Aproximativ 80% din bilirubina circulanta deriva din eritrocitele imbatranite. Cand eritrocitele circulante ating sfarsitul vietii lor normale de aproximativ 120 zile, ele sunt distruse de celulele reticuloendoteliale. Oxidarea hemului genereaza biliverdina, care este metabolizata la randul sau in bilirubina. Restul de 15-20% din bilirubina circulanta provine din alte surse (distrugerea eritrocitelor mature din maduva osoasa hematogenasau metabolismul altor proteine ce\u00a0 contin hem \u2013 citocromii hepatici, mioglobina musculara, enzime)<sup>5;6<\/sup>.<\/p>\n<p>Bilirubina astfel formata circula in sange, fiind transportata la ficat sub forma unui complex solubil bilirubina- albumina. Desi bilirubina este legata destul de puternic de albumina, ea poate fi extrasa cu usurinta din sange de catre ficat. La nivel hepatic are loc conjugarea bilirubinei cu acidul glucuronic, sub actiunea UDP-glucuronil transferazelor. Bilirubina conjugata include bilirubin-monoglucuronid, care predomina in ficat, si bilirubin-diglucuronid, care predomina in lichidul biliar. Bilirubina conjugata este transportata in canaliculele biliare, de unde este deversata impreuna cu bila in caile biliare si apoi in intestin, unde sufera o serie de reduceri succesive cu formare de urobilinogen si stercobilinogen. Stercobilinogenul si o mica parte din urobilinogen sunt eliminate prin fecale; cea mai mare parte a urobilinogenului este reabsorbita intestinal, ajunge prin circulatia portala la ficat (circuitul entero-hepatic), fiind reexcretata prin bila<sup>10<\/sup>.<\/p>\n<p>Nivelurile bilirubinei serice cresc atunci cand productia depaseste metabolismul si excretia acesteia. Clinic hiperbilirubinemia apare ca icter \u2013 pigmentarea galbena a tegumentului si sclerelor, care se instaleaza la valori ale bilirubinei de 2\u20132.5 mg\/dL.<\/p>\n<p>Tulburarile metabolismului bilirubinei pot fi impartite in patru categorii majore:<\/p>\n<ul>\n<li>productie crescuta de pigment;<\/li>\n<li>preluare hepatica scazuta;<\/li>\n<li>conjugare hepatica inadecvata;<\/li>\n<li>excretie redusa de pigment conjugat din ficat si bila<sup>1;2<\/sup>.<\/li>\n<\/ul>\n<p>In laborator se masoara doua fractiuni ale pigmentului \u2013\u00a0 fractiunea hidrosolubila conjugata, care da reactia directa cu reactivul diazo si care reprezinta bilirubina conjugata (sub forma de mono- si diglucuronid) si fractiunea liposolubila care reprezinta bilirubina neconjugata<sup>9<\/sup>.<\/p>\n<p>Primele trei tulburari ale metabolismului bilirubinei sunt asociate cu bilirubinemie predominant neconjugata, iar a patra categorie este asociata cu bilirubinemie predominant conjugata si bilirubinurie.<\/p>\n<p>Bilirubina serica totala prezinta valori crescute in urmatoarele cazuri:<\/p>\n<p style=\"padding-left: 30px;\">\u2022 hemoliza necomplicata;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 icter hepatocelular;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 obstructie biliara extrahepatica;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 hepatita virala;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 hepatita alcoolica.<\/p>\n<p><strong>Bilirubina indirecta <\/strong>(BI)<\/p>\n<p>Bilirubina neconjugata sau bilirubina indirecta se obtine prin diferenta dintre valoarea bilirubinei totale si a celei directe. Concentratia plasmatica a bilirubinei neconjugate este determinata de:<\/p>\n<p style=\"padding-left: 30px;\">\u2022 rata cu care bilirubina nou sintetizata intra in plasma (turnover-ul bilirubinei);<\/p>\n<p style=\"padding-left: 30px;\">\u2022 rata eliminarii bilirubinei de catre ficat (clearance-ul hepatic al bilirubinei)<sup>8<\/sup>.<\/p>\n<p>Cresterile valorilor bilirubinei neconjugate in ser pot avea urmatoarele cauze:<\/p>\n<p>\u2022 <span style=\"text-decoration: underline;\">turnover-ul crescut<\/span> caracterizat prin distrugerea crescuta a eritrocitelor circulante \u2013 apare in afectiuni asociate cu hemoliza:<\/p>\n<p style=\"padding-left: 30px;\">*anemiile hemolitice \u2013 in majoritatea cazurilor de hemoliza necomplicata nivelul mediu al bilirubinei serice este de 3-4 mg\/dL;<\/p>\n<p style=\"padding-left: 30px;\">*icterul datorat productiei crescute de pigment ca urmare a infarctelor tisulare (infarcte pulmonare, colectii sanguine in tesuturi, dupa cateterisme, rupturi de anevrisme aortice);<\/p>\n<p>\u2022 <span style=\"text-decoration: underline;\">preluare hepatica alterata<\/span> produsa de unele medicamente (acid flavaspidic, novobiocina);<\/p>\n<p>\u2022 <span style=\"text-decoration: underline;\">conjugare alterata<\/span> a bilirubinei datorata activitatii scazute a bilirubin-glucuronil-transferazei;<\/p>\n<p style=\"padding-left: 30px;\">*icterul neonatal \u2013 aproape toti nou-nascutii manifesta un grad de hiperbilirubinemie neconjugata tranzitorie intre a doua si a cincea zi de viata ca efect al faptului ca in acest stadiu enzima hepatica glucuroniltransferaza este inca imatura. Valoarea bilirubinei indirecte nu depaseste 5 mg\/dL. La nou-nascutii cu un proces hemolitic supraadaugat, incarcatura de pigment duce la un icter mai pronuntat si nivelul bilirubinei poate depasi 20 mg\/dL.<\/p>\n<p style=\"padding-left: 30px;\">*deficitul ereditar de glucuroniltransferaza:<\/p>\n<p style=\"padding-left: 60px;\">-sindrom Gilbert (hiperbilirubinemie usoara si persistenta cu valori medii de 1.2\u20133 mg\/dL, rareori depasind 5 mg\/dL);<\/p>\n<p style=\"padding-left: 60px;\">-sindrom Crigler Najjar (tipul I cu absenta glucuroniltransferazei si valori ale BI de 20-45 mg\/dL si tipul II cu deficit partial de glucuroniltransferaza si valori ale BI cuprinse intre 6-20 mg\/dL);<\/p>\n<p style=\"padding-left: 30px;\">*deficit dobandit de glucuroniltransferaza;<\/p>\n<p style=\"padding-left: 30px;\">*inhibitie medicamentoasa;<\/p>\n<p style=\"padding-left: 30px;\">*icterul laptelui de san (inhibitie reversibila a glucuroniltransferazei)<sup>3;<\/sup><sup>5;6<\/sup>.<\/p>\n<p><strong>\u0411\u0438\u043b\u0438\u0440\u0443\u0431\u0438\u043d \u043f\u0440\u044f\u043c\u043e\u0439 <\/strong>(BD)<\/p>\n<p>Bilirubina conjugata, hidrosolubila si cu reactivitate crescuta, da o reactie de culoare cu reactivul diazo, fiind cunoscuta sub numele de bilirubina directa. Cresterea valorilor bilirubinei directe in ser este asociata cu excretia redusa de pigment conjugat din ficat si bila si apare in icterele colestatice sau in cele hepatocelulare. Cresterea patologica a bilirubinei directe duce la aparitia acestui pigment in urina. Intrucat bilirubina indirecta nu se elimina prin urina, prezenta bilirubinei in urina denota cresterea in ser a bilirubinei conjugate.<\/p>\n<p>Valori crescute ale bilirubinei directe apar in:<\/p>\n<p>\u2022 <span style=\"text-decoration: underline;\">obstructie intrahepatica<\/span>:<\/p>\n<p style=\"padding-left: 30px;\">*<em>datorata unor defecte familiale ale functiei excretorii hepatice<\/em>:<\/p>\n<p style=\"padding-left: 60px;\">-sindromul Dubin Johnson (icter cronic idiopatic cu valori ale bilirubinei totale intre 3-15 mg\/dL cu preponderenta bilirubinei conjugate;<\/p>\n<p style=\"padding-left: 60px;\">-sindromul Rotor;<\/p>\n<p style=\"padding-left: 30px;\">*<em>datorata unor defecte dobandite ale functiei excretorii hepatice<\/em>:<\/p>\n<p style=\"padding-left: 60px;\">-colestaza indusa de medicamente;<\/p>\n<p style=\"padding-left: 60px;\">-icter postoperator;<\/p>\n<p style=\"padding-left: 60px;\">-hepatita si ciroza \u2013 in bolile hepatocelulare exista de obicei o interferenta intre cele trei etape ale metabolismului bilirubinei: preluare, conjugare si excretie. Excretia este totusi etapa cea mai afectata si ca urmare in ser predomina bilirubina conjugata.<\/p>\n<p>\u2022 <span style=\"text-decoration: underline;\">obstructie extrahepatica<\/span> \u2013 obstructia mecanica a ductelor biliare se datoreaza cel mai adesea calculilor, tumorilor sau stricturilor. Nivelul bilirubinei serice depaseste rareori 35 mg\/dL<sup>3;<\/sup><sup>5;6<\/sup>.<\/p>\n<p><span style=\"text-decoration: underline;\">NOTA:<\/span><\/p>\n<p>Bilirubina totala = Bilirubina neconjugata (indirecta) + Bilirubina directa<\/p>\n<p>Bilirubina totala si directa sunt determinate de analizor; bilirubina indirecta rezulta prin calcul<sup>8<\/sup>.<\/p>\n<p><strong><em>Pregatire pacient <\/em><\/strong>-\u00e0 jeun (pe nemancate)<sup>7;8<\/sup>.<\/p>\n<p><strong><em>Specimen recoltat <\/em><\/strong>-sange venos<sup>7;8<\/sup>.<\/p>\n<p><strong><em>Recipient de recoltare <\/em><\/strong>-vacutainer fara anticoagulant cu\/fara gel separator<sup>7<\/sup>.<\/p>\n<p><strong><em>Prelucrare necesara dupa recoltare <\/em><\/strong>\u2013 se separa serul imediat si se lucreaza in max. 4 ore; in perioada de asteptare, serul este protejat de lumina (expunerea la lumina produce scaderi semnificative dupa 2-3 ore). Daca nu se poate lucra in 4 ore, serul separat se stocheaza la 4\u00b0C sau -20\u00b0C<sup>7<\/sup>.<\/p>\n<p><strong><em>Volum proba<\/em><\/strong> \u2013 minim 0.5 mL ser<sup>7<\/sup>.<strong><em>\u00a0<\/em><\/strong><\/p>\n<p><strong><em>Cauze de respingere a probei <\/em><\/strong>\u2013 specimen hemolizat, specimen neprotejat de lumina<sup>7;8<\/sup>.<\/p>\n<p><strong><em>Stabilitatea probei <\/em><\/strong>\u2013 serul separat este stabil <em>4 ore<\/em> la temperatura camerei;
