Sinteza de ACTH are loc \u00een celulele bazofile adenohipofizare dintr-un precursor denumit proopiomelanocortin<sup>1;5<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: medium;\">ACTH controleaz\u0103 dezvoltarea \u015fi secre\u0163ia hormonal\u0103 a corticosuprarenalei. Prin stimularea zonei fasciculate a corticosuprarenalelor activeaz\u0103 sinteza \u015fi secre\u0163ia de glucocorticoizi (cortizol \u015fi corticosteron). Efectele stimulatoare ale ACTH asupra secre\u0163iei corticosuprarenalei sunt mediate de AMP ciclic<sup>1<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: medium;\">Propriet\u0103\u0163ile secundare ale ACTH se datoreaz\u0103 efectelor metabolice ale hormonilor glucocorticoizi, activand metabolismele glucidic, protidic \u015fi lipidic<sup>1<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: medium;\">Reglarea secre\u0163iei de ACTH se realizeaz\u0103 pe cale neuroumoral\u0103 cu participarea indirect\u0103 a hipotalamusului. Principalul factor umoral de reglare \u00eel constituie concentra\u0163ia sanguin\u0103 a hormonilor glucocorticoizi, \u00eendeosebi a cortizolului. Sub influen\u0163a acestora, secre\u0163ia de ACTH scade, printr-un mecanism de feedback negativ \u015fi, respectiv, cre\u015fte\u00a0\u00een cazul sc\u0103derii glucocorticoizilor din circula\u0163ie<sup>1<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: medium;\"><strong><em>Recomand\u0103ri pentru determinarea ACTH <\/em><\/strong>\u2013 diagnosticul diferen\u0163ial al sindromului Cushing, secre\u0163ia ectopic\u0103 de\u00a0ACTH (carcinom pulmonar cu celule mici, insule celulare tumorale de pancreas, tumori carcinoide, carcinom medular al tiroidei), boala Addison, hipopituitarismul \u015fi tumorile pituitare produc\u0103toare de ACTH (sindromul Nelson)<sup>3;5<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: medium;\"><strong><em>Preg\u0103tire pacient <\/em><\/strong><em>-<\/em>\u00e0 jeun (pe nem\u00e2ncate); pacientul trebuie s\u0103 nu fie stresat \u00een timpul recolt\u0103rii (stres-ul produce o cre\u015ftere a ACTH-ului plasmatic); exist\u0103 o varia\u0163ie diurn\u0103 a nivelului plasmatic de ACTH. Un examen complet prevede 2 recolt\u0103ri de s\u00e2nge pentru determinarea ACTH: a) \u00eentre orele 6-10; b) \u00eentre orele 21-24.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: medium;\">Valoarea ACTH \u00een proba de sear\u0103 reprezint\u0103 1\/2 sau 2\/3 din valoarea de diminea\u0163\u0103. Aceast\u0103 varia\u0163ie diurn\u0103 are semnifica\u0163ie diagnostic\u0103. De obicei se recolteaz\u0103 simultan \u015fi o prob\u0103 de s\u00e2nge pentru determinarea cortizolului<sup>2<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: medium;\"><strong><em>Specimen recoltat <\/em><\/strong><em>- <\/em>s\u00e2nge venos; vacutainerul se agit\u0103 foarte u\u015for, prin r\u0103sturnare \u015fi se pune imediat pe ghea\u0163\u0103; se transport\u0103 la laborator c\u00e2t mai repede posibil<sup>2<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: medium;\"><strong><em>Recipient de recoltare <\/em><\/strong><em>- <\/em>vacutainer cu EDTA K3, r\u0103cit \u00eenainte de utilizare (\u0163inut\u00a0\u00een frigider)<sup>2<\/sup>; se va evita recoltarea\u00a0\u00een tuburi nesiliconate (se ob\u0163in rezultate fals sc\u0103zute datorit\u0103 ader\u0103rii ACTH la sticl\u0103<sup>2<\/sup>).<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: medium;\"><strong><em>Cantitate recoltat\u0103 <\/em><\/strong><em>-<\/em>c\u00e2t permite vacuumul<sup>2<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: medium;\"><strong><em>Cauze de respingere a probei<\/em><\/strong> \u2013 specimen hemolizat, icteric, lipemic, heparinat, care nu a fost adus la laborator pe ghea\u0163\u0103 sau provenit de la pacien\u0163i care au primit izotopi radioactivi<sup>2<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: medium;\"><strong><em>Prelucrare necesar\u0103 dup\u0103 recoltare <\/em><\/strong><em>-<\/em>\u00een maxim 30 de minute de la recoltare se separ\u0103 plasma prin centrifugare (preferabil\u00a0\u00een centrifug\u0103 cu r\u0103cire la 2-8\u00b0C), 15 minute la 1500 x g [g = (1118 + 10<sup>-8<\/sup>) x (raza in cm) x (rpm)<sup>2<\/sup>].<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: medium;\">Pentru separarea plasmei se folose\u015fte o pipet\u0103 de plastic. Se lucreaz\u0103 imediat; dac\u0103 acest lucru nu este posibil, plasma se p\u0103streaz\u0103 la -20\u00b0C \u00een recipient de plastic (polipropilen sau polistiren); probele recoltate\u00a0\u00een afara sediilor laboratorului vor fi transportate\u00a0\u00een recipientul destinat probelor congelate<sup>2<\/sup><sup>;<\/sup><sup>3<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: medium;\"><strong><em>Stabilitate prob\u0103 <\/em><\/strong><em>- <\/em>plasma este stabil\u0103 <em>4 ore<\/em> la 2-8\u00b0C; <em>6 luni<\/em> la -20\u00b0C sau la -70\u00b0C; nu decongela\u0163i\/recongela\u0163i; rezultatul ob\u0163inut pe un e\u015fantion decongelat este cu 20-35% mai mic<sup>2<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\"><strong><em>Metod\u0103 <\/em><\/strong>\u2013 imunochimic\u0103 cu detec\u0163ie prin electrochemiluminiscen\u0163\u0103 (ECLIA)<sup>2<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\"><strong><em>Valori de referin\u0163\u0103 <\/em><\/strong><em>- <\/em>7.2-63.3 pg\/mL (1.6-13.9 pmol\/L); valorile au fost stabilite pentru probe recoltate \u00een intervalul 7-10 a.m<sup>2<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\"><em>Factori de conversie:\u00a0\u00a0 <\/em><em>pg\/mL x 0.2202 = pmol\/L<\/em><em>\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/em><em>pmol\/L x 4.541 = pg\/mL<\/em><\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\"><strong><em>Limita de detec<\/em><\/strong><strong><em>\u0163ie <\/em><\/strong>\u2013 1 pg\/mL (0.22 pmol\/L)<sup>2<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: medium;\"><strong><em>Interpretarea rezultatelor<\/em><\/strong><\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: medium;\">Termenul de sindrom Cushing se refer\u0103 la toate condi\u0163iile clinice asociate cu o produc\u0163ie excesiv\u0103 de cortizol; determinarea ACTH este util\u0103 pentru diagnosticul diferen\u0163ial al acestui sindrom:<\/span><\/p>\n<ul>\n<li style=\"text-align: justify;\"><span style=\"font-size: medium;\">boala Cushing: datorit\u0103 secre\u0163iei hipofizare excesive, nivelul ACTH este moderat crescut sau la limita superioar\u0103 a normalului (dar crescut fa\u0163\u0103 de nivelul cortizolului plasmatic), cu pierderea varia\u0163iei diurne normale;<\/span><\/li>\n<li style=\"text-align: justify;\">secre\u0163ia ectopic\u0103 de ACTH: nivelul ACTH este foarte crescut;<\/li>\n<li style=\"text-align: justify;\">adenom sau adenocarcinom suprarenalian: nivelul ACTH este foarte sc\u0103zut sau nedecelabil.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><span style=\"font-size: medium;\">Determinarea ACTH este de asemenea util\u0103 \u00een diagnosticul diferen\u0163ial al insuficien\u0163ei\u00a0 suprarenaliene:<\/span><\/p>\n<ul>\n<li style=\"text-align: justify;\"><span style=\"font-size: medium;\">boala Addison: datorit\u0103 distruc\u0163iei suprarenalei prin mecanism tumoral, infec\u0163ios sau autoimun, produc\u0163ia de cortizol este suprimat\u0103, iar nivelul ACTH este crescut;<\/span><\/li>\n<li style=\"text-align: justify;\">insuficien\u0163a suprarenalian\u0103 secundar\u0103 insuficien\u0163ei hipofizare: nivelurile ACTH \u015fi ale cortizolului sunt sc\u0103zute.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><span style=\"font-size: medium;\">Un nivel crescut de ACTH mai apare \u00een: sindromul adrenogenital \u015fi sindromul pseudo-Cushing (entitate clinic\u0103 reversibil\u0103 asociat\u0103 cu abuzul de alcool)<sup>3;5<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: medium;\"><strong><em>Limite \u015fi interferen\u0163e<\/em><\/strong><\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: medium;\">\u2022 Medicamente<\/span><\/p>\n<p style=\"text-align: justify; padding-left: 30px;\"><span style=\"font-size: medium;\"><span style=\"text-decoration: underline;\"><strong>Cre\u015fteri<\/strong><\/span>: aminoglutetimid, amfetamine, insulina, levodopa, metoclopramid, metirapon, vasopresina.<\/span><\/p>\n<p style=\"text-align: justify; padding-left: 30px;\"><span style=\"font-size: medium;\"><span style=\"text-decoration: underline;\"><strong>Sc\u0103deri<\/strong><\/span>: dexametazona \u015fi al\u0163i corticosteroizi<sup>4<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">\u2022 Interferen\u0163e analitice<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Pot produce interferen\u0163e cu unele componente ale kit-ului \u015fi conduce la rezultate neconcludente urm\u0103toarele:<\/span><\/p>\n<ul>\n<li style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">tratamentul cu biotin\u0103 \u00een doze mari (&gt;5 mg\/zi);
