Anti-HBe apar dup\u0103 dispari\u0163ia AgHBe \u015fi r\u0103m\u00e2n detectabili timp \u00eendelungat. Indic\u0103 sc\u0103derea gradului de contagiozitate \u015fi sugereaz\u0103 un prognostic bun \u00een ceea ce prive\u015fte rezolu\u0163ia infec\u0163iei acute. Asocierea cu anti-HBc, \u00een absen\u0163a AgHBs \u015fi a anti-HBs, confirm\u0103 diagnosticul de infec\u0163ie acut\u0103 recent\u0103 (2-16 s\u0103pt\u0103m\u00e2ni).<\/span><\/p>\n

Investigarea anti-HBe nu este recomandat\u0103 pentru determinarea prevalen\u0163ei infec\u0163iei cu VHB, av\u00e2nd \u00een vedere c\u0103 anti-HBe se \u00eent\u00e2lnesc mult mai rar dec\u00e2t anticorpii anti-HBc sau anti-HBs. Practic toate serurile anti-HBe pozitive, sunt \u015fi anti-HBc pozitive.<\/span><\/p>\n

At\u00e2t testarea AgHBe c\u00e2t \u015fi a anti-HBe se recomand\u0103 doar la serurile AgHBs pozitive3<\/sup>. \u00ce<\/span>n cursul terapiei cu interferon, dispari\u0163ia antigenului HBe \u015fi apari\u0163ia anticorpilor anti-HBe constituie un element de prognostic favorabil2<\/sup>.<\/span><\/p>\n

Recomand\u0103ri pentru determinarea anti-HBe<\/em><\/strong><\/span><\/p>\n

\u2022\u00a0 investigarea gradului de infec\u0163iozitate a serului;<\/span><\/div>\n
\u2022\u00a0 monitorizarea infec\u0163iilor cu hepatit\u0103 B acut\u0103 sau cronic\u01032<\/sup>.<\/span><\/div>\n

Preg\u0103tire pacient <\/em><\/strong>\u2013 \u00e0 jeun (pe nem\u00e2ncate)1<\/sup>.<\/span><\/p>\n

Anticorpii anti-HBe nu sunt de obicei detectabili \u00een infec\u0163ii cu un VHB mutant, care nu produce antigen HBe.<\/span><\/p>\n

\u00cen unele cazuri serul prezint\u0103 infec\u0163iozitate mare, de\u015fi sunt prezen\u0163i anti-HBe. Singura modalitate de a testa infec\u0163iozitatea serurilor AgHBe negative r\u0103m\u00e2ne determinarea ADN-VHB2<\/sup>.<\/span><\/p>\n

\u2022\u00a0 Interferen\u0163e analitice<\/span><\/p>\n

Pot produce interferen\u0163e cu unele componente ale kit-ului \u015fi conduce la rezultate neconcludente urm\u0103toarele:<\/span><\/p>\n

\u2013 tratamentul cu biotin\u0103 \u00een doze mari (>5 mg\/zi); de aceea se recomand\u0103 ca recoltarea de s\u00e2nge s\u0103 se fac\u0103 dup\u0103 minimum 8 ore de la ultima administrare;<\/span><\/div>\n
\u2013 titrurile foarte crescute de anticorpi anti-streptavidina \u015fi anti-ruteniu;<\/span><\/div>\n
\u2013 anticorpii monoclonali proveni\u0163i de la \u015foarece administra\u0163i la unii pacien\u0163i \u00een scop diagnostic sau terapeutic1<\/sup>.<\/span><\/div>\n
