Streptolizina este o hemolizina produsa de streptococii de grup A. La pacientii infectati, streptolizina O actioneaza ca un antigen fata de care organismul dezvolta un raspuns umoral specific (antistreptolizina O=ASLO). Nivelurile anticorpilor devin detectabile in 1-2 saptamani de la infectia streptococica si ating un maximum dupa 3-6 saptamani; daca nivelurile ridicate persista dupa 6 saptamani sau chiar se inregistreaza o crestere a acestora se va suspecta o reinfectie3;4<\/sup>.<\/p>\n

Recomandari pentru determinarea ASLO<\/em><\/strong><\/p>\n

\u2022 confirmarea expunerii la streptolizina streptococica;3<\/sup><\/p>\n

\u2022 test util in diagnosticul conditiilor asociate cu infectii streptococice: reumatism articular acut, glomerulonefrita, endocardita, scarlatina1<\/sup>;<\/p>\n

\u2022 monitorizarea regulata a pacientilor cu reumatism articular acut (la interval de 2-4 saptamani)4<\/sup>.<\/p>\n

Interpretarea rezultatelor<\/em><\/strong><\/p>\n

Titrul ASLO este crescut in 80-85% din cazurile de infectie streptococica A. Nivelul anticorpilor ASLO atinge valori foarte mari in special in reumatismul articular acut, de aceea titrurile persistent scazute pledeaza impotriva acestei afectiuni1<\/sup>.<\/p>\n

La un pacient cu reumatism articular acut tratat obtinerea unor valori constant scazute ale nivelului ASLO indica o evolutie favorabila4<\/sup>.<\/p>\n

Limite si interferente<\/em><\/strong><\/p>\n

\u2022 Titrul ASLO poate fi crescut si la unii purtatori sanatosi de streptococ betahemolitic grup A1<\/sup>.<\/p>\n

\u2022 Numai 25% dintre pacientii cu streptococie cutanata au titrul ASLO crescut. La acesti pacienti se recomanda determinarea anticorpilor anti-DNAaza B3<\/sup>.<\/p>\n

\u2022 Tratamentul antimicrobian suprima raspunsul umoral fata de streptolizina O, de aceea testul nu este indicat la pacientii care au primit recent antibiotice1<\/sup>.<\/p>\n

\u2022 b-lipoproteinele din serul pacientilor cu boli hepatice neutralizeaza proprietatile hemolitice ale streptolizinei O, determinand obtinerea de rezultate fals crescute1;3<\/sup>.<\/p>\n

\u2022 In cazul specimenelor de ser contaminate cu Bacillus cereus sau Pseudomonas, b-lipoproteinele existente in produsii de crestere bacterieni neutralizeaza proprietatile hemolitice ale streptolizinei O, determinand obtinerea de rezultate fals crescute3<\/sup>.<\/p>\n

\u2022 In cazuri foarte rare, prezenta gamapatiei monoclonale, in special IgM (macroglobulinemia Waldenstr\u00f6m) poate determina rezultate eronate2<\/sup>.<\/p>\n
