacientilor care primesc preparate de crom in cursul alimentatiei parenterale; evaluarea intolerantei dobandite la glucoza la pacientii cu alimentatie parenterala; evaluarea rezistentei la insulina la pacientii fara complicatii septice care sunt alimentati parenteral<sup>4<\/sup>;<\/span><\/p>\n<p style=\"padding-left: 30px;\"><span style=\"font-size: medium;\">-cromul in urina: monitorizarea expunerii profesionale la crom<sup>4<\/sup> .<\/span><\/p>\n<p><span style=\"font-size: medium;\"><strong><em>Pregatire pacient <\/em><\/strong>\u2013 pentru monitorizarea expunerii profesionale recoltarea probei de urina se va face fie in cursul lucrului, fie la sfarsitul schimbului la incheierea saptamanii de lucru<sup>4<\/sup>.<\/span><\/p>\n<p><span style=\"font-size: medium;\"><b><i>Specimen recoltat <\/i><\/b>-<b> <\/b>1) sange venos; 2) urina din 24 ore sau 3) o proba de urina spontana<sup>3;4<\/sup>.<\/span><\/p>\n<p><span style=\"font-size: medium;\"><b><i>Cauze de respingere a probei <\/i><\/b>\u2013 1) specimen coagulat; 2), 3) specimen ce contine conservanti<sup>3;4<\/sup>.<\/span><\/p>\n<p><span style=\"font-size: medium;\"><b><i>Recipient de recoltare <\/i><\/b>\u2013 1) vacutainer special pentru metale ce contine EDTA sau heparinat de litiu ca anticoagulant;
