Acest proces recurent, poate duce la distrugerea total\u0103 a pancreasului, cu insuficien\u021b\u0103 pancreatic\u0103 ulterioar\u0103, diabet secundar, risc crescut de\u00a0cancer pancreatic\u00a0\u0219i sindrom dureros sever. Pancreatita ereditar\u0103 este form\u0103 cu debut precoce a pancreatitei cronice cu transmitere autosomal dominant\u0103 \u0219i penetran\u021b\u0103 variabil\u0103 (<em>LaRusch et al., 2012<\/em>).<\/p>\n<p>Boal\u0103 apare de obicei la mai mult de doi membri ai unei familii sau la mai multe genera\u021b\u00eei ale aceleia\u0219i familii. De precizat este faptul c\u0103 \u00een aceste cazuri agentul etiologic care declan\u0219eaz\u0103 episoadele de pancreatit\u0103 acut\u0103 nu poate fi identificat. Manifest\u0103rile clinice ale formei ereditare a pancreatitei variaz\u0103 de la forme u\u0219oare cu evolu\u021bie favorabil\u0103 p\u00e2n\u0103 la forme severe cu prognostic grav.<\/p>\n<p>Dup\u0103 descrierea corela\u021biei dintre pancreatita ereditar\u0103 \u0219i apari\u021bia unei muta\u021bii la nivelul bra\u021bului lung al cromozomului 7, studiile genetice au dus la eviden\u021bierea altor defecte responsabile de inhibarea activit\u0103\u021bii tripsinogenului, reglarea func\u021biei pancreatice \u0219i modularea procesului inflamator panceratic. S-au identificat o serie de gene \u00een care apar muta\u021bii ce pot determina apari\u021bia unei forme ereditare a pancreatitei:<\/p>\n<ul>\n<li>muta\u021bii \u00een gena care codific\u0103 tripsinogenul cationic (PRSS1);<\/li>\n<li>muta\u021bii \u00een gena care codific\u0103 inhibitorul serin-proteazic al tripsinei (SPINK1);<\/li>\n<li>muta\u021bii la nivelul genei fibrozei chistice (CFTR);<\/li>\n<li>muta\u021bii la nivelul genei CaSR (Calcium-sensing receptor);<\/li>\n<li>muta\u021bii la nivelul genei CPA-1 (Carboxipeptidaza A1);<\/li>\n<li>muta\u021bii la nivelul genei CTRC (Chimotripsin\u0103 C).<\/li>\n<\/ul>\n<p><b><strong>Recomand\u0103ri \u2013 testarea genetic\u0103<\/strong><\/b><\/p>\n<ul>\n<li>pacien\u021bii care documenteaz\u0103 dou\u0103 sau mai multe atacuri de pancreatit\u0103 acut\u0103 de etiologie neprecizat\u0103;<\/li>\n<li>diagnostic de pancreatit\u0103 cronic\u0103 idiopatic\u0103;<\/li>\n<li>istoric familial de pancreatita acut\u0103 de etiologie neprecizat\u00e3, la rude de gradul I sau II;<\/li>\n<li>pancreatita acut\u0103 de etiologie neprecizat\u0103, la copil, care necesit\u0103 spitalizare\u00a0<sup>[1;3].<\/sup><\/li>\n<\/ul>\n<p>Diagnosticul de certitudine se stabile\u0219te prin secven\u021biere genic\u0103.<\/p>\n<p>Prin consilierea genetic\u0103 extins\u0103, se poate oferi un sfat genetic de specialitate, \u00een care se alege \u00eempreun\u0103 cu medicul genetician o solu\u021bie adecvat\u0103 de testare genetic\u0103.<\/p>\n<p>Un alt aspect important \u00eel reprezint\u0103 faptul c\u0103 astfel de teste nu sunt utile pentru estimarea v\u00e2rstei de debut, severit\u0103\u021bii, tipului de simptome sau modalit\u0103\u021bii de progresie a bolii. Deoarece penetrant\u0103 muta\u021biilor genei PRSS1 este incomplet\u0103, manifest\u0103rile clinice ale bolii sunt variabile \u00een majoritatea familiilor, de aceea diagnosticul prenatal nu este \u00eencurajat.<\/p>\n<p>Diagnosticul prenatal la gravidele cu risc crescut pentru o muta\u021bie PRSS1 este posibil prin analiz\u0103 ADN-ul extras din celule fetale ob\u021binute prin amniocenteza, efectuat\u0103 de obicei la aproximativ 15-18 s\u0103pt\u0103m\u00e2ni de gesta\u021bie sau biopsia vilozit\u0103\u021bilor coriale, la aproximativ 10-12 s\u0103pt\u0103m\u00e2ni de gesta\u021bie. Muta\u021biile care stau la originea acestei patologii trebuie s\u0103 fie identificate \u00eenainte de testarea prenatal\u0103, la un membru afectat, din familie\u00a0<sup>[1;3;4].<\/sup><\/p>\n<p><b><strong>Condi<\/strong><\/b><b><strong>\u021b<\/strong><\/b><b><strong>\u00eei de recoltare:<\/strong><\/b><\/p>\n<p><strong><em>Preg\u0103tire pacient<\/em><\/strong>\u00a0\u2013 nu este necesara o preg\u0103tire special\u0103.<\/p>\n<p><strong><em>Specimen recoltat<\/em>\u00a0<\/strong>\u2013 s\u00e2nge venos.<\/p>\n<p><strong><em>Recipient de recoltare<\/em><\/strong>\u00a0\u2013 vacutainer ce con\u021bine EDTA ca anticoagulant.<\/p>\n<p><strong><em>Cauze de respingere a probei<\/em><\/strong>\u00a0\u2013 folosirea heparinei ca anticoagulant; probe coagulate sau hemolizate.<\/p>\n<p><strong><em>Stabilitate prob\u0103<\/em><\/strong>\u00a0\u2013 7 zile la 2-8\u00baC.<\/p>\n<p><b><strong>Raportarea \u0219i interpretarea rezultatelor<\/strong><\/b><\/p>\n<p>Un rezultat negativ este definit prin absen\u021ba muta\u021biilor \u00een genele evaluate.<\/p>\n<p>Muta\u021biile R122H, N29I \u0219i A16V din gena PRSS1 sunt responsabile de peste 90% dintre cazurile de pancreatita ereditar\u0103. Persoanele cu un rezultat pozitiv pentru muta\u021biile majore \u00een cadrul acestor gene trebuie consiliate cu privire la modul de transmitere autozomal-dominant, penetrant\u0103 incomplet\u0103, evolu\u021bia clinic\u0103 variabil\u0103 \u0219i strategiile de preven\u021bie a episoadelor de pancreatit\u0103 acut\u0103, evit\u00e2nd factorii de risc asocia\u021bi: alcool, medicamente, tulbur\u0103ri metabolice.<\/p>\n<p>Pacien\u021bii cu genotip predispozant la pancreatit\u0103 ereditar\u0103 trebuie evalua\u021bi radiologic \u0219i endoscopic pentru a identifica \u0219i trata eventualii factori de risc, \u00een special litiaza coledociana \u0219i al\u021bi factori obstructivi, care pot contribui la declan\u0219area atacurilor de pancreatita acut\u0103. Av\u00e2nd \u00een vedere riscul de cancer pancreatic la pacien\u021bii purt\u0103tori ai muta\u021biilor R122H sau N29I ace\u0219tia trebuie s\u0103 fie consilia\u021bi s\u0103 renun\u021be la fumat.<\/p>\n<p>Diferitele muta\u021bii ap\u0103rute \u00een gena SPINK1 pot conduce la modele de transmitere variabile. \u00cen stare homozigota, aceast\u0103 muta\u021bie poate determina pancreatita, \u00een timp ce la heterozigo\u021bi poate cauza doar o reducere subliminal\u0103 a activit\u0103\u021bii genei SPINK1, declan\u0219area bolii necesit\u00e2nd co-existen\u021ba unor factori exogeni sau endogeni asocia\u021bi\u00a0<sup>[3]<\/sup>.
