Informa<\/em><\/strong>\u0163<\/em><\/strong>ii generale \u0219i recomand\u0103ri pentru determinarea anticorpilor anti-SLA\/LP<\/em><\/strong><\/p>\n

Anticorpii anti-SLA<\/strong> (engl. soluble liver antigen) au fost descri\u015fi \u00een 1987 ca fiind \u00eendrepta\u0163i \u00eempotriva\u00a0 unei proteine din citosol care se g\u0103se\u015fte \u00een concentra\u0163ii crescute \u00een \u0163esutul hepatic \u015fi renal. Anti-LP<\/strong> (engl. liver-pancreas) au fost descri\u015fi \u00eenaintea anti-SLA (1981), de c\u0103tre un grup diferit de cercet\u0103tori, ca av\u00e2nd reactivitate fa\u0163\u0103 de o protein\u0103 solubil\u0103 dintr-un lizat omogen de ficat \u015fi pancreas. Ulterior s-a demonstrat\u00a0 c\u0103 anti-SLA si anti-LP sunt\u00a0 identici (anti SLA\/LP<\/strong>) \u015fi c\u0103 antigenul \u0163int\u0103 este reprezentat de o protein\u0103 citosolic\u0103 de 50 kDa ce este implicat\u0103 \u00een biosinteza seleno-proteinelor. Este vorba de O-fosfoseril-ARNt: selecisteinil-ARNt sintaza, denumit\u0103 interna\u021bional SepSecS, care catalizez\u0103 ultima etap\u0103 \u00een sinteza seleno-cisteinei.<\/p>\n

Anti-SLA\/LP sunt detecta\u021bi izolat sau \u00eempreun\u0103 cu anticorpii antinucleari (ANA) \u015fi\/sau anticorpii anti-mu;chi neted (ASMA) la pacien\u0163ii cu hepatit\u0103 autoimun\u0103 tip 1, av\u00e2nd o prevalen\u0163\u0103 de 15-30% \u0219i fiind asocia\u021bi cu o evolu\u021bie mai sever\u0103 a bolii, cu necesitatea unui perioade mai \u00eendelungate de tratament pentru suprimarea activit\u0103\u021bii inflamatorii, cu o rat\u0103 de rec\u0103deri mai mare la \u00eentreruperea corticoterapiei, cu o frecven\u021b\u0103 mai mare a transplantului hepatic precum \u0219i cu o mortalitate mai ridicat\u0103 prin insuficien\u021b\u0103 hepatic\u0103. Din perspectiva clinic\u0103, determinarea acestor anticorpi este util\u0103 \u00een principal la pacien\u021bii care nu prezint\u0103 markerii clasici de boal\u0103, contribuind semnificativ la reducerea num\u0103rului de cazuri de hepatit\u0103 criptogenic\u0103 sau de hepatit\u0103 autoimun\u0103 cu autoanticorpi negativi.<\/p>\n

A fost \u00eenregistrat\u0103 o varia\u021bie semnificativ\u0103 a prevalen\u021bei anti-SLA\/LP \u00een diverse grupuri etnice, suger\u00e2nd existen\u021ba unor factori genetici predispozan\u021bi. Astfel, ace\u0219ti autoanticorpi au fost asocia\u021bi cu HLA A1-B8 \u00een popula\u021bia german\u0103 \u0219i cu HLA DRB*0301 \u00een Marea Britanie \u0219i USA.<\/p>\n

Depistarea \u021bintei antigenice a dus la dezvoltarea unor tehnici ELISA \u0219i imunoblot ce sunt capabile sa detecteze cu sensibilitate \u0219i specificitate ridicat\u0103 anti-SLA\/LP.<\/p>\n

\u00cen plus, a fost identificat\u0103 o corela\u0163ie semnificativ\u0103 \u00eentre serurile ce prezint\u0103 anti-SLA\/LP \u015fi anti-Ro-52, \u00een sensul c\u0103 pacien\u0163ii anti-SLA\/LP \u015fi anti-Ro-52 pozitivi au o activitate crescut\u0103 a bolii hepatice \u015fi o evolu\u0163ie mai sever\u0103. Se presupune c\u0103 prognosticul nefavorabil asociat cu anti-SLA\/LP s-ar datora prezen\u021bei concomitente a antiRo-521;2;3;4;5;6<\/sup>.<\/p>\n

Preg\u0103tire pacient <\/em><\/strong>- <\/em>\u00e0 jeun (pe nem\u00e2ncate) sau postprandial (dup\u0103 mese)7<\/sup>.<\/p>\n
