Cresterea incidentei acestuia este atribuita cresterii prevalentei bolilor hepatice cronice. Desi acest tip de tumor\u0103 poate fi tratat\u0103 eficient \u00een stadii precoce, cei mai mul\u021bi pacien\u021bi sunt diagnostica\u021bi atunci c\u00e2nd devin simptomatici, \u00een stadii tardive de boal\u0103, c\u00e2nd resursele terapeutice sunt limitate.<\/p>\n<p>Factorii de risc majori pentru dezvoltarea hepatocarcinomului sunt: consumul cronic de alcool, hepatitele virale cronice B \u0219i C \u0219i steatoza hepatic\u0103 non-alcoolic\u0103. Alte cauze mai rare sunt reprezentate de boala Wilson, hemocromatoza ereditar\u0103, deficien\u021ba de alfa-1 antitripsin\u0103, colangita biliar\u0103 primitiv\u0103 \u0219i hepatita autoimun\u0103. Aproximativ 50% din cazurile de carcinom hepatocelular sunt asociate cu hepatita cronic\u0103 B, iar 25% cu hepatita cronic\u0103 C. Detectarea precoce a CHC este important\u0103, \u00een special \u00een cazul pacientilor cu risc crescut.<\/p>\n<p><strong>Scorul GALAD <\/strong>este un model statistic bazat pe biomarkeri serici: <strong>AFP, AFP-L3, DCP (PIVKA II)<\/strong>, pentru estimarea probabilitatii de a dezvolta carcinom hepatocelular (HCC) la pacien\u021bii cu boli hepatice cronice.<\/p>\n<h5>AFP (Alfa-Feto-Proteina)<\/h5>\n<p><strong>\u00a0<\/strong><strong>AFP (Alfa-Feto-Proteina) <\/strong>reprezint\u0103 biomarkerul standard pentru evaluarea pacien\u021bilor cu suspiciune de hepatocarcinom, \u00eens\u0103 acesta are o specificitate sc\u0103zut\u0103. Niveluri crescute de AFP sunt \u00eenregistrate at\u00e2t \u00een afectiuni hepatice (hepatite acute, cronice, ciroz\u0103 \u0219i carcinom hepatocellular \u2013 CHC), c\u00e2t \u0219i \u00een alte tipuri de neoplazii (tumori gastrice, pulmonare, pancreatice, de tract biliar, carcinoame testiculare, tumori cu celule germinale). De asemenea, AFP are o sensibilitate redus\u0103 \u00een identificarea tumorilor mici, cu poten\u021bial curabil. Doar nivelurile serice de AFP persistent crescute &gt;200 ng\/mL sunt \u00eenalt sugestive pentru carcinomul hepatocelular.<\/p>\n<p>De asemenea, cre\u0219terea AFP poate fi adesea observat\u0103 la pacien\u021bi cu boli hepatice cronice care nu au progresat catre CHC. Acest lucru limiteaza utilitatea determinarii AFP ca indicator timpuriu al progresiei catre CHC la ace\u0219ti pacien\u021bi.<\/p>\n<p>AFP este o glicoprotein\u0103 cu un singur situs de glicozilare la un reziduu specific, de arginin\u0103. In ser apar trei variante de glicozilare, care pot fi diferen\u021biate pe baza leg\u0103rii lor relative la o lectin\u0103 izolat\u0103 din semin\u021be de linte. Aceast\u0103 lectin\u0103, denumit\u0103 <em>Lens culinaris agglutinin <\/em>(LCA), se leag\u0103 de variantele AFP-L1, AFP-L2 \u0219i AFP-L3 cu afinitate cresc\u00e2nd\u0103. <strong>AFP-L1<\/strong>, care are cea mai slab\u0103 legare de LCA, este frac\u021bia predominant\u0103 la pacien\u021bii cu boli hepatice benigne, cum ar fi hepatita cronic\u0103 \u0219i ciroz\u0103 hepatic\u0103.<\/p>\n<h5>AFP-L3% \u2013 alfa-feto proteina lentil lectin-reactiva%<\/h5>\n<p><strong>\u00a0<\/strong>Alfa-fetoproteina (AFP) este glicozilat\u0103 diferit \u00een diverse afec\u021biuni hepatice. Prin tehnica electroforezei pot fi identificate diferite forme glicozilate de AFP, pe baza reactivit\u0103\u021bii acestora fa\u021b\u0103 de lectinele din plante, ce prezint\u0103 capacitatea de a lega zaharuri. Forma fucozilat\u0103 de AFP, care este cel mai str\u00e2ns asociat\u0103 cu hepatocarcinomul, este recunoscut\u0103 de c\u0103tre o lectin\u0103 derivat\u0103 dinlintea comun\u0103 (<em>Lens culinaris<\/em>). Aceast\u0103 form\u0103 fucozilat\u0103 este denumit\u0103 AFP-L3 (cea de-a treia variant\u0103 electroforetic\u0103 de AFP cu reactivitate fa\u021b\u0103 de lectina din linte) \u0219i detectarea acesteia prezint\u0103 utilitate \u00een evaluarea pacien\u021bilor cu niveluri serice AFP \u2264 200 ng\/mL. \u00centr-un studiu multicentric recent, fractia AFP-L 3 crescuta (\u226510%) s-a dovedit a fi asociata cu o cre\u0219tere de 7 ori a riscului de a dezvolta CHC \u00een urm\u0103toarele 21 de luni.<\/p>\n<p>AFP-L3 poate fi utilizat \u00een asociere cu ecografia hepatica de \u00eenalt\u0103 rezolu\u021bie pentru:<\/p>\n<ul>\n<li>diferen\u021bierea hepatocarcinomului de afec\u021biunile hepatice cronice benigne;<\/li>\n<li>monitorizarea pacien\u021bilor cu ciroz\u0103 hepatica de diverse cauze;<\/li>\n<li>monitorizarea indivizlor cu istoric familial pozitiv de hepatocarcinom<\/li>\n<li>monitorizarea pacien\u021bilor cu infec\u021bie cronic\u0103 cu virusul hepatitei B dob\u00e2ndit\u0103 la v\u00e2rst\u0103 t\u00e2n\u0103r\u0103.<\/li>\n<\/ul>\n<h5>DCP (PIVKA II)<\/h5>\n<p><strong>\u00a0<\/strong>Des-gama-carboxi-protrombina (DCP) reprezint\u0103 o form\u0103 anormal\u0103, nefunc\u021bional\u0103 de factor II al coagul\u0103rii \u2013 protrombin\u0103 \u2013 cauzat\u0103 de lipsa carboxil\u0103rii reziduurilor de acid glutamic. Este denumit\u0103 alternativ \u0219i proteina indus\u0103 \u00een absen\u021ba vitaminei K sau antagonistul II (PIVKA-II). Carboxilaza dependent\u0103 de vitamina K necesar\u0103 pentru carboxilarea protrombinei este absent\u0103 \u00een multe celule maligne din carcinomul hepatocelular, fiind explicat\u0103 astfel sinteza variantei necarboxilate DCP \u00een conentra\u021bii crescute.<\/p>\n<p>Este considerata un biomarker complementar alfa-feto proteinei (AFP) si alfa-feto proteinei lentil lectin-reactive% (AFP-L3%) pentru evaluarea riscului de a dezvolta un carcinom hepatocelular (HCC). Progresia carcinomului hepatocelular (HCC) este indicata de cresterea atat a AFP-L3% cat si a DCP, desi fiecare dintre acesti doi biomarkeri reflecta aspecte evolutive diferite. Intr-un studiu prospectiv efectuat pe pacienti din Statele Unite, cu diagnostic stabilit de HCC, sensibilitatea testarilor pentru AFP, AFP-L3% si DCP a fost 68%, 62% si, respectiv, 73%. Cand cei trei markeri au fost combinati, sensibilitatea a fost 86%. Intr-un alt studiu, s-a demonstrat ca nivelurile de DCP se coreleaza cu marimea tumorii si prezenta metastazelor. In acest din urma studiu, DCP a avut cea mai mare sensibilitate (87%), comparativ cu AFP si cu AFP-L3%, si cea mai mare valoare predictiva pozitiva (87%) la pacientii cu HCC cauzat de hepatite cronice din infectia cu virusurile hepatice B si C. Un numar de studii au aratat ca nivelurile serice crescute de DCP sunt in relatie directa cu prezenta invaziei venei porte sau a metastazelor intrahepatice, conditii care vor afecta semnificativ prognosticul pentru pacientii cu HCC.<\/p>\n<p>Valorile DCP pot fi crescute si in alte situatii, in absenta HCC: icter obstructiv, colestaza intrahepatica ce determina scaderea cronica a vitaminei K, administrarea de medicamente ca Warfarina sau antibiotice cu spectru larg. In plus 25% pana la 50% dintre pacientii cu HCC vor avea valori ale DCP in limitele de referinta. Din aceasta cauza, o valoare normala a DCP nu exclude HCC.<\/p>\n<p>Prin combinarea tuturor markerilor mentionati cu varsta si sexul pacientului, scorul GALAD s-a dovedit a fi extrem de precis pentru detectarea HCC, performanta sa fiind validata si recunoscuta, in comparatie cu utilizarea biomarkerilor singuri sau impreuna.<\/p>\n<p><strong><em>Specimen recoltat <\/em><\/strong>\u2013 s\u00e2nge venos.<\/p>\n<p><strong><em>Recipient de recoltare <\/em><\/strong><strong>- <\/strong>vacutainer f\u0103r\u0103 anticoagulant, cu\/f\u0103r\u0103 gel separator.<\/p>\n<p><strong><em>Prelucrare necesar\u0103 dup\u0103 recoltare <\/em><\/strong>\u2013 se separ\u0103 serul prin centrifugare.<\/p>\n<p><strong><em>Cauze de respingere a probei <\/em><\/strong>\u2013 specimen intens hemolizat.<\/p>\n<p><strong><em>Stabilitate prob\u0103 <\/em><\/strong>\u2013 serul este stabil 7 zile la 2-8\u00baC, 3 luni la -20 \u00baC<\/p>\n<p><strong><em>Metoda <\/em>- <\/strong>calculul scorulului GALAD se bazeaza pe urmatoarea ecuatie:<\/p>\n<p><em>Z = -10.08 0.09 x varsta + 1.67 x sex + 2.34 log (10) (AFP)0.04 X AFP-L3 1.33x log (10) (DCP)<\/em><\/p>\n<p>unde sexul = 1 pentru barbati si 0 pentru femei.<\/p>\n<p>Estimarea probabilitatii se calculeaza dupa Pr (HCC) = exp(Z) (1 exp[Z]). <a href=\"https:\/\/www.mdcalc.com\/galad-model-hepatocellular-carcinoma-hcc\" target=\"_blank\" rel=\"noopener nofollow external noreferrer\" data-wpel-link=\"external\">https:\/\/www.mdcalc.com\/galad-model-hepatocellular-carcinoma-hcc<\/a><\/p>\n<h5>Interpretare rezultate:<\/h5>\n<p>Scorul combina cei trei biomarkeri serologici (AFP, AFP-L3 si DCP) cu varsta si sexul pacientului pentru a estima probabilitatea de aparitie a CHC la pacientii cu boli hepatice cronice (GALAD vine de la: <strong>\u0416 <\/strong>- <strong>\u0416<\/strong>ender\/sexul\u00a0 <strong>\u0410<\/strong>\u2013 Age\/varsta\u00a0\u00a0 <strong>\u041c <\/strong>-AFP L3\u00a0\u00a0 <strong>\u0410 <\/strong>-AFP\/ Alfafetoproteina\u00a0 <strong>\u0413<\/strong>\u2013 DCP Descarboxi-protrombina).<\/p>\n<p>Caracteristicile de performan\u021b\u0103 (sensibilitate \u0219i specificitate) ale scorului GALAD pentru HCC sunt influen\u021bate alegerea cut-off-ului. Astfel la o valoare de 0.76, scorul GALAD a avut 91% sensibilitate \u0219i 85% specificitate pentru detectarea HCC, iar pentru o valoare de 0.88, sensibilitatea observat\u0103 a fost de 80%, iar specificitatea de 97%. Aceste caracteristici au fost validate in studii ce au inclus cohorte mari de pacien\u021bi cu o prevalen\u021b\u0103 a HCC ce variaza intre 35 \u2013 49%.
