Se estimeaz\u0103 c\u0103 80% dintre decesele de cauz\u0103 cardiovascular\u0103 pot fi prevenite. Detectarea \u0219i interven\u021bia individual\u0103 asupra factorilor \u0219i comportamentelor cu risc de BCV de-a lungul copil\u0103riei \u0219i adolescen\u021bei a fost recomandata ca strategie de reducere a riscului de BCV la v\u00e2rsta adult\u01031.<\/p>\n<p>Lipidele sunt cel mai frecvent masurate pentru a evalua riscul cardiovascular. O condi\u021bie important\u0103 pentru a aborda provoc\u0103rile prevenirii evenimentelor cardiovasculare este armonizarea\/standardizarea testelor ce evalueaza statusul lipidic al pacientilor. Mentinerea concentratiilor dorite de lipide scade riscul de infarct miocardic sau accident vascular cerebral.<\/p>\n<p>\u00cen ciuda dovezilor stiintifice care confirma eficienta tratamentului hipolipemiant ce reduce valoarea LDL-colesterol (LDL-C) \u00een popula\u021bie, multe persoane experimenteaz\u0103 evenimente cardiovasculare sau progresia aterosclerozei, chiar daca au valori ale LDL-C &lt;180mg\/dL1;2;3.<\/p>\n<p>\u00cen noua er\u0103 a concentra\u021biilor foarte sc\u0103zute de LDL-colesterol, realizabile cu ajutorul medicamentelor hipolipemiante de ultima generatie, evaluarea riscului aterosclerotic rezidual de boli cardiovasculare se face folosind si alti biomarkeri decat LDL \u2013 colesterol.<\/p>\n<p>non-HDL colesterol (non-HDL-C) reprezint\u0103 colesterolul cuprins in toate particulele aterogene din plasma: LDL, VLDL, IDL \u0219i Lp (a) si ofer\u0103 o evaluare mai cuprinz\u0103toare a riscului cardiovascular dec\u00e2t LDL-C la indivizii cu hipertrigliceridemie deoarece adaug\u0103 si colesterolul remanent la LDL-C \u0219i, prin urmare, \u021bine cont de poten\u021bialul aterogen al particulelor de lipoproteine.<\/p>\n<p>Studiile au aratat ca masurarea non-HDL-colesterolului ar fi mai benefica pentru terapie, in special in cazul persoanelor cu un nivel al trigliceridelor &gt; 200mg\/ dL1;2;3.<\/p>\n<p><strong>Recomandari pentru determinarea Colesterol non-HDL: <\/strong><\/p>\n<ul>\n<li>Investiga\u021bie de baz\u0103 \u00een evaluarea riscului de boal\u0103 cardiovascular\u0103 (BCV)\n<ul>\n<li>Recomandat ca <strong>test de screening\u00a0 <\/strong>(la pacien\u021bi f\u0103r\u0103 factori de risc BCV): copii \u0219i adolescen\u021bi: o testare \u00een intervalul de v\u00e2rst\u0103 9-11 ani \u0219i ulterior \u00eentre 17-21 ani<\/li>\n<li>adul\u021bi: la fiecare 4-6 ani<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<ul>\n<li>Recomandat ca <strong>test de monitorizare <\/strong>(prin acelea\u0219i metode ca \u0219i la screening) la intervale regulate: persoane cu factori de risc BCV\n<ul>\n<li>persoane depistate anterior cu valori \u2191 ale lipidelor<\/li>\n<li>persoane cu tratament hipolipemiant<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>Exemplu: la persoanele cu diabet zaharat (DZ) \u2013 evaluarea cel pu\u021bin o dat\u0103 pe an a profilului lipidic standard.<\/p>\n<p><strong>Specimen recoltat <\/strong>\u2013 s\u00e2nge venos.<\/p>\n<p><strong>Recipient de recoltare <\/strong>\u2013 vacutainer fara anticoagulant, cu\/fara gel separator4.<\/p>\n<p><strong>Prelucrare necesara dupa recoltare <\/strong>\u2013 se separa serul prin centrifugare; se lucreaza in aceeasi zi sau se stocheaza la 4\u00baC sau la -20\u00baC4.<\/p>\n<p><strong>Volum proba <\/strong>\u2013 minim 0.5 mL ser4.<\/p>\n<p><strong>Cauze de respingere a probei <\/strong>\u2013 specimen intens hemolizat \/ sange recoltat interprandial4.<\/p>\n<p><strong>Stabilitate proba <\/strong>\u2013 serul separat este stabil 5-7 zile la 2-8\u00b0C; 3 luni la -20\u00baC.<\/p>\n<p><strong>Metoda <\/strong>\u2013 test calculat4.<\/p>\n<p><strong>Interpretarea rezultatelor Colesterol non-HDL<\/strong><\/p>\n<p>Diagnosticul modern de laborator al afec\u021biunilor lipidice \u0219i al riscului cardiovascular ar trebui s\u0103 se bazeze pe utilizarea indicatorilor care prezint\u0103 impactul complet al tuturor componentelor plasmatice ale lipidelor implicate \u00een ateroogenez\u0103. Non-HDL-C este suma colesterolului acumulat \u00een toate lipoproteinele, cu excep\u021bia HDL, cum ar fi: chilomicronii, VLDL \u0219i resturile lor, IDL, LDL \u0219i Lp (a).
