Exist\u0103 un antigen central al VHB, Ag HBc, care nu se g\u0103se\u015fte \u00een s\u00e2nge ci numai\u00a0\u00een celulele hepatice infectate. Anticorpii fa\u0163\u0103 de acest antigen, anti-HBc, apar\u00a0\u00een s\u00e2nge destul de timpuriu, \u00eenaintea anti-HBs, av\u00e2nd valoare diagnostic\u01033<\/sup>.<\/span><\/p>\n

Anticorpii anti-HBc devin aproape \u00eentotdeauna detectabili \u00een perioada de incuba\u0163ie a bolii. Dup\u0103 dispari\u0163ia antigenului HBs \u015fi \u00eenainte de apari\u0163ia anticorpilor anti-HBs (\u201cfereastra imunologic\u0103\u201d), anticorpii anti-HBc \u015fi anticorpii anti-HBc-IgM pot fi singurii indicatori ai prezen\u0163ei hepatitei B acute.<\/span><\/p>\n

Datorit\u0103 persisten\u0163ei anticorpilor anti-HBc (de obicei o perioad\u0103 lung\u0103 dup\u0103 infectare), detectarea acestor anticorpi este util\u0103 \u00een scop epidemiologic, pentru a constata prevalen\u0163a infec\u0163iei cu VHB \u00een anumite grupe de popula\u0163ie.<\/span><\/p>\n

Titrul anticorpilor anti-HBc determinat la purt\u0103torii s\u0103n\u0103to\u015fi de antigen HBs este \u00een medie de 10 ori mai mare fa\u0163\u0103 de cel depistat la\u00a0pacien\u0163ii cu hepatit\u0103 acut\u0103 B,\u00a0\u00een timp ce la pacien\u0163ii cu hepatit\u0103 cronic\u0103 B este\u00a0\u00een medie de 100 de ori mai mare. Un titru foarte ridicat de anticorpi anti-HBc chiar \u015fi la persoanele cu Ag HBs negativ, sugereaz\u0103 prezen\u0163a hepatitei cronice B (infec\u0163ie cu VHB criptic\u0103)2<\/sup>.<\/span><\/p>\n

Recomand\u0103ri pentru determinarea anti-HBc <\/em><\/strong><\/span><\/p>\n

\u2022 diagnosticul hepatitei B acute \u015fi cronice;<\/span><\/div>\n
\u2022 verificarea rezultatelor pozitive ob\u0163inute pentru AgHBs \u015fi anti-HBs3<\/sup>.<\/span><\/div>\n

Testele noi utilizate \u00een determin\u0103rile de anti-HBc con\u0163in un agent reduc\u0103tor care distruge anticorpii IgM \u015fi astfel se m\u0103re\u015fte semnificativ specificitatea pentru determinarea anti-HBc-IgG1<\/sup>.<\/span><\/p>\n

\u2022\u00a0 Interferen\u0163e analitice<\/span><\/p>\n

Pot produce interferen\u0163e cu unele componente ale kit-ului \u015fi conduce la rezultate neconcludente urm\u0103toarele:<\/span><\/p>\n

\u2013 tratamentul cu biotin\u0103 \u00een doze mari (>5 mg\/zi); de aceea se recomand\u0103 ca recoltarea de s\u00e2nge s\u0103 se fac\u0103 dup\u0103 minimum 8 ore de la ultima administrare;<\/span><\/div>\n
\u2013 titrurile foarte crescute de anticorpi anti-streptavidin\u0103 \u015fi anti-ruteniu;<\/span><\/div>\n
\u2013 anticorpii monoclonali proveni\u0163i de la \u015foarece administra\u0163i la unii pacien\u0163i \u00een scop diagnostic sau terapeutic1<\/sup>.<\/span><\/div>\n
