{"id":9174,"date":"2020-04-23T04:30:17","date_gmt":"2020-04-23T01:30:17","guid":{"rendered":"https:\/\/www.synevo.ro\/product\/bicarbonat-eco2\/"},"modified":"2026-02-18T23:37:49","modified_gmt":"2026-02-18T21:37:49","slug":"bicarbonat-eco2","status":"publish","type":"product","link":"https:\/\/old.synevo.md\/ru\/shop\/bicarbonat-eco2\/","title":{"rendered":"\u0411\u0438\u043a\u0430\u0440\u0431\u043e\u043d\u0430\u0442\u044b (ECO2)"},"content":{"rendered":"<p><strong><em>Sinonime<\/em><\/strong> \u2013 rezerva alcalina, CO<sub>2<\/sub>. <strong>\u00a0<\/strong><\/p>\n<p><strong><em>Informatii generale si recomandari pentru determinarea bicarbonatului<\/em><\/strong><\/p>\n<p>Dioxidul de carbon total este alcatuit din CO<sub>2 <\/sub>aflat in solutie sau legat de proteine, HCO<sub>3<\/sub><sup>-<\/sup>, CO<sub>3<\/sub><sup>2-<\/sup> si H<sub>2<\/sub>CO<sub>3<\/sub>. In practica, 80-90% din CO<sub>2<\/sub> total este reprezentat de ionul bicarbonat (HCO<sub>3<\/sub><sup>-<\/sup>)<sup>4<\/sup>.<\/p>\n<p>Bicarbonatul constituie cea de a doua fractiune anionica plasmatica, de aceea concentratia sa este un indicator important al distributiei electrolitilor si al deficitului de anioni. Impreuna cu determinarea pH-ului sanguin, dozarea bicarbonatului este utila in diagnosticul si tratamentul numeroaselor afectiuni insotite de tulburari ale echilibrului acido-bazic de cauza respiratorie sau metabolica<sup>3<\/sup>.<\/p>\n<p>Cel mai frecvent, o valoare anormala a HCO<sub>3<\/sub><sup>-<\/sup> semnifica o tulburare metabolica, mai degraba decat una respiratorie; astfel, scaderea HCO<sub>3<\/sub><sup>-<\/sup> indica acidoza metabolica (ex.: diabet cu cetoacidoza) si cresterea HCO<sub>3<\/sub><sup>-<\/sup> indica alcaloza metabolica (indusa, de exemplu, de varsaturi prelungite)<sup>2<\/sup>.<\/p>\n<p>In acidoza respiratorie se produce ca mecanism compensator reabsorbtia HCO<sub>3<\/sub><sup>- <\/sup>la nivelul tubilor renali, iar concentratia serica a HCO<sub>3<\/sub><sup>- <\/sup>se modifica in functie de presiunea partiala a CO<sub>2<\/sub> (PaCO<sub>2<\/sub>):<\/p>\n<p style=\"padding-left: 30px;\">-in faza acuta HCO<sub>3<\/sub><sup>- <\/sup>creste cu 1 mmol\/L pentru fiecare crestere cu 10mmHg a PaCO<sub>2<\/sub>;<\/p>\n<p style=\"padding-left: 30px;\">-in faza cronica HCO<sub>3<\/sub><sup>-<\/sup> creste cu 3.5 mmol\/L pentru fiecare crestere cu 10mmHg a PaCO<sub>2.<\/sub><\/p>\n<p style=\"padding-left: 30px;\">Pe de alta parte, in alcaloza respiratorie mecanismul compensator este reprezentat de cresterea excretiei renale a HCO<sub>3<\/sub><sup>-<\/sup>, iar concentratia serica a HCO<sub>3<\/sub><sup>-<\/sup> se modifica astfel:<\/p>\n<p style=\"padding-left: 30px;\">-in faza acuta HCO<sub>3<\/sub><sup>- <\/sup>scade cu 2 mmol\/L pentru fiecare scadere cu 10mmHg a PaCO<sub>2<\/sub>;<\/p>\n<p style=\"padding-left: 30px;\">\u2013 in faza cronica HCO<sub>3<\/sub><sup>-<\/sup> scade cu 5 mmol\/L pentru fiecare scadere cu 10mmHg a PaCO<sub>2<\/sub><sup>1;2;3<\/sup><sub>.<\/sub><\/p>\n<p><strong><em>Pregatire pacient <\/em><\/strong>\u2013 \u00e0 jeun (pe nemancate) sau postprandial<sup>3<\/sup>.<\/p>\n<p><strong><em>Specimen recoltat <\/em><\/strong>\u2013 sange venos<sup>3<\/sup>.<\/p>\n<p><strong><em>Cantitate recoltata<\/em><\/strong> \u2013 cat permite vacuumul<sup>3<\/sup>.<\/p>\n<p><strong><em>Recipient de recoltare <\/em><\/strong>\u2013 vacutainer fara anticoagulant, cu\/fara gel separator<sup>3<\/sup>.<\/p>\n<p><strong><em>Prelucrare necesara dupa recoltare<\/em><\/strong> \u2013 se separa serul prin centrifugare;
