De la nivelul ficatului, este transportat prin plasma la rinichi, unde este filtrat si excretat intr-un procent de aproximativ 70%. Restul de acid uric este eliminat si degradat in tractul gastrointestinal. Supraproductia de acid uric are loc in urmatoarele situatii: catabolism excesiv al acizilor nucleici (guta), produceri si distrugeri masive de celule (leucemie) sau incapacitate de excretie a produsului final (insuficienta renala)<sup>2<\/sup>.<\/p>\n<p>Hiperuricemia si hipouricemia nu constituie boli propriu-zise. In timp ce hiperuricemia este un factor de risc metabolic si poate sta la baza unor afectiuni, hipouricemia poate indica o conditie patologica neobservata<sup>6<\/sup>.<\/p>\n<p><strong><em>Recomandari pentru determinarea acidului uric seric<\/em><\/strong><\/p>\n<p style=\"padding-left: 30px;\">\u2022 evaluarea insuficientei renale;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 monitorizarea tratamentului gutei;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 monitorizarea tratamentului citostatic<sup>1;<\/sup><sup>3<\/sup>;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 determinari seriate in toxemia de sarcina pentru urmarirea raspunsului terapeutic<sup>1;3<\/sup>.<\/p>\n<p><strong><em>Pregatire pacient<\/em><\/strong><strong>\u00a0<\/strong><\/p>\n<p style=\"padding-left: 30px;\">\u2022 \u00e0 jeun (ideal) sau sa nu manance patru ore inainte de recoltare;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 exista variatii diurne \u2013 dimineata valoarea obtinuta pentru acidul uric seric este mai mare decat seara<sup>4<\/sup>.<\/p>\n<p><strong><em>Specimen recoltat <\/em><\/strong>\u2013 sange venos<sup>4;5<\/sup>.<strong>\u00a0<\/strong><\/p>\n<p><strong><em>Recipient de recoltare <\/em><\/strong>\u2013 vacutainer fara anticoagulant cu\/fara gel separator<sup>4<\/sup>.<\/p>\n<p><strong><em>Prelucrare necesara dupa recoltare <\/em><\/strong>\u2013 se separa serul prin centrifugare; se lucreaza in aceeasi zi; daca acest lucru nu este posibil, serul se poate stoca la 2-8\u00b0C sau la -20\u00b0C; recipientul\u00a0 in care este stocat serul trebuie sa fie inchis pentru a evita evaporarea si contaminarea<sup>4<\/sup>.<\/p>\n<p><strong><em>Volum proba <\/em><\/strong>\u2013 minim 0.5 mL ser<sup>4<\/sup>.<strong>\u00a0<\/strong><\/p>\n<p><strong><em>Stabilitate proba <\/em><\/strong>\u2013 serul separat este stabil\u00a0 <em>5-7 zile<\/em> la 2-8\u00b0 C ; <em>6 luni<\/em> la -18\u00b0C<sup>4<\/sup>.<\/p>\n<p><strong><em>Metoda <\/em><\/strong><em>- <\/em><span style=\"text-decoration: underline;\">spectrofotometrica (enzimatica colorimetrica)<\/span><sup>4<\/sup>.<\/p>\n<p><strong><em>Valori de referinta<\/em><\/strong><sup>4<\/sup><\/p>\n<p>Barbati: &lt; 7 mg\/dL<\/p>\n<p>Femei: &lt;5.7 mg\/dL.<\/p>\n<p><strong><em>Limita de dete<\/em><\/strong><strong><em>ctie <\/em><\/strong>\u2013 0.2 mg\/dL (11.9 \u03bcmol\/L)<sup>4<\/sup>.<\/p>\n<p><strong><em>\u00a0<\/em><\/strong><strong><em>Interpretarea rezultatelor <\/em><\/strong><\/p>\n<table class=\"wp-block-table w-100\" border=\"1\" cellspacing=\"0\" cellpadding=\"0\">\n<tbody>\n<tr>\n<td valign=\"top\" width=\"367\">\n<p style=\"text-align: center;\"><strong><em>Cresterea acidului uric seric<\/em><\/strong><\/p>\n<\/td>\n<td valign=\"top\" width=\"252\">\n<p style=\"text-align: center;\"><strong><em>Scaderea acidului uric seric<\/em><\/strong><\/p>\n<\/td>\n<\/tr>\n<tr>\n<td valign=\"top\" width=\"367\">\n<p style=\"text-align: center;\">\u2022 insuficienta renala; \u2022 guta; \u2022 hiperuricemie asimptomatica (trebuie evaluata periodic pentru guta); \u2022 cresterea distructiei de nucleoproteine: leucemii si sindroame mieloproliferative cronice, mielom multiplu, limfoame (mai ales postiradiere), alte neoplasme diseminate, chimioterapie antineoplazica, anemie hemolitica, anemie pernicioasa, toxemie de sarcina, psoriazis (1\/3din pacienti); \u2022 administrarea unor medicamente: doze mici de salicilati (&lt;4g\/zi), intoxicatii (barbiturice, alcool metilic, amoniac, monoxid de carbon), substante care determina scaderea clearance-ului renal sau a secretiei tubulare (tiazide, furosemid), nefrotoxice (mitomicina C); \u2022 acidoza metabolica, cetoacidoza diabetica;\u00a0 hipertrigliceridemie (80% din pacienti);<\/p>\n<p style=\"text-align: center;\">\u2022 regim alimentar: dieta cu scaderea proteinelor cu greutate moleculara mare, nucleoproteine in exces, consumul de alcool; \u2022 cauze diverse: intoxicatie cu plumb, boala von Gierke, sindrom Lesch- Nyhan (guta ereditara), sindrom Down, rinichi polichistic, hipoparatiroidism, hiperparatiroidism primar, hipotiroidism, sarcoidoza<sup>2;<\/sup><sup>3;6<\/sup>.<\/p>\n<\/td>\n<td valign=\"top\" width=\"252\">\n<p style=\"text-align: center;\">\u2022 administrarea unor medicamente: ACTH,\u00a0 uricozurice (cortizon, alopurinol);<\/p>\n<p style=\"text-align: center;\">\u00a0\u2022cauze diverse: boala Wilson, sindrom Fanconi, SIADH, acromegalie (unii pacienti), boala celiaca (crestere usoara), xantinurie (deficitul de xantin oxidaza), unele neoplasme (ex.: boala Hodgkin), adulti sanatosi cu defect izolat de transport tubular al acidului uric (mutatia \u201ecainelui dalmatian\u201d)<sup>2;<\/sup><sup>3;6<\/sup>.<\/p>\n<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p><strong><em>Limite si interferente<\/em><\/strong><strong>\u00a0<\/strong><\/p>\n<p>Se pot inregistra variatii ale acidului uric de la o zi la alta sau de la un anotimp la altul, la aceeasi persoana. Cresterea acidului uric este cauzata de stresul emotional, exercitiul fizic intens, postul negru, obezitate. <strong>\u00a0<\/strong><\/p>\n<p>S-au constatat scaderi ale acidului uric la 5% dintre pacientii spitalizati, cauza fiind statusul postoperator<sup>3<\/sup>.<\/p>\n<p>\u2022 Medicamente<\/p>\n<p style=\"padding-left: 30px;\"><span style=\"text-decoration: underline;\"><strong>Cresteri<\/strong><\/span>: acid nicotinic (doze mari), blocanti betaadrenergici (atenolol, propanolol, nadolol, timolol), corticosteroizi (in leucemii acute), ciclosporina, diazoxid, diuretice (acetazolamida, clortalidon, acid etacrinic, furosemid, tiazide, triamteren), epinefrina, etanol, etambutol, fenotiazine, norepinefrina, pirazinamida, salicilati (doze mici), unele antineoplazice (asparaginaza, cisplatin, clorambucil, fludarabina, hidroxiuree, idarubicin, mecloretamina, vincristin), teofilina<sup>1<\/sup>.<\/p>\n<p style=\"padding-left: 30px;\"><span style=\"text-decoration: underline;\"><strong>Scaderi<\/strong><\/span>: acid ascorbic, alfametildopa, alopurinol, aspirina, desferoxamin, corticosteroizi, dietilstilbestrol, enalapril, ibuprofen, indometacin, manitol, probenecid, spironolactona, verapamil<sup>1<\/sup>.<\/p>\n<p>\u2022 Interferente analitice<\/p>\n<p>Nivelurile ridicate ale trigliceridelor (2000 mg\/dl) duc la cresterea acidului uric<sup>4<\/sup>.<\/p>\n<p><span style=\"font-size: small;\">\u00a0<\/span><\/p>\n<p><span style=\"font-size: small;\">\u0411\u0438\u0431\u043b\u0438\u043e\u0433\u0440\u0430\u0444\u0438\u044f<\/span><\/p>\n<p><span style=\"font-size: small;\">1.
