\u041e\u0431\u0449\u0430\u044f \u0438\u043d\u0444\u043e\u0440\u043c\u0430\u0446\u0438\u044f<\/em><\/strong><\/span><\/p>\n

Virusul hepatitei A este un virus mic, lipsit de \u00eenveli\u015f, ce face parte din familia Picornaviridae<\/em>, genul Hepatovirus<\/em>; este alc\u0103tuit dintr-un genom ARN monocatenar de polaritate pozitiv\u0103 \u015fi o capsida cu simetrie icosaedric\u0103; \u00een func\u0163ie de proteinele capsidale (VP1 \u2013 VP3) a fost descris\u0103 o singur\u0103 variant\u0103 antigenic\u0103 major\u0103. Anticorpii care apar fa\u0163\u0103 de aceste antigene sunt anticorpi neutralizan\u0163i, protectori3;4;6<\/sup>.<\/span><\/p>\n

Virusul se transmite predominat fecal-oral, prin ap\u0103 \u015fi alimente contaminate cu fecale, dar \u00een condi\u0163ii de mare promiscuitate este posibil\u0103 transmiterea \u015fi prin contact interuman str\u00e2ns4;6<\/sup>; este raportat\u0103 \u015fi transmiterea pe cale sexual\u0103 la homosexuali6<\/sup> \u015fi rar parenteral3;7<\/sup>.<\/span><\/p>\n

Perioada de incuba\u0163ie a bolii este \u00een medie 28 zile (15-60 zile). Formele inaparente reprezint\u0103 peste 80% din cazurile \u00eenregistrate la copii; cu v\u00e2rsta num\u0103rul acestora scade. Manifest\u0103rile hepatitei sunt cel mai frecvent de gravitate medie, dar pot evolua \u015fi sever, \u00eens\u0103 foarte rar (1%) ca hepatite fulminante (insuficien\u0163a hepato-renal\u0103, cu mortalitate 80-90% ). Hepatita viral\u0103 A nu cronicizeaz\u0103 \u015fi virusul nu persist\u0103\u00a0\u00een organism3;4;6;7<\/sup>.<\/span><\/p>\n

Virusul este prezent \u00een s\u00e2nge \u015fi fecale cu 2 s\u0103pt\u0103m\u00e2ni \u00eenaintea debutului bolii \u015fi 1-2 s\u0103pt\u0103m\u00e2ni dup\u0103 dispari\u0163ia icterului4<\/sup>.<\/span><\/p>\n

Diagnosticul de rutin\u0103 al infec\u0163iei este serologic; testele comerciale pun \u00een eviden\u0163\u0103 anticorpii de tip IgM \u015fi totali.<\/span><\/p>\n

Primii anticorpi care apar sunt de tip IgM, urma\u0163i ulterior de apari\u0163ia IgG. Anti-HAV-IgM apar concomitent cu debutul simptomelor, ating un nivel maxim \u00een cursul primei luni \u015fi devin nedetectabili dup\u0103 6 luni2;4<\/sup>. Anticorpii anti- HAV totali sunt predominant de tip IgG, cu excep\u0163ia perioadei infec\u0163iei acute cu VHA, c\u00e2nd predomin\u0103 anticorpii de tip IgM. Ei sunt aproape \u00eentotdeauna prezen\u0163i la debutul hepatitei acute \u015fi sunt de regul\u0103 detectabili tot restul vie\u0163ii (45% dintre adul\u0163i au ace\u015fti anticorpi prezen\u0163i in ser). Prezen\u0163a lor indic\u0103 o expunere anterioar\u0103 la VHA, convalescen\u0163\u0103, precum \u015fi imunitate fa\u0163\u0103 de hepatita de tip A7<\/sup>; anti-HAV-IgG sunt pu\u015fi \u00een eviden\u0163\u0103 \u00een ser la 2 s\u0103pt\u0103m\u00e2ni de la vaccinare4<\/sup> \u015fi dup\u0103 administrarea de imunoglobuline3;7<\/sup>; titrul anticorpilor postinfec\u0163ie este mai mare dec\u00e2t cel prezent dup\u0103 transfer pasiv3;4<\/sup>.<\/span><\/p>\n

Titrul minim protector nu este bine definit, dar este foarte mic: < 45 mUI\/mL6<\/sup>. <\/span>Anti-HAV-IgG transfera\u0163i de la mam\u0103 la f\u0103t transplacentar pot fi detecta\u0163i la copii, chiar \u015fi peste v\u00e2rsta de 1 an3<\/sup>. <\/span>Absen\u0163a anticorpilor anti-HAV totali exclude practic infec\u0163ia recent\u0103 cu VHA. <\/span>Profilaxia infec\u0163iei se realizeaz\u0103 prin vaccinare7<\/sup>.<\/span><\/p>\n

Recomand\u0103ri pentru determinarea anti-HAV<\/em><\/strong><\/span><\/p>\n

\u2022\u00a0 verificarea imunit\u0103\u0163ii fa\u0163\u0103 de VHA postinfec\u0163ie sau postvaccinare3<\/sup>.<\/span><\/p>\n

Preg\u0103tire pacient <\/em><\/strong>\u2013 \u00e0 jeun (pe nem\u00e2ncate)1<\/sup>.<\/span><\/p>\n

Specimen recoltat <\/em><\/strong>\u2013 s\u00e2nge venos1<\/sup>.<\/span><\/p>\n

Recipient de recoltare <\/em><\/strong>\u2013 vacutainer f\u0103r\u0103 anticoagulant cu\/f\u0103r\u0103 gel separator1<\/sup>.<\/span><\/p>\n

Prelucrare necesar\u0103 dup\u0103 recoltare <\/em><\/strong>\u2013 se separ\u0103 serul prin centrifugare; se lucreaz\u0103 imediat; dac\u0103 acest lucru nu este posibil, serul se p\u0103streaz\u0103 la 2\u00b0- 8\u00b0C sau la -20\u00b0C1<\/sup><\/span><\/p>\n

Stabilitate prob\u0103 <\/em><\/strong>\u2013 serul este stabil 7 zile<\/em> la 2-8\u00b0C; timp \u00eendelungat<\/em> la -20\u00b0C; evita\u0163i decongelarea\/recongelarea1<\/sup>.<\/span><\/p>\n

Metod\u0103 <\/em><\/strong>- imunochimic\u0103 cu detec\u0163ie prin electrochemiluminiscen\u0163\u0103 (ECLIA)<\/span>1<\/sup>.<\/span><\/p>\n

\u2022\u00a0 anti-HAV: < 20 UI\/L1<\/sup>.<\/span><\/p>\n

Limite \u015fi interferen\u0163e<\/em><\/strong><\/span><\/p>\n

Anticorpii anti-HAV totali nu fac distinc\u0163ia \u00eentre o infec\u0163ie recent\u0103 \u015fi una veche, pentru aceasta fiind necesar\u0103 determinarea anti-HAV-IgM. Efectuarea de teste seriale nu este de obicei indicat\u01037<\/sup>.<\/span><\/div>\n
Prezen\u0163a anticorpilor anti-HAV nu exclude hepatita B sau alte forme de hepatit\u0103 viral\u01032<\/sup>. <\/span>Pot s\u0103 apar\u0103 rezultate fals pozitive pentru anti-HAV-IgM, dac\u0103 anti-HAV-IgG sunt prezen\u0163i \u00een ser\u00a0\u00een concentra\u0163ii foarte mari6<\/sup>. <\/span>Foarte rar anti-HAV-IgM se pot dezvolta dup\u0103 vaccinarea anti hepatit\u0103 tip A5<\/sup>.<\/span><\/div>\n

\u2022\u00a0 Interferen\u0163e analitice<\/span><\/p>\n

Pot produce interferen\u0163e cu unele componente ale kit-ului \u015fi conduce la rezultate neconcludente urm\u0103toarele:<\/span><\/p>\n

-tratamentul cu biotin\u0103 \u00een doze mari (>5 mg\/zi); de aceea se recomand\u0103 ca recoltarea de s\u00e2nge s\u0103 se\u00a0<\/span>fac\u0103 dup\u0103 minimum 8 ore de la ultima administrare;<\/span><\/div>\n
-titrurile foarte crescute de anticorpi anti-streptavidin\u0103 \u015fi anti-ruteniu;<\/span><\/div>\n
-anticorpii monoclonali proveni\u0163i de la \u015foarece administra\u0163i la unii pacien\u0163i \u00een scop diagnostic sau terapeutic1<\/sup>.<\/span><\/div>\n
