Sinteza\u00a0 celor 3 tipuri de hormoni\u00a0 porne\u015fte de la un precursor comun (pregnenolon). 17-hidroxiprogesteron reprezint\u0103 substratul pentru procesele de hidroxilare \u00een pozi\u0163iile 21 \u015fi 11 \u00een etapa de sintez\u0103 a cortizolului, un rol critic av\u00e2ndu-l enzimele 21-hidroxilaza \u015fi 11-beta-hidroxilaza. Dac\u0103 hidroxilarea nu poate avea loc datorit\u0103 unui deficit enzimatic, sinteza cortizolului se reduce foarte mult, fiind \u00eenso\u0163it\u0103 de cre\u015fterea ACTH<sup>1<\/sup>.<\/p>\n<p>Hiperplazia adrenal\u0103 congenital\u0103 sau sindromul adrenogenital include un grup de afec\u0163iuni caracterizate printr-o anomalie ereditar\u0103 \u00een sinteza cortizolului, transmis\u0103 autozomal recesiv, asociat\u0103 de obicei cu o cre\u015ftere a produc\u0163iei de androgeni. Cea mai frecvent\u0103 cauz\u0103 de sindrom adrenogenital (prezent\u0103 \u00een 95% din cazuri) este deficitul de 21-hidroxilaz\u0103. Datorit\u0103 perturb\u0103rii sintezei cortizolului, metabolismul steroizilor este redirec\u0163ionat c\u0103tre sinteza de androgeni; clinic apar semne de virilizare la nou-n\u0103scutul de sex feminin (hermafroditism) \u015fi pubertate precoce izosexual\u0103 la b\u0103ie\u0163i. Adeseori se asociaz\u0103 \u015fi deficitul de aldosteron. \u00cen peste 50% din cazuri debutul formei homozigote a bolii se \u00eenregistreaz\u0103 \u00een primele 3 s\u0103pt\u0103m\u00e2ni de via\u0163\u0103. Deficitul de 21-hidroxilaz\u0103 determin\u0103 acumularea de 17-hidroxiprogesteron, care \u00een mod normal este metabolizat, sub ac\u0163iunea acestei enzime, \u00een 11-deoxicortizol. Este posibil \u015fi un diagnostic prenatal al fe\u0163ilor afecta\u0163i, prin detectarea unor concentra\u0163ii crescute de 17-hidroxiprogesteron \u00een lichidul amniotic, \u00een s\u0103pt\u0103m\u00e2nile 14-16 de sarcin\u0103.<\/p>\n<p>Pe l\u00e2ng\u0103 sindromul adrenogenital clasic, mai exist\u0103 deficitul de 21-hidroxilaz\u0103 cu debut tardiv \u015fi forma heterozigot\u0103 de hiperplazie adrenal\u0103, care se manifest\u0103 clinic \u00een perioada peripubertar\u0103 sub form\u0103 de tulbur\u0103ri menstruale \u015fi hirsutism sau direct la adult sub form\u0103 de hirsutism. \u00cen aceste cazuri, nivelul bazal de 17-hidroxiprogesteron poate fi normal sau u\u015for crescut, de aceea se va efectua un test de stimulare cu ACTH.
