Cromul este un metal tranzi\u0163ional, al c\u0103rui nume provine din cuv\u00e2ntul grecesc \u201cculoare\u201d, deoarece majoritatea compu\u015filor s\u0103i sunt intens colora\u0163i. Cel mai important minereu de crom este cromitul. Cromul \u015fi s\u0103rurile sale anorganice prezint\u0103 numeroase aplica\u0163ii industriale: placarea metalelor, producerea de aliaje diverse, materiale refractare, pigmen\u0163i \u015fi conservan\u0163i2<\/sup>.<\/span><\/p>\n

Cromul \u00een stare trivalent\u0103 este cea mai abundent\u0103 form\u0103 de crom din mediu. Fiziologic, cromul de\u0163ine un rol important\u00a0\u00een metabolismul glucozei2<\/sup> \u015fi, de asemenea, \u00een sinteza colesterolului1<\/sup>.\u00a0 Sc\u0103derea cromului \u00een organism este asociat\u0103 cu rezisten\u0163a la insulin\u0103 (hiperinsulinism), risc crescut de insuficien\u0163\u0103 cardiac\u0103 congestiv\u0103, hipercolesterolemie, reducerea fertilit\u0103\u0163ii1<\/sup>. De\u015fi cromul trivalent \u00een sine are o biodisponibilitate oral\u0103 redus\u0103, absorb\u0163ia sa intestinal\u0103 este crescut\u0103 \u00een prezen\u0163a \u201cfactorului de toleran\u0163a la glucoza\u201d, care formeaz\u0103 un complex cu Cr3+<\/sup>. Cromul trivalent este considerat a fi relativ netoxic in vivo<\/em>2<\/sup>.<\/span><\/p>\n

Fierul inhib\u0103 competitiv legarea cromului de transferin\u0103, astfel c\u0103 pacien\u0163ii cu hemocromatoz\u0103 prezint\u0103 o reten\u0163ie sc\u0103zut\u0103 de crom \u00een organism. S-a formulat ipoteza c\u0103 deficien\u0163a cromului la nivel celular ar avea un rol\u00a0\u00een dezvoltarea diabetului la pacien\u0163ii cu hemocromatoz\u01034<\/sup>.<\/span><\/p>\n

Pe de alt\u0103 parte, cromul hexavalent reprezint\u0103 forma cea mai important\u0103 de crom din punct de vedere toxicologic. Spre deosebire de Cr3+<\/sup>, Cr6+<\/sup> este absorbit cu u\u015furin\u0163\u0103 de majoritatea \u0163esuturilor, astfel c\u0103 ingestia accidental\u0103 sau inten\u0163ionat\u0103 de Cr6+<\/sup> determin\u0103 intoxica\u0163ie acut\u0103 manifestat\u0103 prin v\u0103rs\u0103turi \u015fi leziuni gastrointestinale generalizate, cu hemoragii digestive grave ce pot induce stare de \u015foc hipovolemic. Pacien\u0163ii care supravie\u0163uiesc efectelor toxice ini\u0163iale dezvolt\u0103 necroz\u0103 tubular\u0103 acut\u0103, necroz\u0103 hepatocelular\u0103 \u015fi afectarea \u0163esuturilor hematoformatoare2<\/sup>.<\/span><\/p>\n

Expunerea cronic\u0103 prin inhalarea compu\u015filor de crom insolubili poate produce pneumoconioz\u0103 cu alterarea func\u0163iei pulmonare. Expunerea la s\u0103ruri anorganice solubile poate precipita apari\u0163ia de ulcera\u0163ii cutanate, dermatit\u0103, perforarea septului nazal \u015fi manifest\u0103ri respiratorii de hipersensibilizare4<\/sup>.<\/span><\/p>\n

Efectul carcinogen al cromului asupra tractului respiratorii a fost documentat prin numeroase studii2<\/sup>.<\/span><\/p>\n

Recomand\u0103ri pentru determinarea cromului<\/em><\/strong><\/span><\/p>\n

\u2013 cromul \u00een s\u00e2nge: evaluarea posibilului deficit indus de denutri\u0163ie (\u00een special la v\u00e2rstnici), traumatisme severe \u015fi stres1<\/sup>; monitorizarea pacien\u0163ilor care primesc preparate de crom \u00een cursul alimenta\u0163iei parenterale; evaluarea intoleran\u0163ei dob\u00e2ndite la glucoz\u0103 la pacien\u0163ii cu alimenta\u0163ie parenteral\u0103; evaluarea rezisten\u0163ei la insulin\u0103 la pacien\u0163ii f\u0103r\u0103 complica\u0163ii septice care sunt alimenta\u0163i parenteral4<\/sup>;<\/span><\/p>\n

\u2013 cromul \u00een urin\u0103: monitorizarea expunerii profesionale la crom4<\/sup> .<\/span><\/p>\n

Preg\u0103tire pacient <\/em><\/strong>\u2013 pentru monitorizarea expunerii profesionale recoltarea probei de urin\u0103 se va face fie \u00een cursul lucrului, fie la sf\u00e2r\u015fitul schimbului la \u00eencheierea s\u0103pt\u0103m\u00e2nii de lucru4<\/sup>.<\/span><\/p>\n
