Spre deosebire de ALT care se gaseste in principal la nivelul ficatului, AST este intalnita in mai multe tesuturi: miocard, ficat, muschi scheletici, rinichi, pancreas, tesut cerebral, splina, fiind astfel un indicator mai putin specific al functiei hepatice. La nivelul celulei hepatice, izoenzimele AST se gasesc atat in citosol, cat si in mitocondrii<sup>5;6<\/sup>.<\/p>\n<h2><strong>Recomandari pentru determinarea AST\/ ASAT\/ GOT<\/strong><\/h2>\n<p>Valorile AST si ALT cresc in aproape toate hepatopatiile, dar cresterea nivelului seric al AST poate fi intalnita si in numeroase afectiuni extrahepatice, cu precadere in infarctul miocardic si in maladii ale muschiului striat<sup>5<\/sup>.<\/p>\n<h2><strong>Pregatire pacient<\/strong><\/h2>\n<p>\u00e0 jeun (pe nemancate)<sup>4<\/sup>.<\/p>\n<p><strong>Specimen recoltat<\/strong> \u2013 sange venos<sup>4;5<\/sup>.<\/p>\n<p><strong>Recipient de recoltare <\/strong>\u2013 vacutainer fara anticoagulant cu\/fara gel separator<sup>4<\/sup>.<\/p>\n<p><strong>Prelucrare necesara dupa recoltare<\/strong> \u2013 se separa serul prin centrifugare<sup>4<\/sup>.<\/p>\n<p><strong>Volum proba <\/strong>\u2013 minim 0.5 mL ser<sup>4<\/sup>.<\/p>\n<p><strong>Cauze\u00a0 de respingere a probei<\/strong> \u2013 specimen intens hemolizat, lipemic sau contaminat bacterian<sup>4;5<\/sup>.<\/p>\n<p><strong>Stabilitate proba<\/strong> \u2013 serul separat este stabil 24 ore la temperatura camerei; valori ale AST de 1000-9000 U\/l, care scad la 50% in urmatoarele 3 zile sugereaza ficat de soc cu necroza centrolobulara (insuficienta cardiaca congestiva, aritmii, sepsis, hemoragii); o crestere brusca poate aparea si in hepatita virala acuta fulminanta (rar &gt;4000 U\/l);<\/p>\n<p style=\"text-align: center;\">\u2022 o crestere si o scadere rapida sugereaza obstructia biliara extrahepatica<sup>3<\/sup>;<\/p>\n<p style=\"text-align: center;\">\u2022 cresteri marcate (peste 3000 U\/l) pot aparea\u00a0 in infarctul miocardic acut, stari septice, interventii pe cord;<\/p>\n<p style=\"text-align: center;\">\u2022 in cazul infarctului miocardic eliberarea enzimei din celule are loc la 6-12 ore de la declansare, gradul de crestere fiind rareori proportional cu extinderea leziunii; valorile maxime se obtin la 48 de ore, dupa care se revine la normal in 3-5 zile;<\/p>\n<p style=\"text-align: center;\">\u2022 valori marcat crescute mai pot aparea in cazul traumatismelor hepatice, metastazelor hepatice si rabdomiolizei;<\/p>\n<p style=\"text-align: center;\">\u2022 valori mai putin crescute apar in formele usoare de hepatita acuta virala, hepatopatii cronice (hepatita cronica activa, ciroza), hepatita alcoolica, dar rareori depasesc 300U\/l (in hepatopatii alcoolice, ciroza: AST&gt;ALT);<\/p>\n<p style=\"text-align: center;\">\u2022 in mononucleoza infectioasa cresterea nivelului AST este proportionala cu afectarea hepatica<sup>2;3<\/sup>;<\/p>\n<p style=\"text-align: center;\">\u2022 administrarea de opiacee la pacienti cu afectiuni ale tractului biliar (de 2.5-65 de ori)<sup>3<\/sup>.<\/p>\n<\/td>\n<\/tr>\n<tr>\n<td valign=\"top\" width=\"624\">\n<p style=\"text-align: center;\"><strong>Scaderi<\/strong><\/p>\n<\/td>\n<\/tr>\n<tr>\n<td valign=\"top\" width=\"624\">\n<p style=\"text-align: center;\">\u2022azotemie; \u2022dializa renala cronica; \u2022deficit cronic de piridoxal fosfat (malnutritie, consum de alcool)<sup> 2;3<\/sup>.<\/p>\n<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p><span style=\"color: #ff0000;\"><strong>Valori critice \u2013 <\/strong>sindromul alcool-acetaminofen: &gt;20000 U\/l.<\/span><\/p>\n<p><strong>Limite si interferente<\/strong><\/p>\n<p><strong>\u2022\u00a0 <\/strong>Conditii fiziologice \u2013 in sarcina pot sa apara valori scazute &lt;10 U\/L<sup>2<\/sup>.<strong>\u00a0<\/strong><\/p>\n<p><strong>\u2022 <\/strong>Conditii patologice \u2013 cresteri moderate apar in unele forme de distrofie musculara, in dermatomiozita, traumatisme, interventii chirurgicale, injectii intramusculare<sup>3<\/sup>, eclampsie, pancreatita acuta, leziuni intestinale, post iradiere locala, infarctul pulmonar, infarctul cerebral, infarctul renal, arsuri, hipotermie, hipertermie, intoxicatii (ciuperci), anemii hemolitice<sup>2;5<\/sup>, hipotiroidism.<\/p>\n<p><strong>\u2022\u00a0 <\/strong>Medicamente<\/p>\n<p><strong><span style=\"text-decoration: underline;\">Cresteri<\/span>\u00a0<\/strong><\/p>\n<p>Medicamente care produc colestaza: acid aminosalicilic, amitriptilina, steroizi anabolizanti, androgeni, azatioprina, benzodiazepine, carbamazepin, carbazona, clorotiazida, clorpropamida, acid clavulanic, dapsone.<\/p>\n<p>Medicamente care produc o afectare hepatocelulara: acetaminofen, alopurinol, acid aminosalicilic, amiodarona, amitriptilina, steroizi anabolizanti, androgeni, asparaginaza, aspirina, azatioprina, carbamazepin, chenodiol, clorambucil, cloramfenicol, clorpropamida, cimetidina, ciclosporina, danazol, dantrolen, dapsona, diclofenac, dicumarol (rar), disulfiram, eritromicina, estrogeni, etionamida, gliburid (glibenclamid), saruri de aur, imipramina, mercaptopurina, metimazol, acid nicotinic, nitrofurantoin, contraceptive orale, etanol (exces), etionamida, fluconazol, halotan, ibuprofen, imipramina, indometacin, saruri de fier (supradozare), izoniazida, ketoconazol, inhibitori MAO, mercaptopurina, metotrexat, metoxifluran, metildopa, naproxen, papaverina, parametadion, penicilamina, peniciline, fenotiazine, fenilbutazona, progestative, propoxifen, sulfonamide, sulfone, sulindac, tamoxifen, tolbutamida, perhexilin, fenazopiridina, fenobarbital, fenilbutazona, fenitoin, plicamicin (mitramicin), probenecid, procainamida, propiltiouracil, pirazinamida, chinidina, rifampin, salicilati, sulfasalazina, sulfonamide, tetracicline, trimetadiona, acid valproic, vitamina A, warfarina (rar).<\/p>\n<p>Multe alte medicamente pot determina cresteri care in general sunt tranzitorii dar in anumite cazuri indica o hepatotoxicitate. Intre acestea sunt incluse : acebutolol, aminoglutetimid, aminoglicozide, azitromicin, bromocriptin, captopril, carboplatin, carmustin, cefalosporine, ciclosporine, clindamicin, clofibrat, clotrimazol, citarabina, dacarbazin, dapsona, didanosin, disopiramid, enfluran, acid etacrinic, etambutol, etopozid, fenofibrat, fluorochinolone (ex: ciprofloxacin), ganciclovir, heparina, inhibitori ai 3 hidroxi-3 metilglutaril CoA reductazei (ex: lovastatin, simvastatin), idarubicin, interferon, isotretinoin, labetalol, levamisol, levodopa, lincomicin, mebendazol, mefloquin, metoprolol, mexiletin, nifedipin, omeprazol, peniciline, pentamidina, piroxicam, propoxifen, protriptilina, piridoxina, ranitidina, ritodrina, sargramostim, streptozocin, sulfoniluree, tiotixen, tiabendazol, tioguanina, ticlopidina, tobramicina, tretinoin, verapamil, zalcitabin<sup>1<\/sup>.<\/p>\n<p><span style=\"text-decoration: underline;\"><strong>Scaderi<\/strong>:<\/span> acid ascorbic, alopurinol, ciclosporina, clomipramina, ibuprofen, metronidazol, penicilamina, pindolol, prednison, progesteron, rifampicina, simvastatin, ursodiol<sup>1<\/sup>.<\/p>\n<p>\u2022 Interferente analitice<\/p>\n<p>Prezenta macroenzimei AST (formarea de complexe intre AST si imunoglobuline) poate determina cresteri neexplicate de AST (de pana la 30 ori fata de limita superioara a normalului), cu niveluri normale de ALT si creatinkinaza<sup>5<\/sup>.<\/p>\n<p>In cazul unui ser hemolizat, pot aparea valori fals crescute datorita activitatii AST din eritrocite.<\/p>\n<p>Lipemia poate creste absorbanta probei, valoarea AST neputand fi calculata.<\/p>\n<p>Foarte rar, la pacientii cu gamopatii monoclonale, in special de tip IgM (boala Waldenstr\u00f6m) rezultatele obtinute pot fi neconcludente<sup>4<\/sup>.<\/p>\n<p><span style=\"font-size: small;\">\u00a0<\/span><\/p>\n<h2><span style=\"font-size: small;\">\u0411\u0438\u0431\u043b\u0438\u043e\u0433\u0440\u0430\u0444\u0438\u044f<\/span><\/h2>\n<p><span style=\"font-size: small;\">1.
