Anticorpii anti-Ro sunt \u00eendrepta\u0163i \u00eempotriva a 2 proteine (Ro60 si Ro52) legate intracelular, sub forma unui complex, de acizi ribonucleici de dimensiuni mici.<\/p>\n

S-a constatat c\u0103 exist\u0103 o baz\u0103 genetic\u0103 \u00een apari\u0163ia anticorpilor anti-Ro, prin asocierea acestora cu antigenul HLA DR3.<\/p>\n

Anticorpii anti-Ro au semnifica\u0163ie clinic\u0103 \u00een diagnosticul sindromului Sj\u00f6gren (prevalen\u0163a 70-100% \u00een sindromul Sj\u00f6gren primar) \u015fi LES (prevalen\u0163a 25-60%). Ace\u015fti anticorpi se asociaz\u0103 de obicei cu urm\u0103toarele forme clinice de boal\u0103: lupus eritematos cutanat subacut, lupus eritematos neonatal, vasculita din sindromul Sj\u00f6gren.<\/p>\n

Pacien\u0163ii cu boal\u0103 lupic\u0103 care asociaz\u0103 anticorpi anti-Ro prezint\u0103 mai frecvente simptome sicca sau exanteme fotosensibile, \u00een special atunci c\u00e2nd anticorpii se g\u0103sesc \u00een titru crescut.<\/p>\n

Anticorpii anti-Ro \u00eempreun\u0103 cu anticorpii anti-La au un rol diagnostic important \u00een monitorizarea prenatal\u0103 a gravidelor. Anti-Ro \u015fi adesea anti-La sunt prezen\u0163i \u00eentr-un procent de 100% la mamele fe\u0163ilor sau nou-n\u0103scu\u0163ilor cu bloc atrio-ventricular congenital sau lupus neonatal (exanteme fotosensibile, citopenii). Ace\u015fti anticorpi sunt considera\u0163i a fi un model de fenomene autoimune dob\u00e2ndite transplacentar. Riscul unei mame cu anti-Ro de a da na\u015ftere unui copil cu aceste manifest\u0103ri de boal\u0103 este de 5-10%. \u00cen timpul sarcinii \u015fi la na\u015ftere, marea majoritate a mamelor nu \u00eentrunesc criteriile de diagnostic pentru boala lupic\u0103 sau sindrom Sj\u00f6gren primar, unele dintre acestea fiind chiar asimptomatice. \u00cen decursul timpului, multe din paciente dezvolt\u0103 simptome u\u015foare de boal\u0103 lupic\u0103, sindrom Sj\u00f6gren primar sau o boal\u0103 de \u0163esut conjunctiv nediferen\u0163iat\u01033<\/sup>.<\/p>\n

Recomand\u0103ri pentru determinarea anticorpilor anti-Ro <\/em><\/strong>\u2013 suspiciune sindrom Sj\u00f6gren primar sau secundar; suspiciune LES; suspiciune lupus eritematos cutanat subacut; bloc atrioventricular congenital sau lupus neonatal \u00een asociere cu LES, sindrom Sj\u00f6gren sau colagenoz\u0103 nediferen\u0163iat\u01032<\/sup>;3<\/sup><\/p>\n

Limite \u015fi interferen\u0163e<\/em><\/strong><\/p>\n

Sindromul Sj\u00f6gren primar nu poate fi diferen\u0163iat de forma secundar\u0103 de boal\u0103 pe baza determin\u0103rii anticorpilor anti-Ro. Acest fapt este valabil \u015fi pentru sindromul Sj\u00f6gren secundar asociat cu poliartrita reumatoid\u0103.<\/p>\n

Deoarece reactivul folosit con\u0163ine ambele proteine Ro, nu se poate face o distinc\u0163ie \u00eentre anticorpii anti-Ro52 (asocia\u0163i mai frecvent cu sindromul Sj\u00f6gren) \u015fi anticorpii anti-Ro60 (asocia\u0163i mai frecvent cu LES); pentru aceasta trebuie s\u0103 se recurg\u0103 la tehnica imunoblot3<\/sup>.<\/p>\n
